배경: 심혈관질환 (CVD)과 치매는 전 세계적으로 주요한 공중보건 과제로서, 사망, 장애 및 경제적 부담의 주요 원인이 되고 있다. 비감염성 질환 (NCDs)은 전 세계 사망의 막대한 비중을 차지하며, 그중 심혈관질환은 지속적으로 전 세계 사망 원인 1위를 기록하고 있다. 또한, 치매는 긴 유병 기간을 특징으로 하여 개인의 건강을 점진적으로 악화시킬 뿐만 아니라 가족, 지역사회, 의료 시스템 및 경제 전반에 심대한 부담을 초래한다. 심혈관질환과 치매는 전신 염증 및 산화 스트레스와 같은 공통된 병태생리학적 기전을 공유하며 상호 밀접하게 연관된 복합 질환으로 인식되고 있다. 따라서 제한된 자원과 환경 내에서 심혈관질환과 치매를 예방하고 관리하기 위해서는 효율적이고 효과적인 의학 및 공중보건 전략이 필수적이다. 이는 심혈관질환과 치매에 대한 개인 및 지역사회 수준의 요인을 규명할 필요성을 시사한다. 구체적으로 본 연구는 행동 요인과 같은 수정 가능한 위험 요인과 지역사회 및 환경 수준의 요인을 포괄하는 건강의 사회적 결정요인 (SDoH)에 초점을 맞추어 심혈관질환과 치매의 관련 요인을 규명하고자 하였다. 전통적인 위험 요인들은 잘 확립되어 있으나, 본 연구는 생물학적 차원과 심리사회적 차원을 아우르는 새롭게 대두되는 다차원적 요인에 초점을 맞추었다. 특히, 개인 수준에서는 장기적인 항생제 노출이 장내 미생물 불균형과 그에 따른 염증 반응을 통해 질병 경로에 잠재적으로 영향을 미칠 수 있는 위험 요인으로 제시되고 있다. 한편, 복약 순응도와 신체 활동과 같은 행동 요인은 질병 관리에 중요하며, 환경 노출과 건강결과 간 연관성을 잠재적으로 조절하는 데 기여할 수 있다. 지역사회 수준에서는 대기오염이 혈관 및 신경 퇴행성 손상과 연관된 잘 알려진 환경적 요인인 반면, 녹지와 사회적 자본은 신체 활동을 촉진할 가능성이 있고, 스트레스를 완화하며, 심리사회적 유대감을 지지함으로써 완충 요인으로 작용할 수 있다. 본 연구는 국가 행정 및 감시 데이터베이스와 연계된 대한민국 국민건강보험공단 (NHIS) 데이터베이스를 활용하여 다음을 조사하였다. (1) 개인 수준 요인 (항생제 노출, 복약 순응도, 신체 활동); (2) 지역사회 수준 결정요인 (대기오염, 녹지, 사회적 자본); (3) 개인 및 지역사회 수준 요인의 결합 효과 (스타틴 복약 순응도와 대기오염, 그리고 신체 활동 강도와 대기오염).
방법: 본 연구는 한국 국민건강보험공단 청구 데이터베이스와 참가자의 거주지 행정 구역을 기준으로 연계된 국가 행정 및 감시 데이터베이스를 활용한 대규모 인구 기반 후향적 코호트 연구이다. 초미세먼지 농도와 같은 대기오염 데이터는 에어코리아 (AirKorea)의 관측 데이터를 활용하여 배정하였다. 도시 녹지 면적은 국가통계포털 (KOSIS)에서 확보하였다. 사회적 신뢰 정보는 지역사회건강조사 자료로부터 도출하였다. 주요 결과변수인 심방세동 (AF)을 포함한 심혈관질환 및 치매는 진단 코드와 관련 약물 처방 또는 의료 이용 이력을 결합하여 정의하였다. 본 연구는 (1) 개인 수준 요인 (누적 항생제 처방 노출, 스타틴 복약 순응도 [MPR], 및 신체 활동 강도 [METs]), (2) 지역사회 수준 요인 (초미세먼지 [PM2.5] 장기 노출, 도시 녹지 공간 비율, 및 사회적 신뢰), 그리고 (3) 초미세먼지 노출과 스타틴 복약 순응도 및 초미세먼지 노출과 신체 활동 강도의 결합 효과와 심혈관질환 및 치매 위험 간의 연관성을 조사하였다. 다변량 분석은 Cox 비례 위험 회귀 모형을 사용하여 보정 위험비 (aHRs)를 산출하고, 모든 원인의 사망을 경쟁 위험으로 고려하여 Fine and Gray 하위분포 위험 모형을 사용하여 하위분포 위험비 (sHRs)를 산출하였다. 통계 모형은 인구사회학적 특성, 건강 검진 결과 및 임상 정보 등을 보정하였다. 또한, 주요 결과의 견고성을 평가하기 위해 층화 분석 및 민감도 분석을 수행하였다.
