본 연구는 말더듬 성인을 대상으로 이들의 주관적 말더듬 심각도가 삶의 질에 미치는 영향을 살펴보고, 그 과정에서 지각된 사회적 지지의 매개효과와 말더듬에 대한 부정적 신념의 조절된 매개효과를 검증하는 것을 목표로 한다. 말더듬은 단순한 유창성 장애를 넘어 부정적 정서와 인지적 왜곡을 동반하며 개인의 삶 전반에 광범위한 영향을 미치는 복합적인 문제이다. 그러나 국내 선행연구들은 주로 아동을 대상으로 하거나 외현적인 말더듬 빈도 감소에 초점을 맞추어 왔으며, 말더듬 성인의 심리내적 기제와 삶의 질에 관한 통합적 연구는 부족한 실정이다. 이에 본 연구는 말더듬 성인이 지각하는 주관적 말더듬 심각도가 삶의 질로 이어지는 구체적인 경로를 규명하고, 이 과정에서 인지적 요인인 부정적 신념이 어떠한 보호적 혹은 위험 요인으로 작용하는지 파악하고자 한다.
이를 위해 온라인 커뮤니티를 통해 모집한 말더듬 성인 150명을 대상으로 한국판 주관적 말더듬 중증도 선별검사, 말더듬에 대한 비합리적 생각과 신념 척도 단축형, 다차원적 지각된 사회적 지지 척도, 한국판 세계보건기구 삶의 질 척도를 사용하여 설문조사를 실시하였고, 수집된 자료는 SPSS 27.0과 PROCESS Macro를 사용하여 기술통계, 상관분석, 매개효과, 조절된 매개효과 분석을 수행하였다.
분석 결과, 첫째, 지각된 사회적 지지는 삶의 질에 유의한 정적 영향을 미치는 것으로 나타났으며, 주관적 말더듬 심각도 역시 삶의 질에 유의한 부적 영향을 미치는 것으로 확인되었다. 둘째, 주관적 말더듬 심각도가 지각된 사회적 지지에 미치는 영향은 말더듬에 대한 부정적 신념 수준에 따라 유의하게 조절되었다. 구체적으로, 부정적 신념이 낮은 집단에서는 말더듬 심각도가 높을수록 사회적 지지가 유의하게 증가하는 정적 효과가 나타난 반면, 부정적 신념이 높은 집단에서는 이러한 정적 효과가 사라지고 부적 방향으로 전환되는 경향이 관찰되었으나 통계적으로 유의하지는 않았다. 셋째, 조절된 매개효과 분석 결과, 주관적 말더듬 심각도가 지각된 사회적 지지를 매개로 삶의 질에 미치는 간접효과는 부정적 신념이 낮은 집단에서만 유의하게 나타났으며, 조절된 매개지수 역시 통계적으로 유의하였다. 이러한 결과는 부정적 신념이 낮은 말더듬 성인은 말더듬이 심하더라도 지각된 사회적 지지 수준이 유지되며, 이것이 삶의 질과 정적으로 연관되는 반면, 부정적 신념이 높은 경우에는 이러한 보호적 매개경로가 작동하지 않음을 시사한다. 본 연구는 말더듬 성인의 삶의 질 향상을 위한 개입에 있어 단순한 유창성 증진을 넘어, 말더듬에 대한 부정적 신념의 완화와 사회적 지지 체계의 강화를 동시에 고려하는 통합적 접근의 필요성을 제안하였다.

The purpose of this study was to examine the effect of self-reported stuttering severity on quality of life in adults who stutter, testing perceived social support as a mediator and negative beliefs about stuttering as a moderator of that mediation. Beyond a fluency disorder, stuttering is a complex condition involving negative affect and cognitive distortions that pervade daily life. Yet research in Korea has focused mainly on children or on reducing the overt frequency of stuttering, leaving the intrapsychic mechanisms and quality of life of adults who stutter understudied. The study therefore sought to clarify the pathway from self-reported stuttering severity to quality of life and to determine whether negative beliefs act as a protective or risk factor in that process.
A total of 150 adults who stutter, recruited from online stuttering communities, completed the Subjective Screening of Stuttering–Research Edition (SSS–R), the brief Unhelpful Thoughts and Beliefs About Stuttering Scales (UTBAS-6), the Multidimensional Scale of Perceived Social Support (MSPSS), and the Korean version of the World Health Organization Quality of Life Assessment–BREF (K–WHOQOL–BREF). Data were analyzed using SPSS 27.0 and the PROCESS Macro, including descriptive statistics, correlation analysis, and mediation and moderated mediation analyses.
The main results were as follows. First, perceived social support had a significant positive effect on quality of life, whereas self-reported stuttering severity had a significant negative effect. Second, the effect of stuttering severity on perceived social support was significantly moderated by negative beliefs: when negative beliefs were low, greater severity was associated with a significant increase in perceived social support, but this positive effect disappeared (and tended to reverse, though not significantly) when negative beliefs were high. Third, the indirect effect of stuttering severity on quality of life through perceived social support was significant only when negative beliefs were low, and the index of moderated mediation was also significant. Thus, adults who stutter with low negative beliefs maintained perceived social support, and therefore quality of life, despite greater severity, whereas this protective pathway did not operate when negative beliefs were high.
The study suggests that interventions to improve quality of life in adults who stutter should go beyond enhancing fluency to address both the reduction of negative beliefs about stuttering and the strengthening of social support.
