배경 및 목적: 직업성 소음은 근로자의 신체 및 정신 건강에 영향을 미치며 청력 손실을 유발한다. 이 연구는 boothless 환경에서 청력보호구(hearing protection devices, HPDs) 착용 습관이 청력 역치(hearing threshold level, HTL)와 외유모세포 기능에 미치는 영향을 확인하고, 변조이음향방사(distortion product OAE, DPOAE)가 청력보존프로그램(hearing conservation program, HCP)의 조기 선별 및 추적 관리를 위한 관리 지표로 활용 가능한지 평가하는 것을 목표로 하였다.
대상 및 방법: 소음 작업장 근로자 26명(여:남=1:25)을 대상으로 순음청각선별(pure tone screening, PTS; 500–6,000 Hz)과 DPOAE 신호대잡음비(signal-to-noise ratio, SNR)(f2기준: 842–6,726 Hz)을 측정하였다. 근무 경력은 10년 미만, 10년 이상 – 20년 미만, 20년 이상으로 HPD 착용 시간은 1시간 미만, 1시간 이상 – 4시간 미만, 4시간 이상 – 8시간 미만, 항상 착용으로 분류하였다. 통계는 일원분산분석(one-way ANOVA), 피어슨 상관분석(Pearson correlation), 다변량분산분석(MANOVA)으로 주효과와 상호 작용 효과를 검증하였다.
결과: HPDs 착용 습관에서 ‘항상 착용’ 비율은 15%로 낮고, 2시간 미만의 응답이 과반을 넘어 간헐적인 양상을 보였다. 근무 경력 증가에 따라 HTL은 500 Hz 제외한 모든 주파수에서 상승하였고(p < .05), DPOAE SNR은 전반적으로 감소했으며, 2,002, 3,369 Hz에서 유의하였다(p < .05). HPD 착용 시간에 따라 HTL 1,000, 3,000, 4,000, 6,000 Hz 및 hf-PTAs에서 유의하였고(p < .05). DPOAE SNR은 3,369 Hz 및 hf-SNR에서 유의하였다(p < .001). PTS와 DPOAE 간 상관분석은 중간 및 고주파수 모두 유의한 음의 상관관계(r = -.671, p < .001)이었다.. 근무 경력과 HPD 착용의 상호 작용은 HTL의 3,000, 6,000 Hz과 DPOAE SNR의 4,004 Hz, hF-SNR에서 유의하였다(p < .05).
요약 및 결론: HPDs의 착용 가중 시간은 고주파수(3-6 kHz) HTL 및 DPOAE 보존에 영향을 미친다. HPDs의 지속적 사용은 DPOAE SNR이 평균 3 dB 이상 높게 나타났으며, HTL 상승이 유의하게 적었다. DPOAE는 2–4 kHz 주파수 범위에서 SNR가 안정적으로 유지되었으며, boothless 환경에서도 재현성이 높게 나타났다. PTS와 병행 시 근로자의 조기 청력 변화를 선별하고 추적 관리에 활용할 수 있는 지표로 확인되었다.

Background and Objectives: Occupational noise affects workers physical and mental health and can lead to hearing loss. This study examined the effects of hearing protection device (HPD) wearing habits on hearing threshold levels (HTLs) and outer hair cell function using hearing tests conducted under boothless conditions. In addition, the study assessed whether distortion product otoacoustic emissions (DPOAEs) could be used as an indicator for early screening and follow-up management in hearing conservation program (HCPs).
Subjects and Methods: Twenty-six noise-exposed workers (female:male = 1:25) underwent pure tone screening (PTS; 500–6,000 Hz) and distortion-product otoacoustic emission (DPOAE) signal-to-noise ratio (SNR) (f₂: 842–6,726 Hz). Participants were grouped by work tenure (<10 years, 10–20 years, ≥20 years) and HPD wearing time (<1 hour, 1–4 hours, 4–8 hours, always). Statistical analyses included one-way ANOVA, Pearson correlation, and MANOVA to verify main and interaction effects.
Results: Only 15% of participants reported always wearing HPDs, while more than half wore them intermittently for less than 2 hours per day. Hearing threshold levels increased significantly with work tenure at most frequencies, whereas DPOAE signal-to-noise ratios decreased, with significant reductions at 2,002 and 3,369 Hz (p < .05). HPD wearing time significantly affected HTLs at mid- to high frequencies and high-frequency pure-tone averages, as well as DPOAE SNRs at 3,369 Hz and high-frequency SNRs (p < .05). A significant negative correlation was observed between pure-tone thresholds and DPOAE SNRs in the mid- and high-frequency ranges (r = −.671, p < .001).
Summary and Conclusion: Extended HPD use was associated with better preservation of high-frequency (3–6 kHz) hearing thresholds and DPOAE responses. Continuous HPD use showed higher DPOAE SNRs and lower HTL elevations. DPOAE SNRs in the 2–4 kHz range remained stable and reproducible under boothless conditions. These findings suggest that DPOAE, combined with pure-tone screening, may serve as a useful index for early detection and longitudinal monitoring of hearing changes in noise-exposed workers.
