본 연구는 일상생활 수행에서 도움이 필요한 60세 이상 재가 노인을 대상으로, 돌봄 제공자별 월평균 돌봄 시간을 기반으로 노인의 돌봄 수혜 유형을 도출하고, 이러한 시간 기반 돌봄 수혜 유형에 영향을 미치는 개인, 가족, 환경 요인을 규명하고자 하였다. 한국 사회는 급속한 고령화로 인해 노인의 돌봄 수요가 지속적으로 증가하고 있으며, 장기요양보험 제도 도입 이후에도 돌봄의 상당 부분은 여전히 가족에 의해 수행되고 있다. 그간 노인 돌봄 연구는 주로 공식·비공식 돌봄의 이용 여부나 제공자의 존재 여부를 중심으로 유형을 구분해 왔으나, 다수 제공자 간 돌봄 시간이 어떻게 분담되거나 집중되는지, 즉 돌봄의 시간 구조를 반영한 유형화는 충분히 검토되지 않았다. 이에 본 연구는 제공자별 돌봄 제공 시간을 기반으로 돌봄 수혜 유형을 도출하고, 노인 돌봄 수혜 유형에 영향을 미치는 요인을 분석하였다.
분석 자료는 고령화연구패널조사(Korean Longitudinal Study of Ageing, KLoSA) 제9차(2022년) 자료이며, 일상생활 수행에 도움이 필요하다고 응답한 60세 이상 노인 중 돌봄 제공 관련 문항(제공자 유형, 돌봄 제공 일수 및 시간)에 응답한 920명을 최종 분석 대상으로 선정하였다. 돌봄 수혜 유형을 도출하기 위해 제공자 유형별 월평균 돌봄 시간을 산출하여 K-medoids 군집분석을 실시하였고, 유형 간 특성 차이는 일원분산분석과 카이제곱 검정으로 확인하였다. 또한 도출된 유형을 종속변수로 설정하여, 개인 요인(성별, 연령, 최종 학력, ADL, IADL, 주관적 건강, 치매 진단 여부, 주택 소유 여부, 가구소득), 가족 요인(동거 자녀 수, 생존 자녀 수, 혼인 상태), 환경 요인(거주지역, 의료급여 수급 여부, 노인장기요양보험 이용 여부, 노인맞춤돌봄서비스 이용 여부)이 유형에 미치는 영향을 확인하기 위해 다항 로지스틱 회귀분석을 실시하였다.
군집분석 결과, 노인의 돌봄 수혜 유형은 배우자 전담형(36.4%), 딸 중심형(10.7%), 아들 기반 혼합형(16.8%), 돌봄 공백형(24.1%), 공공 중심형(12.0%)의 다섯 가지 유형으로 도출되었다. 배우자 전담형은 가장 많은 비중을 차지하고 있는 유형으로 돌봄의 대부분이 배우자에게 집중된 유형이며, 딸 중심형은 딸의 돌봄 시간 비중이 가장 높은 유형이다. 아들 기반 혼합형은 아들이 주요 비중을 차지하며 자녀의 배우자와 개인 간병인 등 복수의 제공자가 함께 관여하는 구조를 보였다. 돌봄 공백형은 모든 제공자의 월평균 돌봄 시간이 0시간으로 나타났고, 공공 중심형은 공공기관에서 보내준 간병인의 돌봄 비중이 가장 높은 유형이다.
다항 로지스틱 회귀분석 결과, 돌봄 공백형을 기준으로 유형에 영향을 미치는 요인은 개인, 가족, 환경 요인에서 다차원적으로 확인되었다. 개인 요인 중에서는 IADL이 모든 유형에서 공통적으로 유의하여 기능 제약이 클수록 돌봄 공백형에 비해 다른 돌봄 유형에 속할 가능성이 전반적으로 높았다. 연령은 배우자 전담형에서는 음(-)의 관련성을, 아들 기반 혼합형에서는 양(+)의 관련성을 보였다. 성별은 여성의 경우 돌봄 공백형에 비해 딸 중심형, 아들 기반 혼합형, 공공 중심형에 속할 가능성이 높았다. 가족 요인에서는 혼인 상태와 동거 자녀 수의 관련이 두드러졌는데, 혼인 중인 경우 돌봄 공백형에 비해 배우자 전담형에 속할 가능성이 매우 높게 나타난 반면, 딸 중심형·아들 기반 혼합형·공공 중심형에 속할 가능성은 낮은 경향을 보였다. 또한 동거 자녀 수가 많을수록 돌봄 공백형에 비해 딸 중심형과 아들 기반 혼합형에 속할 가능성이 높았다. 환경 요인에서는 중소도시 및 읍면부 거주자가 대도시 거주자에 비해 돌봄 공백형 대비 아들 기반 혼합형에 속할 가능성이 높았고, 노인장기요양보험과 노인맞춤돌봄서비스 이용자는 공공 중심형에 속할 가능성이 높았다.
