정서표현 양가성, 건강통제 소재가 암 환자의 적응에 미치는 영향: 사회적 지지의 조절효과 = The Influence of Ambivalent Emotional Expressiveness, Health Locus of Control on Psychosocial Adjustment of Cancer Patients: Moderating Effect of Social Support

The purpose of this study was to investigate the effects of ambivalent emotional expressiveness and health locus of control on the psychosocial adjustment of cancer patients, and its moderating effect of social support.
The subjects of this study were 118 cancer patients, currently receiving chemotherapy and radiation therapy in hospitals. The Mini-Mental Adjustment to Cancer (Mini-MAC), Hospital Anxiety and Depression Scale (HADS), Ambivalent Emotional Expressiveness Questionnaire (AEQ), Multidimensional Health Locus of Control Scale (MHLC), and Illness-Specific Social Support Scale (ISSS-GR) were administrated to them.
The results of a multiple regression analysis were as follow. If patients have shown more ambivalent emotional expressiveness, their psychological disturbances such as anxiety, depression, helpless-hopeless and anxious preoccupation, were higher and the will to overcome cancer was lower. Also, if patients had more 'other health locus of control' and 'chance health locus of control' than 'internal health locus of control', their psychological disturbances were higher and the will to overcome cancer was lower.
By examining the moderating effect of social support on cancer patients, in the relationship between ambivalent emotional expressiveness and psychosocial adjustment, positive support was an important factor in reducing helpless-hopeless. But, in the relationship between health locus of control and psychosocial adjustment, social support had no effect in reducing helpless-hopeless.
The suggestions of this study and implications for further research were discussed.

국문 초록 (Abstract)

본 연구는 정서표현 양가성과 건강통제소재가 암 환자의 심리사회적 적응에 미치는 영향과 사회적 지지의 조절효과를 알아보았다. 암 진단을 받은 후 암 재활 전문 요양병원에 입원한 환자 중현재 항암화학요법 및 방사선 치료중인 118명을 대상으로 암 적응척도, 병원 불안-우울척도, 정서표현 양가성 척도, 건강통제소재 척도, 사회적 지지 척도를 사용하여 관련 변인들 간의 관계를 알아보았다. 단계적 회귀분석을 실시한 결과, 정서표현 양가성이 높을수록 불안, 우울, 무력감-무망감 및 불안몰두와 같은 심리적 고통이 높은 반면, 질병에 대한 투지는 낮은 것으로 나타났다. 또한 내적 건강통제소재 보다 타인이나 우연건강통제소재를 보일수록 불안, 우울, 무력감-무망감 및불안몰두와 같은 심리적 고통이 높은 반면, 투지에 대한 적응지표는 낮은 것으로 나타났다. 이에질병에 대한 자기 통제감을 가지고 건강관리에 적극적으로 참여하는 것이 중요한 것으로 보인다.
한편 사회적 지지의 조절효과를 살펴본 결과 정서표현 양가성과 심리사회적 적응과의 관계에서긍정적 지지의 조절효과는 무력감-무망감을 낮추는 것으로 나타났다. 그러나 건강통제소재와 심리사회적 적응과의 관계에서 사회적 지지의 조절효과는 없었다. 이러한 결과를 바탕으로 본 연구의 의의와 제안점, 후속 연구에 대한 제언을 논의하였다.