결과: 한국 인구를 대상으로 공변량을 보정한 결과, 각 연구 구성 요소에서 연관성이 관찰되었다. (1) 개인 수준 요인 분석에서, 장기적인 누적 항생제 노출은 심혈관질환 및 치매 위험 증가와 연관된 반면, 높은 스타틴 복약 순응도는 심혈관질환의 위험 감소와 연관되었으며, 높은 신체 활동 강도는 심방세동 및 치매 위험 감소와 연관되었다. (2) 지역사회 수준 요인 분석에서, 초미세먼지 농도 증가는 심혈관질환 위험 상승과 연관되었다. 대조적으로, 높은 도시 녹지 공간 비율과 사회적 신뢰는 각각 심혈관질환과 치매 위험 감소와 연관되었다. (3) 결합 효과와 관련하여, 높은 스타틴 복약 순응도는 고농도 초미세먼지 노출과 심혈관질환 위험 간 연관성이 더 약하게 관찰되었다. 또한, 고농도 초미세먼지 노출 지역에서도 높은 신체 활동 강도는 심혈관질환에 대한 보호적 연관성이 유지되었다. 이러한 연관성은 층화 분석 및 민감도 분석에서 전반적으로 일관되게 나타났다.
결론: 본 대규모 후향적 연구는 다차원적 관점에서 요인들을 포괄적으로 조사함으로써, 심혈관질환과 치매의 관련 요인에 대한 역학적 근거를 제시하였다. 한국 인구 집단 대상의 본 연구는 장기적인 항생제 사용 및 대기오염 노출의 잠재적인 부정적 연관성을 시사하는 한편, 복약 순응도, 신체 활동, 도시 녹지, 및 사회적 신뢰의 잠재적인 보호 역할을 시사하였다. 특히 결합 효과 분석 결과, 스타틴 복약 순응도 및 신체 활동으로 대표되는 건강 관련 행동은 고농도 초미세먼지 노출 지역에서도 일관되게 낮은 심혈관질환 위험과 연관된 것으로 나타났다. 지역사회 수준의 구조적 개선과 정책 수준의 개입은 필요하지만 많은 시간이 소요됨을 고려할 때, 이러한 결과는 개인 수준의 행동 개선이 환경 유해요인 노출과 관련된 불리한 연관성을 완화하는 데 고려될 수 있음을 시사한다. 다만, 이러한 연관성의 기저에 있는 인과관계를 보다 명확하게 규명하기 위해서는 향후 추가적인 전향적 연구가 필요하다. 본 연구는 단일 수준 분석을 넘어 개인 및 환경 요인의 복합적인 영향을 고려하는 통합적 접근 방식의 가치를 부각시키며, 궁극적으로는 건강 형평성 증진과 효과적인 공중보건 계획 수립에 기여할 수 있다.

