    알츠하이머병(Alzheimer’s disease, AD)은 아밀로이드 베타(Aβ)와 타우(tau) 병리, 신경염증, 대사 이상 등 복합적인 병인 기전을 가지며, 현재까지 승인된 치료제들은 그 효과가 제한적이다. 이에 개발 비용과 시간을 절감할 수 있는 약물 재창출(drug repurposing)이 유망한 전략으로 대두되고 있다. 본 학위논문에서는 임상 역학적 근거(retrospective clinical analysis-based approach)와 시스템 생물학적 네트워크 분석(network-based approach)이라는 서로 다른 두 가지 약물 재창출 전략을 통해 선정된 약물들의 효능과 기전을 규명하고, 이를 통해 AD 병리 조절의 새로운 가능성을 제시하고자 하였다.
    파트 1에서는 당뇨와 치매의 역학적 연관성에 기반하여 SGLT2 저해제인 enavogliflozin을 AD에 적용하며 효능을 평가하고자 했다. 5XFAD 마우스 모델을 통해 해당 약물이 뇌 내 미세아교세포의 에너지 대사를 재편성(metabolic reprogramming) 한다는 것을 확인하였다. Enavogliflozin은 미세아교세포의 식세포 작용을 회복시켜 Aβ 병리를 감소시키고, 시냅스 기능 및 인지 저하를 완화하는 결과를 확인하였다.
    파트 2에서는 AD 관련 다중오믹스 데이터를 통합한 네트워크 기반 스크리닝(network-based screening)을 통해 도출된 abemaciclib의 효능을 평가하였다. 아밀로이드와 타우 병리가 공존하는 dKI 마우스 (double knock-in mouse model, dKI) 및 환자 유래 뇌 오가노이드 모델을 활용하여 검증한 결과, abemaciclib은 본래 표적인 CDK4/6 경로와는 독립적으로 작용하여, 주요 타우 인산화 효소인 CaMKII와 GSK3β의 활성을 직접적으로 억제하는 동시에 자가포식(autophagy) 유동을 활성화하여 축적된 타우의 분해를 촉진하는 이중 기전을 나타냈다. 이는 abemaciclib이 인지 기능과 직접적으로 연관이 있는 타우 병리를 완화하여 신경 세포 사멸을 막고 인지 기능을 보존하는 데 기여할 수 있음을 보여주었다.

This dissertation aims to elucidate the efficacy and mechanisms of candidate drugs identified through two distinct repurposing strategies: a retrospective clinical analysis-based approach and a network-based approach utilizing systems biology, thereby presenting novel possibilities for modulating AD pathology.
    Part I investigates enavogliflozin, an SGLT2 inhibitor selected based on epidemiological links between diabetes and dementia. Pharmacological intervention in 5XFAD mice revealed that enavogliflozin drives metabolic reprogramming in microglia, restoring phagocytic capacity and enhancing Aβ clearance. These cellular adaptations translated to preserved synaptic integrity and attenuated cognitive deficits, establishing microglial energetics as a therapeutically druggable target.
    Part II examines abemaciclib, prioritized through network-based integration of AD multi-omics datasets. Mechanistic investigation in AppNL-F/MAPT double knock-in (dKI) mice and AD patient-derived brain organoids demonstrated that abemaciclib mitigates tau pathology through CDK4/6-independent mechanisms. Abemaciclib directly inhibited CaMKII and GSK3β, principal tau kinases, while concurrently activating autophagic flux to promote clearance of phosphorylated tau proteins. This dual mechanism effectively reduced neuronal loss and preserved cognitive function in models recapitulating both amyloid and tau pathologies.
    Collectively, these studies validate complementary repurposing strategies as viable frameworks for identifying disease-modifying therapeutics. Cross-validation in transgenic mouse models and human-derived systems strengthens mechanistic interpretation and translational potential, demonstrating that drug repurposing strategies can serve as valuable approaches for developing Alzheimer's disease therapeutics.
