Social communication refers to the ability to express one’s intentions and emotions in everyday life, interpret others’ verbal and nonverbal cues in context, and respond appropriately; it serves as a foundation for peer relationships and school adjustment. In particular, preschool-aged children with high-functioning autism spectrum disorder (ASD) may continue to experience pragmatic and discourse-level difficulties—such as considering the listener, managing conversational flow, and adjusting expressions to situational contexts—even when their language form (e.g., vocabulary and sentence structure) is comparable to that of typically developing peers. Accordingly, this study aimed to improve social communication in preschool-aged (4–7 years) children with high-functioning ASD by developing a picture-book-based social communication intervention program and exploring its content validity and change trends through a pilot study.
The program was developed by first organizing the theoretical rationale through a literature review. Target functions (intentionality and mental-state inference, consideration and politeness, play and conflict negotiation, self-expression, relationship repair, and greeting/explaining/requesting) were arranged as session goals, and appropriate picture books and extension activities were matched to each goal to create the initial draft. The program was then refined based on expert review to establish content validity. The pilot study was conducted with one child with high-functioning ASD from October to November 2025, delivered twice weekly for 50 minutes per session, totaling 12 sessions (including pre- and post-assessments). Each session used shared picture-book reading to facilitate identification of situational cues and inference of emotions and intentions, and included extension activities—such as worksheets, play, role-play, art, and cooking—to provide repeated practice of target expressions and interaction skills. To explore potential effects, pre- and post-intervention spontaneous speech samples were collected and analyzed, and caregiver ratings were obtained to examine the direction of change.
Pilot results indicated positive change trends in overall social communication, as caregiver ratings showed increases across all CPLC subdomains (discourse management, contextual adjustment, communicative functions, and nonverbal communication). In terms of interpretation, function-focused expressions such as requesting, responding, and questioning were relatively prominent at baseline, whereas post-intervention data suggested an increased tendency to monitor conversational flow and produce utterances that reflected situational cues. In addition, caregiver ratings of social validity after program completion were high, supporting satisfaction with learning emotional understanding/expression and social communication skills through picture books, as well as perceived feasibility for practical implementation.
In discussion, picture books functioned as a medium for establishing and maintaining joint attention between the therapist and the child, and served as a “meaning-making framework” that helped link emotions, intentions, and others’ responses based on scene cues such as characters’ facial expressions, actions, and relationship changes. Notably, in session delivery, the principle of “flexible implementation”—adjusting pacing and activity sequencing based on the child’s responses, particularly during moments when predictability decreased (e.g., transitions between contexts)—emerged as important rather than rigidly applying fixed procedures. This suggests that participation in interaction for preschool-aged children with ASD may be strongly influenced by a balance between environmental structuring and responsive support. In clinical practice, it is therefore necessary to consider modeling at a level the child can accept, designing predictable structures, and making fine-grained adjustments based on the child’s responses.
In conclusion, this study presented the development procedures and implementation principles of a picture-book-based intervention program that integratively addresses emotional understanding/expression and social communication skills for preschool-aged children with high-functioning ASD, and explored the direction of change and potential applicability through a pilot study. However, because this was a single-case pilot study, generalizability is limited, and interpretation should be restricted to identifying change trends given the use of caregiver-report measures and limitations related to the applicable age range. In addition, maintenance and generalization (follow-up) data were insufficient, and when observational analyses are included, objectivity may be weakened unless inter-rater reliability and treatment fidelity are reported. Future research should therefore expand participants and conduct systematic efficacy verification incorporating observational indicators, follow-up designs, and reporting of reliability and fidelity.
