The purpose of this study was to investigate the effects of diaphragmatic breathing exercise using an air stacking technique on respiratory function and trunk function in patients with stroke. Twenty stroke patients admitted to a general hospital located in Dong-gu, Daegu, South Korea, were randomly assigned to either an experimental group (n = 10), which performed diaphragmatic breathing exercise using the air stacking technique, or a control group (n = 10), which performed diaphragmatic breathing exercise alone.
Both groups received conventional physical therapy, including neurodevelopmental treatment and gait training, along with respiratory intervention conducted three times per week for 20 minutes per session over a period of four weeks.
To evaluate respiratory and trunk function before and after the intervention, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), the ratio of FEV1 to FVC (FEV1/FVC), peak expiratory flow (PEF), chest expansion, the Korean version of the Trunk Impairment Scale (K-TIS), the Five Times Sit-to-Stand Test (FTSTS), and the Borg Rating of Perceived Exertion (Borg RPE) were measured.
The results showed that both groups demonstrated significant improvements in FVC, FEV1, PEF, chest expansion, K-TIS, FTSTS, and Borg RPE after the intervention. Additionally, the experimental group showed greater improvements than the control group in the changes observed between pre- and post-intervention measurements. Although the FEV1/FVC ratio showed a slight decrease in both groups, no significant differences were found in terms of time, group, or group-by-time interaction.
Furthermore, changes in FVC and chest expansion were significantly positively correlated with changes in K-TIS scores, suggesting that improvements in respiratory capacity and chest mobility are associated with enhanced trunk control.
In conclusion, diaphragmatic breathing exercise using an air stacking technique significantly improves respiratory function, trunk control, functional mobility, and perceived exertion in patients with stroke, and these findings suggest that this intervention may serve as a useful therapeutic approach for respiratory rehabilitation and enhancement of trunk function in this population.
