Studying of cell adhesion reinforcing surface modification of PEEK for spinal cage

This study aims to overcome disadvantage, low cell adhesion of PEEK spinal cage. Hydroxylation and phosphorylation modify PEEK surface with hydrophilic hydroxyl group and phosphate group. Biomineralization generate hydroxyapatite (HA) crystal on the modified surface using the phosphate group as nuclear. HA crystal detected by SEM image, XPS, water contact angle, and surface roughness evaluation.
Cell adhesion reinforcements were tested by human osteoblast (HOb) incubation on the modified PEEKs. SEM image, EDS, and Alizarin red s staining clarified the change of cell adhesion and effect of each surface modification.
The results show HA crystal grow up on the modified PEEK surface.
Unpresented sphere structure detected after phosphorylation and biomineralization. Calcium and phosphorous also detected after the surface modification from XPS. Water contact angle continuously decrease during biomineralization. Surface micro-roughness increases with surface modifications.
These results clearly inform that the surface modifications positively effect on the cell adhesion. After those surface modifications, cell counting of each sample were promoted with SEM/EDS and Alizarin red s staining.
As a result of this study, cell adhesion increased surface modified PEEK could be adapted to spinal cage.

추간체 유합 보형제용 PEEK의 표면개질을 통한 세포 접착성 향상에 관한 연구
본 연구에서는 기존 PEEK 보형제의 단점인 시술 후 추간판으로부터 미끄러져 탈리되는 현상을 낮은 세포 접착성을 향상시켜 보완할 목적으로 PEEK의 표면개질을 진행한 다음, 표면에서의 세포 접착 거동을 검토하여 인체의 골세포와의 접합성을 향상시켜 시술 상의 단점을 개선하고자 한다.
PEEK의 세포 접착성 보강은 PEEK 표면에 biomineralization을 통해 hydroxyapatite를 도입하여 수행하고자 하였다. 이 과정에서 PEEK의 높은 화학적 안정성 및 낮은 반응성으로 인한 HA 도입의 저하를 해결하고, biomineralization을 진행하는데 필요한 핵 역할을 수행할 히드록실기와 인산기를 각각 hydroxylation과 phosphorylation을 통해 도입하고, 연이어 유사 체액을 이용한 biomineralization을 통해 hydroxyapatite(HA)를 도입하였다.
화학적 표면 개질 반응의 진행 확인을 위해 ATR-FTIR 측정을 진행하여 화학적 결합의 변화를 관측하였고, biomineralization으로 각 시편에 도입된 HA의 존재 여부를 확인하기 위해 SEM image 측정, XPS, water contact angle, 표면 조도의 측정 및 Alizarin red s 염색을 진행하였으며, 세포 접착성의 개선을 확인하기 위한 SEM image 측정으로 부착된 세포들의 단위면적당 세포 접착수 및 surface morphology를 관찰하였다.
결과로써, hydroxylation 및 phosphorylation을 진행한 시료들은 ATR-FTIR 측정 결과 O-H peak, P-O stretch, 및 P=O stretch 등 표면 개질에 의한 화학적 결합 변화를 관찰할 수 있었다. 또한 SEM image를 통하여 HA를 도입한 시료들은 기존에 존재하지 않던 구형의 결정형 상태로 HA가 존재함을 알 수 있었고, XPS를 통하여 칼슘, 인 등 HA에만 존재하는 원소들이 PEEK 표면에서 확인되었다. 또한 water contact angle 측정 결과 역시 biomineralization 진행 일수에 비례하여 접촉각이 감소하는 경향을 보였으며, 표면의 미세 조도 역시 증가하는 경향을 보여 biomineralization을 통해 HA 결정이 성장함을 확인할 수 있었다. 추가적으로 Alizarin red의 색 염색으로 인한 발색 정도를 확인함으로써 biomineralization이 진행된 시료의 표면에서 HA의 존재 여부를 확인할 수 있었다.
이후 표면 개질이 진행된 PEEK 표면에 human osteoblast(HOb) 세포를 배양하였으며, 관측 결과 표면 개질이 진행된 시료에서 접착된 세포의 수가 증가하는 경향을 보였다.
