정서장애에 대한 단일화된 범진단적 치료 프로토콜(unified protocol for transdiagnostic treatment of emotional disorders: UP)은 정서에 초점을 둔 인지행동치료로서 수년 전부터 국외에서 그 효능과 효과가 보고되고 있다. 하지만, 국외와 달리 국내에서는 UP와 관련된 연구가 수행되기 시작한 것은 불과 최근 일이며, UP의 효과를 더 검증하고 그 변화기제를 규명하기 위한 추가적인 연구의 필요성이 대두되었다. 이에 따라 임상 환자 집단을 대상으로 집단 형태의 UP를 제공한 후, 그 효과를 개인치료 형태의 UP와 비교 검증하고, 준임상 집단 및 임상 집단을 대상으로 한 UP의 효과가 어떤 기제를 통하여 나타나는지를 규명하고자 세 가지 연구를 수행하였다.
연구 1의 주요 목적은 집단 형태의 UP 효과를 검증하는 것이며, UP의 개인치료와 비교 검증하고자 하였다. 주 진단이 사회불안장애, 주요우울장애, 지속성 우울장애 또는 강박장애인 진단적으로 이질적인 임상 표본 중에서 21명과 22명이 각각 8회기로 구성된 UP 집단치료와 UP 개인치료를 제공받았으며, 이들은 모두 치료 전 및 치료 후 평가와 3개월 추후 평가를 완료하였다. 연구 결과, UP 집단 및 개인치료 조건 모두 사회불안증상, 범불안증상, 공황장애 심각도, 우울증상, 임상적 불안증상, 부적 정서, 일상적 기능의 손상 및 임상적 심각도 평정의 측정치들이 치료 전과 치료 후 간에, 치료 전과 추후 간에 유의하게 감소하였다. 정적 정서 측정치 상에서 UP 치료 포맷 간 차이는 세 시기 모두에서 유의하지 않았으나, 개인치료 조건 참가자들은, 집단치료 조건과 달리, 치료 전에 비해 치료 후 및 추후에 정적 정서가 유의하게 증가하였다. 임상적 유의성 검증 결과, UP 집단치료와 개인치료 조건 간에 유의한 차이는 없었다.
연구 2와 3의 주요 목적은 UP의 변화기제를 검증하는 것이다. 먼저, 연구 2에서는 선행연구들과 문헌에 기초하여, 사회불안증상, 범불안증상, 우울증상 및 부적 정서에 대한 단기 단일화된 범진단적 개입의 효과가 정서조절곤란의 감소에 의해 매개될 것이라는 가설을 상정하고 검증하였다. 준임상적 수준의 다양한 정서장애 증상들을 호소하는 대학생 62명이 UP에 기초하여 5회기로 구성된 단기 범진단적 개입(n = 31)과 심리교육 개입(n = 31)을 제공받았으며, 이들은 모두 치료 전 및 치료 후 평가를 완료하였다. 연구 결과, 심리교육에 비해 단기 단일화된 범진단적 개입은 사회불안증상, 범불안증상, 우울증상 및 부적 정서를 완화하는 데 있어 더 큰 효과를 보였다. 나아가, 앞의 네 증상들에 대한 단기 단일화된 범진단적 개입의 효과는 정서조절곤란의 감소에 의해 완전 매개되었다.
연구 3에서는 UP의 효과를 설명하는 변화기제를 규명하기 위하여, 선행연구들과 문헌에 기초하여, 사회불안증상, 범불안증상, 공황장애 심각도, 우울증상, 부적 정서, 일상적 기능의 손상 수준 및 주 진단의 임상적 심각도에 대한 UP의 효과가 정서조절곤란의 감소에 의해 매개될 것이라는 가설을 상정하고 검증하였다. 연구대상은 DSM-5의 여러 종류의 불안 및 우울장애 중에서 적어도 1가지 이상의 진단을 충족시킨 대학생들이며, 단일화된 범진단적 치료를 제공받은 참가자 44명과 대기통제조건에 속한 참가자 15명이 8주 간격의 사전 및 사후 평가와 3개월 추후 평가를 모두 완성하였다. 연구 결과, 사회불안증상 및 주 진단의 임상적 심각도에 대한 UP의 효과는 정서조절곤란의 감소에 의해 부분 매개되었고, 다섯 가지 다른 결과 측정치에 대한 UP의 효과는 정서조절곤란의 감소에 의해 완전 매개되었다.
