노인장기요양보험제도에서의 케어매니지먼트 기능 분석 = A Study on Care Management Function in the Long-Term Care Insurance for Elder

국문 초록 (Abstract)

본 연구는 노인장기요양보험제도 내에서 이용자 중심의 서비스가 이루어지기 위해서는 케어매니지먼트가 중요하다는 관점으로부터 케어매니지먼트 기능을 둘러싼 제규정과 장기요양관리요원의 역할을 중심으로 현행 노인장기요양보험제도에서의 케어매니지먼트 기능을 분석해보고자 하는 것이다. 이를 위해 케어매니지먼트 기능을 둘러싼 제규정의 분석으로는 케어매니지먼트 기능 관련 제규정과 장기요양관리요원의 자격요건, 교육 및 연수 관련 제규정에 대해 살펴보았다. 그리고 장기요양관리요원의 역할 분석을 위해서는 국민건강보험공단 장기요양관리요원으로 경남지역에서 근무하고 있는 직원 두 명을 대상으로 노인장기요양보험제도 내에서 이용자 중심의 케어매니지먼트와 관련하여 장기요양관리요원이 케어매니지먼트 기능을 어떻게 수행하고 있는지에 대해 집중 인터뷰를 실시하였다. 이러한 연구의 결과는 다음과 같다.
첫째, 케어매니지먼트 기능을 둘러싼 제규정과 관련해서는 케어매니지먼트 기능 관련 제규정과 장기요양관리요원의 자격요건, 교육 및 연수 관련 제규정이 체계적으로 제도화되어 있지 못하다는 것을 알 수 있었다. 둘째, 장기요양관리요원의 역할에 대한 분석은 먼저, 장기요양 이용절차와 관련하여 보면, 장기요양관리요원의 구체적인 업무에 관한 명확한 법적 기준이 없으며, 과중한 업무로 인해 ‘공급자 중심의 전달자’로서 이용자 중심의 케어매니지먼트 기능을 수행하기에 어려움이 있음을 알 수 있었다. 또한 표준장기요양이용계획서와 관련하여 보면, 대상자의 다양한 욕구와 필요정도에 초점을 맞추어 계획되는 것이 아니라 대상자의 장기요양등급별, 급여 종류별 상한제 등 관리운영측면을 더 중시하여 대상자의 욕구를 충분히 반영하지 못한 채 표준장기요양이용계획서가 작성되고 있는 실정임을 알 수 있었다. 그리고 예방중심 및 지역사회자원 연계 사업과 관련하여 보면, 장기요양관리요원이 관리해야 할 대상자가 너무 많고 시·군·구와의 협력 및 인식부족 등으로 인해 형식적인 행정업무위주로 이루어지고 있어 이용자 중심의 전달자로서 서비스 제공이 제대로 수행되지 못하고 공급자 위주의 편의주의 식으로 서비스가 제공되고 있음을 엿볼 수 있었다.
이상과 같은 분석결과에 근거하여 이용자 중심의 케어매니지먼트 기능 제도화를 위한 개선방안을 제시하면 다음과 같다. 첫째, 케어매니지먼트 기능 관련 제규정의 체계적인 정비로서 장기요양관리요원의 명칭과 의미, 업무의 유형과 내용, 그리고 자격요건 및 교육·연수 관련 제규정에 대해 법적인 규정이 필요하다는 것이다. 둘째, 장기요양관리요원의 역할이 ‘공급자 중심의 전달자’로부터 ‘이용자 중심의 전달자에로’ 전환되어야 한다는 것이다.

다국어 초록 (Multilingual Abstract)

This study is giving the survey of Care Management function in the Long-Term Care Insurance for Elder by all the regulations relevant to care management functions and roles of care management in order to practically provide with a service centering on users in the Long-Term Care Insurance for Elder. All the regulations relevant to care management functions have been thoroughly analyzed, the regulations relevant to care management functions and qualifications, education and training of care managers. And a survey on roles of care manager was carried out by having a personal interview with two care managers who are working in South Gyeongsang Province of National Health Insurance Corporation, concerning how they are conducting the care management functions that were constituted by focusing on user's right in the Long-Term Care Insurance for elder. The survey results are summarized as follows.
The first, there no the systematic institutionalization of all the regulation relevant to care management function and qualifications, education and training of care managers. The second, roles of care managers analyzed, the utilization procedures of Long-term care facilities: They have a restriction to conduct the care management functions by focusing on user's right since they are overburdened in the affairs caused by that there are no clear legal standards in the affairs of care manager regarding the utilization procedures of long-term care facilities. The standard long-term care utilization plan: A service is practically provided not by having satisfied users with their needs since the standard long-term care utilization plan has been prepared not by focus on a variety of needs and need levels in each of users but having focused on administration of the long-term care grades and the limit of supply. The prevention focus and community resources link: A service is provided not by focusing on centering on user, they have so many users to manage, and there are significant lacks of cooperation and recognition from Province·County·District Offices.
The way improving institutionalization, which is summarized in the following;
The first, priority will be given to systematically reorganize all the regulations relevant to care management functions for the meaning, the affairs and qualifications, education and training of care managers. The second, roles of care managers will be required to convert from centering on provider to centering on user.