Introduction: Cardiovascular disease (CVD) and dementia pose major global public health challenges, contributing significantly to mortality, disability, and economic burden. Noncommunicable diseases (NCDs) account for a vast proportion of global mortality, with CVD consistently remaining the primary cause of death worldwide. Furthermore, dementia is characterized by a prolonged period of morbidity, leading to progressive deterioration in individuals’ health and imposing substantial burdens on families, communities, healthcare systems, and the broader economy. CVD and dementia are increasingly recognized as complex, interrelated conditions that share common pathophysiological mechanisms, such as systemic inflammation and oxidative stress. Therefore, efficient and effective medical and public health strategies are essential for the prevention and management of CVD and dementia, particularly within the constraints of limited resources and settings. This underscores the need to identify both individual- and community-level factors for CVD and dementia. Specifically, this study explores factors associated with CVD and dementia, focusing on modifiable risk factors, such as behavioral factors, and the social determinants of health (SDoH), which encompass community-level and environmental factors. Beyond the well-established scope of traditional risk factors, this study focuses on emerging multidimensional determinants incorporating both biological and psychosocial dimensions. At the individual level, long-term antibiotic exposure has been suggested as a potential risk factor, possibly influencing disease pathways through gut microbiota dysbiosis and subsequent inflammation. Meanwhile, behavioral factors such as medication adherence and physical activity are important for disease management and could potentially modify associations between environmental exposures and health outcomes. At the community level, air pollution is a well-known environmental factor associated with vascular and neurodegenerative damage, whereas green space and social capital may act as buffers by potentially facilitating physical activity, relieving stress, and supporting psychosocial cohesion. This study utilized the Korean National Health Insurance Service (NHIS) database, linked with national administrative and surveillance databases, to examine: (1) individual-level factors (antibiotic exposure, medication adherence, and physical activity); (2) community-level determinants (air pollution, green space, and social capital); and (3) the joint effects of these individual- and community-level factors (statin medication adherence and air pollution, and physical activity intensity and air pollution) on CVD and dementia.
Methods: This large-scale, population-based retrospective cohort study utilized the Korean NHIS claims database, linked with national administrative and surveillance databases, based on participants’ residential districts. Air pollution data, such as ambient PM2.5 concentrations, were assigned using AirKorea monitoring data. Urban green space coverage was obtained from the Korean Statistical Information Service (KOSIS), while social trust was derived from the Community Health Survey. The primary outcomes, CVD (including atrial fibrillation [AF]) and dementia, were defined using diagnostic codes in conjunction with relevant medication prescriptions or healthcare utilization history. This study examined the associations of: (1) individual-level factors (cumulative antibiotic prescription exposure, statin medication adherence [Medication Possession Ratio, MPR], and physical activity intensity [Metabolic Equivalent of Task, METs]); (2) community-level factors (long-term fine particulate matter [PM2.5] exposure, urban green space coverage, and social trust); and (3) the joint effects of PM2.5 exposure and statin adherence, as well as PM2.5 exposure and physical activity intensity, with the risks of CVD and dementia. Multivariable analyses were performed using the Cox proportional hazards regression models to estimate adjusted hazard ratios (aHRs) and the Fine and Gray subdistribution hazard models to estimate subdistribution hazard ratios (sHRs), explicitly accounting for all-cause mortality as a competing risk. Statistical models were adjusted for sociodemographic characteristics, health examination results, and clinical information. Furthermore, stratified and sensitivity analyses were conducted to assess the robustness of the major findings.
Results: In the Korean population, after adjustment for covariates, associations were observed across the study components. (1) In the analysis of individual-level factors, long-term cumulative antibiotic exposure was associated with increased risks of both CVD and dementia. Conversely, high statin adherence was associated with a reduced risk of CVD, and greater physical activity intensity was linked to decreased risks of AF and dementia. (2) In the analysis of community-level factors, elevated PM2.5 exposure was associated with an increased risk of CVD. In contrast, greater urban green space coverage and higher social trust were associated with decreased risks of CVD and dementia, respectively. (3) Regarding joint effects, high statin adherence attenuated the adverse CVD risk associated with high PM2.5 exposure. Furthermore, greater physical activity intensity maintained its protective benefit against CVD, even in areas with high PM2.5 exposure. These associations were consistently observed across stratified and sensitivity analyses.
Conclusion: This large-scale retrospective study provides epidemiological evidence on factors associated with CVD and dementia by comprehensively examining determinants from a multidimensional perspective. Within the Korean population, the findings suggest potential adverse associations of long-term antibiotic use and air pollution exposure with the risk of these diseases, while highlighting the potential protective roles of medication adherence, physical activity, urban green space, and social trust. The assessment of joint effects indicated that positive individual behaviors, exemplified by statin adherence and physical activity, were consistently associated with lower CVD risk, even in areas with high PM2.5 concentrations. While community-level structural reforms and policy-level interventions are needed but time-intensive, these results suggest that individual-level behavioral modifications may be considered a practical and accessible approach to potentially mitigate the impact of environmental hazards. However, further prospective research is warranted to clarify causal relationships underlying these associations. Collectively, this study highlights the potential value of moving beyond single-level analyses to an integrated approach that considers the combined impact of individual and environmental factors, with potential implications for health equity and more effective public health planning.
