    Introduction
    Multicenter consortium for human or porcine islet transplantation and research is preferable around a single islet isolation facility. In this study, we investigated the optimal method for transportation of isolated porcine islet from the isolation facility to the transplant hospital or research center in terms of temperature, oxygen supply and shaking effect.
    Methods
    Commercially available insulator boxes with thermo-regulators were used as a container to maintain an internal temperature close to target value of 22 ℃. Experimental containers were exposed for 5 hours under two external temperature (4 and 37 °C) and monitored using HOBO® temperature loggers. To compare the effects of transportation devices on the islets, we compared conical tubes, gas-permeable bags (GPB) and gas-permeable flasks (GPF). After 1, 3 and 5 hours, islet counts, AO/PI staining, assessment of ADP/ATP ratio and glucose-stimulated insulin secretion (GSIS) were performed. To evaluate the shaking effect on islets, we also analyzed the difference between the control group and shaking group in each device with time.
    Results
    Commercially available Styrofoam insulators with thermo-regulators were able to maintain the internal temperature near the target temperature. Islet recovery rate for GPF was higher than other devices and was maintained with time, while that for conical tube and GPB decreased with time. ADP/ATP ratio for GPF was lower than other devices, although statistical significance was not shown. AO/PI ratio for GPF was higher than other devices after 5 hours. Assessment of endocrine function by GSIS was not different between the 3 devices. Compared to the control group, the shaking group was comparable in islet survival, viability and function, except AO/PI ratio and GSIS after 5 hours.
    Conclusion
    Our study demonstrated that the use of commercially available insulator boxes with thermo-regulators can maintain internal temperature close to target value and GPF is more favorable for islet oxygenation during transportation. This study also showed the effect of shaking on isolated porcine islets is negligible during transportation. However, in vivo study for islet transplantation are needed to confirm these results.
