Normal Pressure Hydrocephalus (NPH) and Parkinson’s Disease (PD) share neurological damage in the basal ganglia-cerebellar-frontal network, presenting with highly similar initial clinical features, including gait disturbances. NPH is classified as a reversible form of dementia, with symptoms that can improve through shunt surgery. However, due to the difficulty in differentiating NPH from other conditions, it is estimated that approximately 80% of patients remain undiagnosed and untreated (Kiefer, M., 2012). The diagnosis of PD primarily relies on observing motor symptoms such as bradykinesia, rest tremor, and rigidity, followed by a diagnosis of exclusion. In this process, 'gait characteristics' serve as the most pivotal clinical indicator. Currently, the diagnosis of both diseases heavily depends on the observation of external motor symptoms and exclusion criteria, leading to limitations in clear differentiation. Therefore, it is necessary to explore new objective diagnostic indicators that can complement the ambiguity of existing markers and enhance diagnostic accuracy.
The subjects of this study included a total of 90 participants: 30 pre-operative NPH patients, 30 PD patients, and 30 healthy elderly individuals. All participants were assigned 'The Walk (산책)' paragraph reading task. From the collected acoustic data, word-phrase (eojeol) duration and its coefficient of variation (CV), as well as pause duration and its CV, were measured using Praat. These metrics were selected to identify defects in speech timing patterns caused by basal ganglia-frontal network damage. A Two-way Repeated Measures ANOVA was conducted to verify the main effects and interaction effects between the groups and the indices.
The results showed that the difference in word-phrase duration according to the number of syllables was statistically significant, but the interaction effect between the number of syllables and the group was not significant. This indicates a common production characteristic across all groups, where the word-phrase duration significantly increases as the number of syllables increases. In the analysis of word-phrase duration CV, the NPH group exhibited the highest variability, while the healthy elderly group showed the lowest. Although there was no significant main effect of group on word-phrase duration CV overall, the post-hoc test revealed a significant difference between the healthy elderly group and the NPH group. Regarding pause duration, the difference according to the number of syllables was significant, and the interaction effect between group and syllable count approached marginal significance. Both the NPH and PD groups showed a tendency for a substantial increase in pause duration as the number of syllables increased. While the difference in pause CV according to the number of syllables was not statistically significant, there was a trend toward a difference in pause CV among the groups. Notably, the pause CV of the PD group was relatively higher compared to the other two groups.
In conclusion, word-phrase duration CV was identified as a sensitive index reflecting the decline in temporal control ability in NPH due to defects in the frontal-basal ganglia-cerebellar network. Conversely, the PD group exhibited an abrupt extension of pause duration and high pause CV in response to increased speech load, indicating a heterogeneous pattern of speech rhythm breakdown depending on the disease. Although both patient groups demonstrate defects in achieving temporal goals due to neurological damage, this study proved that the patterns manifest differently according to the unique pathological characteristics of each disease. This study is clinically significant in that it confirmed temporal variability indices of speech as objective and useful clinical markers that can contribute to the precise differential diagnosis of NPH and PD, which are otherwise difficult to differentiate due to the similarity of their early clinical symptoms.