본 연구의 주요 함의는 다음과 같다. 첫째, 본 연구는 돌봄 제공자별 돌봄 제공 시간을 기준으로 노인의 돌봄 수혜 유형을 도출함으로써, 돌봄이 특정 제공자에게 집중되는지 혹은 여러 제공자 간에 분담되는지를 실증적으로 확인하였다. 분석 결과, 배우자 전담형, 딸 중심형, 그리고 공공 중심형에서는 특정 제공자에게 돌봄이 집중되었지만, 아들 기반 혼합형에서는 아들을 중심으로 자녀의 배우자와 개인 간병인 등 복수의 제공자가 돌봄 시간을 분담하는 구조가 확인되었다. 이는 돌봄 제공자의 존재 여부나 조합을 중심으로 돌봄 유형을 구분해 온 기존 연구와 달리, 시간을 반영하여 돌봄 집중과 분담의 양상을 구체적으로 식별하였다는 점에서 기존 연구와 차별되는 시사점을 제시하였다. 둘째, 본 연구는 돌봄 유형 연구에서 주로 활용되어 온 제공자 존재 여부 중심의 이분법적 접근을 넘어, 돌봄 제공 시간을 활용한 유형화를 시도하였다는 점에서 방법론적 함의를 갖는다. K-medoids 군집분석을 활용한 본 연구의 접근은 평균 중심 분석이 지니는 극단치 민감성의 한계를 보완하고, 돌봄 제공 시간 분포의 특성을 보다 안정적으로 반영함으로써 돌봄 유형의 다양성을 포착하는 데 기여하였다. 이는 향후 돌봄 연구에서 시간 단위 정보를 활용한 정량적 접근의 가능성을 제시한다. 셋째, 본 연구의 결과에서 배우자 전담형, 딸 중심형, 아들 기반 혼합형과 같은 가족이 중심이 되는 유형이 전체의 상당 부분을 차지하고, 혼인 상태와 동거 자녀 수와 같은 돌봄 자원의 가용성이 돌봄 유형과 유의하게 관련된 점은 배우자와 자녀가 존재할 때 이들이 주요 돌봄 자원으로 기능한다는 위계적 보상모형이 유용한 관점을 제공함을 시사한다.
본 연구는 정책적·실천적 측면에서도 다음의 함의를 갖는다. 첫째, 배우자 전담형은 돌봄 제공자인 배우자의 건강 악화나 부재 시 돌봄 공백이 즉각적으로 발생할 위험이 크다. 이에 따라 단기보호, 주야간보호, 방문요양 등 대체 돌봄 자원의 사전적 연계와 지역사회 기반 지원체계의 강화가 필요하며, 돌봄 제공자에 대한 경제적 지원과 함께 건강관리, 상담, 휴식 지원 등 돌봄 부담 완화 정책이 병행될 필요가 있다. 둘째, 딸 중심형은 성별화된 가족 규범 속에서 돌봄 부담이 개인의 삶과 경제활동에 부정적 영향을 미칠 위험이 크다. 이에 따라 가족돌봄휴가의 실효성 제고, 단기·시간제 대체 돌봄 서비스 확대 등 돌봄 부담을 완화할 수 있는 제도적 지원이 강화될 필요가 있다. 셋째, 아들 기반 혼합형은 아들을 중심으로 복수의 제공자가 관여하는 특성을 고려한 돌봄 조정과 사례관리 기능의 중요성을 부각시킨다. 넷째, 돌봄 공백형은 현재 돌봄 필요도가 낮은 집단과 제도 및 가족 돌봄망에서 포착되지 않은 미충족 집단이 함께 포함될 가능성을 고려하여, 조기 발견과 예방적 접근을 중심으로 한 선별적 지원이 필요하다. 다섯째, 공공 중심형은 가족 돌봄 기반이 취약한 상황에서 공식 서비스가 돌봄 제공의 핵심 역할을 수행하고 있어, 개별 서비스의 확대보다 서비스 간 연계와 조정을 강화하는 통합적 접근이 중요하다. 이러한 유형별 접근은 돌봄 제공자 간 시간 분담과 특성을 고려한 맞춤형 개입을 가능하게 하며 지속 가능한 돌봄 체계 구축에 기여할 수 있을 것이다.

