호스피스 말기암 환자를 대상으로 한 원예치료 회상 소재 및 주제 분석 = Recollective Material and Topic Analysis of Horticultural Therapy for Hospice Terminal Cancer Patients

This study designed and implemented a horticultural therapy recall program for patients with terminal hospice cancer by classifying 34 types of plants used in a horticultural therapy program with reminiscence therapy into cut flowers, potted plants, and processed flowers. In the course of executing the program, we attempted to provide basic data when designing a horticultural therapy program that is applied as an alternative therapy by identifying the recollection materials and topics that hospice patients prefer. The subjects of this study were patients with end-stage hospice cancer in D City D Hospital, who were in the process of hospice palliative care and dying and who no longer require medical treatment by the law on life-sustaining medical decisions, and were composed of clear communication. During the study period, from July 2016 to December 2019, horticultural therapy was conducted once a week with patients on a one-to-one basis for 10-15 minutes. When the subjects preferred and applied a horticultural therapy program using plants that could recall, they expressed their happy memories and present feelings through various images of plants. And as the stories of people around the subject of love, the emotions of the past and the present were expressed in various stories. The stories of the plants they saw in their childhood could give a sense of happiness with unforgettable memories even when they were old, and reminiscing about their parents, brothers, friends, and hometown also stabilized their unstable emotions. In this way, when a horticultural therapy program is implemented for terminal hospice patients, it is judged that the overall therapeutic effect for hospice patients will increase if the various materials that can be recalled and the associated recall topics are combined through storytelling.

국문 초록 (Abstract)

본 연구는 회상요법을 적용한 원예치료 프로그램에 사용된 식물 34 종류를 절 화, 분화, 가공화로 분류하여 호스피스 말기암 환자들을 대상으로 원예치료 회상 프로그램을 설계 및 실행하였다. 프로그램을 실행하는 과정에서 호스피스 환자들 이 선호하는 회상 소재와 주제를 파악하여 대체 요법으로 적용되는 원예치료 프로 그램 설계 시에 기초자료를 제공하고자 하였다. 연구 대상자는 D시 D병원 호스피 스 말기암 환자들로 호스피스 완화의료 및 임종 과정에 있고 연명 의료결정에 관한 법률에 의해 의학적인 치료를 더 이상 필요로 하지 않는 환자들로 의사소통이 분명 한 분들로 구성하였다. 연구 기간은 2016년 7월부터 2019년 12월까지 매주 1회 환 자와 일대일로 10~15분간 원예치료를 실시하였다. 대상자들이 선호하면서 회상이 가능한 식물을 이용한 원예치료 프로그램을 적용하였을 때 식물의 다양한 이미지 를 통해 행복했던 추억과 현재의 마음들을 표현하였다. 그리고 사랑과 관련된 주 변 사람들의 이야기로는 과거와 현재의 정서를 다양한 이야기로 표현하였다. 유년 에 보았던 식물들에 대한 이야기는 나이가 들어서도 잊을 수 없는 추억으로 행복감 을 줄 수 있었고 부모, 형제, 친구, 고향 등에 대한 회상은 불안정한 정서를 안정적 으로 만들어 주기도 하였다. 이와 같이 호스피스 말기암 환자들을 위해 원예치료 프로그램을 실시할 때는 회상할 수 있는 다양한 소재와 그와 연관된 회상 주제를 스토리텔링을 하여 접목한다면 호스피스 환자를 위한 전반적인 치료 효과가 높아 질 것이라고 판단된다.
