본 연구의 목적은 정상 골격양상을 보이는 젊은 성인 여성에서 안모의 심미도를 평가한 후 삼차원적 좌표 및 두부 계측치를 이용하여 교정의, 일반치과의, 그리고 일반인 세 군 간에 안모의 심미성에 대한 인식의 차이가 있는지를 알아보고자 하는 것이다.
연구 대상은 전후방, 수직 및 수평적으로 정상 골격양상을 보이며 교정 치료를 위해 콘빔전산화단층촬영(CBCT) 영상을 채득한 40명의 젊은 성인 여성(23.9±4.7세)으로 하였다. 이들의 CBCT 영상을 ON3D (3DONS, INC., Seoul, Korea) 소프트웨어를 이용해 리오리엔테이션한 후 삼차원적 두부 계측 및 분석을 시행하였다. 다섯 장의 삼차원적 안모 영상 (정면, 45도 좌우 측면, 90도 좌우 측면)과 두 개의 질문으로 구성된 컴퓨터 기반 설문지를 이용하여 세 군으로 구성된 총 124명(교정의 42명, 일반치과의 42명, 일반인 42명)의 평가자가 안모의 심미성을 numeric rating scale(NRS)을 이용하여 1점 (가장 비심미적)에서 10점 (가장 심미적) 사이의 점수로 평가하였다. 심미군 (상위 20%)과 비심미군 (하위 20%)의 삼차원 계측치를 분석하여 세 군간의 심미적 인식의 차이를 비교하였다. 추가적으로, 안모 심미성을 평가 시 가장 큰 영향을 준 안면 영상과 영역을 조사 및 비교하였다.
심미군에서 비심미군에 비해 더 큰 Pog-NB(P < 0.05), 상순의 두께(P < 0.05) 및 비순각(P < 0.001)을 보였고, FH plane에 대하여 더 작은 상순의 경사도(P < 0.001) 및 상악 중절치 치축각(P < 0.001), 더 큰 하악 중절치의 치축각(P < 0.01)을 보였다. 또한 심미선에 비해 상순(P < 0.05)과 하순(P < 0.001)은 더 후방으로 위치하였다.
각각의 세 군에서도 이와 유사한 경향을 나타냈으며, 세 군 간의 안면 심미도 순위도 서로 높은 상관관계를 보였다. 안모 심미성을 판단하는데 영향을 미친 안면 영상 및 영역은 세 군 사이에 유의한 차이가 있었다. 교정의와 일반치과의에서는 90도 측면 영상을 보는 빈도가 각각 59.5%, 57.2%로 유의하게 가장 높았다. 일반인에서는 정면 영상을 보는 빈도가 40.5%로 가장 높았으며, 다른 영상과의 빈도 차이가 유의하지 않았다. 안모의 심미성을 평가 시 주목받는 안면영역을 비교해 본 결과, 교정의와 일반인은 각각 입술(73.8%)과 턱(45.2%)을 유의하게 높은 비율로 주목하였으며, 일반치과의는 턱(38.1%)을 가장 높은 비율로 주목하였으나 유의하지는 않았다.
정상 골격 양상을 보이는 성인 여성의 안모 심미성을 평가 시 교정의, 일반치과의, 일반인의 모든 군에서 전치 및 입술이 더 후방으로 위치 및 경사진 경우에 더 높은 심미적 선호도를 보였다. 그러나 심미성을 평가하는 방식에서는 군 간의 차이를 보였다.

Objective: The purpose of this study was to evaluate the facial attractiveness for young female adults with normal skeletal pattern and then to investigate the differences of esthetic perception between orthodontists, general dentists, and laypersons using 3-dimensional coordinates and cephalometric measurements.
Methods: The subjects were consisted of 40 young female adults (23.9± 4.7 years) with normal skeletal pattern who had taken cone-beam computed tomography (CBCT) scans for orthodontic treatment. Their CBCT images were reoriented and 3-dimensional cephalometric measurement and analysis were done using ON3D (3DONS, INC., Seoul, Korea) software. A computer-based questionnaire that was comprised of 3-dimensional facial images of 40 subjects (5 images; frontal, 2 oblique, and 2 lateral) and two questions was used for a total of 124 evaluators (42 orthodontists, 42 general dentists, and 42 laypersons) to estimate the facial attractiveness of 40 subjects and to score them between 1 (very unattractive) and 10 (very attractive) using numeric rating scale (NRS). According to the ranking of the facial attractiveness, 40 subjects were divided into attractive group (top 20%) and unattractive group (bottom 20%), and 3D measurements were analyzed and esthetic perception was compared between 3 groups of evaluators. Additionally, most influential facial view and component on evaluating facial esthetics were examined and compared.
Results: Compared to the unattractive group, the attractive group showed greater Pog-NB (P < 0.05), U lip thickness (P < 0.05), L1SI-FH (P < 0.01), and Nasolabial angle (P < 0.001), and lesser Cant of upper lip (P < 0.001) and U1SI-FH (P < 0.001), and more retruded position of U lip (P < 0.05) and L lip (P < 0.001) relative to esthetic lines. A similar tendency was shown in each group of evaluators and significantly high correlation was found in the ranking of facial attractiveness between 3 groups of evaluators. When evaluating facial esthetics, affecting facial views and components showed significant differences between 3 groups. Orthodontists (59.5%) and general dentists (57.2%) showed significantly higher rates for considering profile view. Laypersons (40.5%) considered frontal view the most but with no significance. When evaluating facial esthetics depending on the facial components, orthodontists and laypersons focused on lips (73.8%) and chin (45.2%) at significantly higher rates, respectively, and general dentists focused on chin (38.1%) at a higher rate but with no significance.
Conclusions: When evaluating the facial esthetics for young female adults with normal skeletal pattern, all 3 groups (orthodontists, general dentists, and laypersons) showed a high esthetic preference for more retroclined and retruded position of the incisors and lips. But they showed some differences on the way to evaluate facial soft tissue esthetics.