결론적으로, 본 연구 결과들은 국내 정서장애 대학생들을 대상으로 한 집단 형태의 UP는 정적 정서를 제외한 거의 모든 종속측정치들에서 개인치료 형태의 UP와 비견될 정도로 효과적임을 시사한다. 아울러, 준임상 집단 및 임상 집단을 대상으로 한 UP의 변화기제로서 정서조절곤란의 매개역할을 지지하는 증거를 국내에서 처음으로 제공하였다. 끝으로, 이 연구들의 제한점과 추후 연구방향, 그리고 연구 및 이론적 측면, 방법론적 측면 및 임상 실무의 측면에서 이 연구들의 여러 가지 의의에 관해 논의하였다.

Unified protocol for transdiagnostic treatment for emotional disorders(UP) is a emotion-focused cognitive behavioral therapy, and its efficacy and effects have been reported abroad for several years. However, in Korea, a few UP studies have only recently begun to be conducted. Further research is needed to examine the effects of the UP and identify the mechanisms of change in the UP. This dissertation describes three studies to compare the UP in a group format with UP individual treatment for Korean undergraduates with emotional disorders, and shed light on the mechanism of change in the UP for Korean subclinical and clinical groups with emotional disorders.
The main purpose of Study 1 was to examine the effects of the UP group treatment, and to compare it with UP individual treatment. Participants were diagnostically heterogeneous patients with a principal diagnosis of social anxiety disorder, major depressive disorder, persistent depressive disorder, or obsessive-compulsive disorder. Among them, 21 and 22 participants received an 8-session UP group and individual treatments, respectively, and completed outcome assessments at pre- and post-treatment and 3-month follow-up. The UP group and individual treatments produced in significant improvements on the several outcome measures including clinical severity rating of a principal diagnosis at both posttreatment and follow-up. On the positive affect measure, participants in the individual treatment condition showed a significant increase in positive affect from baseline to post-treatment and follow-up, but those in the group treatment condition did not. On any clinically significant change indices, the UP group treatment was not significantly different from the UP individual treatment.
The main purpose of Study 2 and Study 3 is to shed light on the mechanism of change in the UP. First, in Study 2, we proposed emotion dysregulation as a potential mediator of the effects of the brief transdiagnostic intervention for social anxiety, generalized anxiety, depression symptoms, and negative emotions, based on previous findings and literature review, and then examined its mediating effects. Sixty-two undergraduate students complaining of various emotional disorder symptoms at a subclinical level were provided with a brief unified transdiagnostic intervention(n=31) and a psychoeducational intervention (n=31), and completed outcome assessments at pre- and post-treatment. Compared to the participants in the psychoeducation condition, those in the brief unified, transdiagnostic intervention condition demonstrated significant improvements in social anxiety, generalized anxiety, depression symptoms, negative emotion, and emotion dysregulation. Moreover, change in emotion dysregulation fully mediated its efficacy on all of the outcome measures.
The Study 3 was conducted to shed light on the mechanism of change in the unified protocol for transdiagnostic treatment for emotional disorders(UP). Based on previous findings and literature review, we hypothesized that reduction in emotion dysregulation would act as a mediator of the effects of the UP for social anxiety, generalized anxiety, depression symptoms, panic disorder severity, negative affect, impairment in daily functioning, and clinical severity rating of a principal diagnosis and then examined its mediating effects. Participants were undergraduate students with at least one DSM-5 diagnosis of various anxiety and depressive disorders. Forty-four participants who received individual or group UP treatments and 15 those in the wait-list control condition completed outcome assessments at pre- and posttreatment and 3-month follow-up. Consistent with our hypothesis, bootstrapping-based mediational analyses demonstrated that reduction in emotional dysregulation partially mediated the effects of the UP on social anxiety symptoms and clinical severity rating of a principal diagnosis and also fully mediated its effect on generalized anxiety, depression symptoms, panic disorder severity, negative affect, and impairment in daily functioning.
In conclusion, these results provide support for the effects of the UP group treatment comparable to the UP individual treatment for Korean undergraduates with emotional disorders on almost all outcome measures, except for positive affect. In addition, these studies provide the first evidence to support the mediating role of emotional dysregulation as a change mechanism for UP in Korean subclinical and clinical groups. Lastly, limitations of these studies and future research directions, as well as their several implications in terms of research and theory, methodology, and clinical practice are discussed.
