서론 사이토크롬 P450 2D6 (CYP2D6) 효소는 벤라팍신을 포함한 다양한 승인 의약품의 대사에 중요한 역할을 한다. CYP 효소의 유전적 다형성과 병용 약물에 의한 상호작용은 약물–약물–유전자 상호작용을 유발할 수 있다. 본 연구의 목적은 CYP2D6 기질인 벤라팍신과 벤라팍신의 활성 대사체인 O-데스메칠벤라팍신(ODV)의 CYP2D6 관련 약물-약물-유전자 상호작용을 정량적으로 평가하기 위한 생리학적 기반 약물동태(PBPK) 모델을 개발 및 검증하는 데 있다.
방법 벤라팍신과 ODV의 생리학적 기반 약물동태학 모델은 Simcyp Simulator를 이용하여 구축하였다. 약물의 물리화학적 특성, 용출 및 흡수 파라미터, 시험관 내 미소체 연구 결과, 신배설 청소율 값은 문헌 및 공개 자료에서 수집하였다. 예측 모델의 최적화는 전향적 임상시험에서 확보한 약동학 데이터를 통합하여 수행하였다. 벤라팍신의 약물-약물-유전자 상호작용 평가를 위해 전향적 임상시험이 수행되어 파록세틴 병용 전후로 벤라팍신 서방형 제형이 투여되었다. 약동학 평가를 위해 벤라팍신 투여 후 혈장 시료가 채취되었다. 약동학 파라미터는 CYP2D6 표현형, CYP2D6 활성도 점수(activity score), 파록세틴 병용 여부에 따라 요약되었다. 최종 모델은 기존에 발표된 임상 연구 자료를 이용하여 외부 검증을 거쳤으며, 이후 서울대학교병원 데이터웨어하우스로부터 확보한 실제 임상 환자 데이터를 기반으로 한 시뮬레이션에 적용되었다.
결과 전향적 임상시험에 20명의 한국인 CYP2D6 정상대사자(normal metabolizer) 또는 중간대사자(intermediate metabolizer)가 참여하였다. CYP2D6 정상대사자에서 중간대사자에 비해 벤라팍신의 노출은 높았으며, 반대로 ODV의 노출은 낮았다. 파록세틴에 의한 CYP2D6 억제 이후에는 이 차이가 감소하였다. CYP2D6 표현형 만큼은 아니지만 CYP2D6 활성도 점수도 벤라팍신과 ODV의 약동학적 특성에 영향을 주었다. 최초 PBPK 모델은 벤라팍신의 대사를 과소 추정하는 경향이 있어 관측값에 비해 벤라팍신의 노출은 높고 ODV의 노출은 낮았다. 전향적 임상시험에서 얻은 약동학 데이터를 바탕으로 미소체 비결합 분율(fumic) 값을 조정하여 대사능을 증량하였다. 아담(ADAM) 흡수 모델, full PBPK 모델 및 효소 동역학을 통합한 최종 PBPK 모델은 사이토크롬 P450 2D6 표현형별 벤라팍신과 ODV의 약물동태학적 특성과 파록세틴에 의한 효소 억제 후의 약동학 변화를 성공적으로 재현하였다. 최종 PBPK 모델은 외부 검증을 통해 추가적으로 평가되어 다양한 인구집단과 느린대사자(Poor metabolizer)에서도 벤라팍신과 ODV의 약동학 양상을 성공적으로 예측함을 확인하였다. 벤라팍신 및 ODV의 노출량의 합은 연령 증가에 따라 상승하였으며, 느린대사자에서 가장 높게 나타났다. 또한 파록세틴에 의한 대사 억제 효과는 고령 환자에서 더욱 두드러져, 독성 위험이 증가함을 시사하였다.
결론 본 연구에서 중간 단계 기반 접근법(middle-out approach)을 통해 구축한 생리학적 기반 약물동태학 모델은 벤라팍신 및 ODV의 CYP2D6 표현형에 따른 약물-약물-유전자 상호작용을 효과적으로 예측하였다. 생리학적 기반 약물동태학 모델은 약물-약물-유전자 상호작용 평가에 있어 기존 임상시험의 보완적인 도구가 될 수 있을 것으로 기대된다. 고령 환자에서 고용량 벤라팍신 투여는 안전하고 효과적인 약물 치료를 위해 신중하게 고려되어야 한다.

Introduction CYP2D6 plays a crucial role in the metabolism of approved drugs, including venlafaxine. Genetic polymorphisms and concomitant drugs can lead to drug–drug–gene interactions (DDGIs). This study aimed to develop and validate a physiologically based pharmacokinetic (PBPK) model to characterize CYP2D6 DDGIs of CYP2D6 substrate venlafaxine and its active metabolite, O-desmethylvenlafaxine (ODV).
Methods A PBPK model for venlafaxine and ODV was constructed using the Simcyp Simulator. Physicochemical parameters, dissolution and absorption parameters, in vitro microsome study results, and renal excretion clearance value were derived from literatures and public sources. Model optimization was performed by integrating PK data from a prospective clinical trial. In a prospective trial evaluating DDGIs of venlafaxine, extended release form of venlafaxine was administered before and after paroxetine co-administration. Plasma samples for PK analysis were collected after each venlafaxine administration. PK parameters were summarized according to CYP2D6 phenotype, activity score, and concomitant paroxetine administration. The final model was externally validated using published clinical studies, and subsequently applied to simulate PK profiles in real-world patient populations derived from the Seoul national university hospital data warehouse.
Results A total of 20 healthy Korean normal or intermediate metabolizers (NMs, IMs) were enrolled in the clinical trial. Venlafaxine exposure was higher but ODV exposure was lower in NMS compared with IMs. After CYP2D6 inhibition by paroxetine, the difference was attenuated. Not as significant as difference observed between phenotypes, CYP2D6 activity score influenced PKs of venlafaxine and ODV. Initial PBPK model tended to underestimate metabolism of venlafaxine, with higher venlafaxine and lower ODV exposure compared with observed data. The metabolic capacity was increased by adjusting fumic as the scaling factor based on the clinical trial PK data. The final PBPK model incorporating ADAM, full PBPK, and enzyme kinetics successfully described venlafaxine and ODV PKs across CYP2D6 phenotypes and following paroxetine-mediated CYP2D6 inhibition. The final PBPK was further evaluated by external validation, where it successfully predicted PKs of venlafaxine and ODV across different populations and in CYP2D6 poor metabolizers (PMs). Combined exposure of venlafaxine and ODV increased with age and was highest in CYP2D6 PMs. The impact of CYP2D6 inhibition by paroxetine was more prominent in geriatric patients, indicating elevated risk of toxicity.
Conclusion The middle-out PBPK modeling approach effectively characterized DDGIs of CYP2D6 substrate venlafaxine and its metabolite, ODV. PBPK modeling may serve as a complementary tool to conventional clinical trials in the assessment of DDGIs. Given the increased systemic exposure in geriatric patients, high-dose venlafaxine should be carefully managed to ensure safe and effective pharmacotherapy.