This study aims to identify typologies of care receipt among community-dwelling older adults aged 60 years and older who require assistance with activities of daily living, based on the monthly average hours of care provided by different caregivers, and to examine individual, family, and environmental factors associated with these time-based care typologies. In South Korea, rapid population aging has led to a continuous increase in care needs among older adults, and despite the introduction of the Long-Term Care Insurance system, a substantial share of care provision still relies on family members. Previous research on older adult care has primarily classified care types based on the use of formal and informal care or the presence of specific caregivers. However, limited attention has been paid to how care time is distributed or concentrated across multiple caregivers—that is, to the temporal structure of care provision. To address this gap, this study adopts a time-based approach to derive care receipt typologies and to analyze their associated factors.
Data were drawn from the ninth wave(2022) of the Korean Longitudinal Study of Ageing(KLoSA). The analytical sample consisted of 920 community-dwelling older adults aged 60 years and older who reported needing assistance with daily activities and provided valid responses to questions on care providers, care days, and care hours. Monthly average care hours were calculated by caregiver type, and K-medoids cluster analysis was employed to identify care receipt typologies. Differences across typologies were examined using analysis of variance and chi-square tests. Multinomial logistic regression analysis was further conducted to assess the associations of individual factors(gender, age, educational attainment, ADL, IADL, subjective health, dementia diagnosis, homeownership, and household income), family factors(number of co-residing children, number of surviving children, and marital status), and environmental factors(region of residence, medical aid status, use of long-term care insurance, and use of customized care services for older adults) with care typologies.
The cluster analysis identified five distinct care receipt typologies: spouse-only care(36.4%), daughter-centered care(10.7%), son-based mixed care(16.8%), no-care receipt(24.1%), and public-care-centered care(12.0%). The spouse-only care type was the most prevalent and was characterized by a strong concentration of care time on spouses. The daughter-centered care type showed the highest proportion of care time provided by daughters. The son-based mixed care type involved sons as primary caregivers, alongside multiple additional providers such as children’s spouses and privately hired caregivers. The no-care receipt type was characterized by zero monthly care hours from all caregiver categories, while the public-care-centered type relied primarily on caregivers dispatched by public institutions.
Multinomial logistic regression analyses, using the no-care receipt type as the reference category, revealed that factors associated with care typologies operated across individual, family, and environmental dimensions. Among individual factors, IADL limitations were consistently significant across all care types, indicating that greater functional limitations increased the likelihood of belonging to any care type relative to no-care receipt. Age was negatively associated with spouse-only care and positively associated with son-based mixed care. Female older adults were more likely than males to belong to the daughter-centered, son-based mixed, and public-care-centered types. Family factors showed pronounced effects: being married substantially increased the likelihood of spouse-only care, while reducing the likelihood of belonging to daughter-centered, son-based mixed, and public-care-centered types. A higher number of co-residing children was associated with greater likelihood of daughter-centered and son-based mixed care. Regarding environmental factors, residence in small or medium-sized cities and rural areas was associated with a higher likelihood of son-based mixed care compared to residence in large metropolitan areas, while the use of long-term care insurance and customized care services for older adults was positively associated with public-care-centered care.
This study contributes to the literature by empirically demonstrating whether care is concentrated on a single caregiver or shared among multiple caregivers through a time-based classification of care receipt typologies. Unlike previous studies that primarily relied on caregiver presence or combinations, this approach captures patterns of care concentration and division by incorporating care time as a key dimension. Methodologically, the use of K-medoids cluster analysis mitigates sensitivity to extreme values and allows for a more stable representation of care time distributions, highlighting the diversity of care arrangements among older adults. Furthermore, the predominance of family-centered care types and the significant associations of marital status and co-residing children with care typologies support the relevance of the hierarchical compensatory model, suggesting that spouses and children function as primary care resources when available.
From a policy and practice perspective, the findings underscore the need for differentiated interventions tailored to each care typology. Spouse-only care highlights the risk of sudden care gaps in the event of caregiver health deterioration, calling for proactive linkage to substitute care resources and strengthened community-based support. Daughter-centered care raises concerns about gendered caregiving burdens and the need for policies that mitigate negative impacts on women’s employment and well-being. Son-based mixed care underscores the importance of care coordination and case management across multiple caregivers. The no-care receipt type suggests the need for early identification and preventive interventions for potentially unmet care needs. Finally, public-care-centered care emphasizes the importance of integrated service coordination rather than isolated service expansion. A typology-based approach that accounts for the temporal structure of care provision may facilitate more targeted and sustainable care systems for older adults.
