Electrocardiography (ECG) is a widely used, non-invasive diagnostic tool that reflects integrated electrical activity of the heart. With recent advances in artificial intelligence (AI), standard 12-lead ECGs have been increasingly explored as a source of latent physiological information beyond conventional interpretation. In this study, we developed and evaluated an AI-based model for estimating ECG-derived heart age (ECG-HA) using standard 12-lead ECG recordings obtained from individuals without overt cardiovascular disease. This retrospective study analyzed digitally stored ECGs acquired at a single tertiary medical center between January 2010 and January 2024. After applying predefined exclusion criteria to remove ECGs associated with significant cardiovascular disease, a total of 292,484 ECGs were included and partitioned into training, validation, and test datasets with patient-level separation. A convolutional neural network based on the DenseNet-121 architecture was trained to predict chronological age from ECG signals in a supervised regression framework. ECG-HA was defined as the continuous age value predicted by the model, and the heart age gap was calculated as the difference between ECG-HA and chronological age. Model performance was evaluated using the coefficient of determination (R²), mean absolute error (MAE), root mean squared error (RMSE), and Pearson correlation coefficient. In the test cohort, the model demonstrated strong predictive performance, with an R² of 0.783, a Pearson correlation coefficient of 0.885, an MAE of 5.023 years, and an RMSE of 6.389 years. Systematic age-dependent patterns were observed, with ECG-HA tending to be overestimated in younger individuals and underestimated in older adults. Supplementary analyses showed that a larger ECG-HA–chronological age gap was associated with higher rates of cardiovascular hospitalization, particularly when the gap exceeded five years. In addition, individuals with hypertension exhibited consistently higher ECG-HA values across the age spectrum compared with those without hypertension. In conclusion, this study demonstrates that an AI-based model can reliably estimate ECG-derived heart age from standard 12-lead ECGs in individuals without overt cardiovascular disease. While predictive accuracy was highest in middle-aged individuals, age-dependent bias was observed at the extremes of age. Despite this limitation, the present findings support the potential role of ECG-HA as a non-invasive marker of biological cardiac aging. Further refinement and validation in broader clinical populations are warranted.
