Chronic obstructive pulmonary disease (COPD) is characterized by progressive declines in physical activity. Although pulmonary rehabilitation (PR) is a core component of management, its real-world impact varies widely and strategies to improve physical activity remain limited.
This randomized controlled trial evaluated whether structured clinician phone-call interventions could enhance physical activity in COPD patients participating in a home-based PR program supported by wearable activity trackers. Subgroup analyses were additionally performed according to baseline physical activity levels to examine differential intervention effects. Qualitative interviews were conducted to further explore factors influencing engagement in PR, and the transcripts were analyzed using thematic analysis.
While intervention did not produce significant overall group differences, its impact was substantially greater in the “Do do” group, participants with higher baseline physical activity, with notable gains in steps, activity time, and MVPA. Qualitative findings revealed that dispositional motivation, caregiver involvement, and structured daily life in “Do do” group, whereas fear-driven symptom interpretation, low support, and disrupted routines hindered participation in the “Don’t do” group.
Structured clinician intervention was particularly effective among patients with higher baseline physical activity, with motivation and psychosocial context shaping differential responsiveness. These findings underscore the need for personalized PR strategies that incorporate patients’ behavioral readiness and social environments.
