Background: Blood creatinine (Cr) testing is an important indicator used to diagnose kidney diseases and evaluate the effectiveness of treatments. Recently, it has been widely used for renal function assessment to prevent side effects of contrast media, which are essential in imaging tests such as CT and coronary angiography. For this purpose, rapid serum Cr testing is required in clinical settings; however, although some rapid Cr testing systems have been introduced, their clinical use is limited due to high testing costs. Therefore, in this study, a low-cost, high-efficiency rapid serum Cr testing system was developed, and its utility was evaluated using clinical specimens. Methods: To achieve rapid detection of serum Cr, a paper-based sensor based on enzymatic reactions was designed. Optimal conditions were determined by adjusting the concentrations of enzymes and chromogenic reagents and analyzing signal intensity using a digital imaging system (ChemiDoc MP). Stability was evaluated under accelerated conditions, and selectivity against interfering substances was confirmed. The correlation of clinical serum specimens was assessed by comparing the results with measurements from the Roche Cobas 8000 c702, a conventional standard testing method. Results: The Cr-paper sensor consisted of a sample pad, a first reaction pad (creatine detection), and a second reaction pad (Cr and creatine detection). To enhance selectivity for Cr, the calculation formula Cr = R2 - R1 was applied, subtracting the signal intensity of the first reaction pad (R1) from that of the second reaction pad (R2). High signal intensity was secured by establishing optimal concentrations of enzymes and chromogenic reagents. The Cr-paper sensor maintained stable performance for approximately 30 days at 4°C. Furthermore, it demonstrated linear signal intensity within a concentration range of 0-20 mg/dL, and its correlation with the standard testing method showed a coefficient of determination of R2=0.97, confirming its feasibility for clinical use. In clinical serum specimen testing, the sensor clearly distinguished between normal and abnormal samples, achieving high sensitivity and specificity. Conclusion: The researchers successfully developed a paper-based Cr measurement sensor that is cost-effective and capable of rapid clinical use. The developed sensor demonstrated higher economic efficiency and equivalent or superior selectivity and stability compared to existing Cr detection devices. In particular, it is expected to be widely utilized not only in clinical radiology departments to prevent side effects before contrast media administration but also in various clinical settings where rapid assessment of renal function is required.
