        Validation of the numerical simulations of flow around a scaled-down turbine using experimental data from wind tunnel

        Aerodynamic characteristic of a small scale wind turbine under the influence of an incoming uniform wind field is studied using k-ω Shear Stress Transport turbulence model. Firstly, the lift and drag characteristics of the blade section consisting of S826 airfoil is studied using 2D simulations at a Reynolds number of 1×105. After that, the full turbine including the rotational effects of the blade is simulated using Multiple Reference Frames (MRF) and Sliding Mesh Interface (SMI) numerical techniques. The differences between the two techniques are quantified. It is then followed by a detailed comparison of the turbine's power/thrust output and the associated wake development at three tip speeds ratios (λ = 3, 6, 10). The phenomenon of blockage effect and spatial features of the flow are explained and linked to the turbines power output. Validation of wake profiles patterns at multiple locations downstream is also performed at each λ. The present work aims to evaluate the potential of the numerical methods in reproducing wind tunnel experimental results such that the method can be applied to full-scale turbines operating under realistic conditions in which observation data is scarce or lacking.

      • Frequency of Chromosomal Abnormalities in Pakistani Adults with Acute Lymphoblastic Leukemia

        Background: The difference in prognosis of adult and childhood acute lymphoblastic leukemia (ALL) can be attributed largely to variation in cytogenetic abnormalities with age groups. Cytogenetic analysis in acute leukemia is now routinely used to assist patient management, particularly in terms of diagnosis, disease monitoring, prognosis and risk stratification. Knowing about cytogenetic profile at the time of diagnosis is important in order to take critical decisions in management of the patients. Aim and Objectives: To determine the frequency of cytogenetic abnormalities in Pakistani adult patients with ALL in order to have insights regarding behavior of the disease. Materials and Methods: A retrospective analysis of all the cases of ALL (${\geq}15$years old) diagnosed at Aga Khan University from January 2006 to June 2014 was performed. Phenotype (B/T lineage) was confirmed in all cases by flow cytometry. Cytogenetic analysis was made for all cases using the trypsin-Giemsa banding technique. Karyotypes were interpreted using the International System for Human Cytogenetic Nomenclature (ISCN) criteria. Results: A total of 166 patients were diagnosed as ALL during the study period, of which 151 samples successfully yielded metaphase chromosomes. The male to female ratio was 3.4:1. The majority (n=120, 72.3%) had a B-cell phenotype. A normal karyotype was present in 51% (n=77) of the cases whereas 49% (n=74) had an abnormal karyotype. Of the abnormal cases, 10% showed Philadelphia chromosome; t(9;22)(q34;q11.2). Other poor prognostic cytogenetic subgroups were t(4;11)(q21;q23), hypodiploidy (35-45 chromosomes) and complex karyotype. Hyperdiploidy (47-57 chromosomes) occurred in 6.6%; all of whom were younger than 30 years. Conclusions: This study showed a relatively low prevalence of Philadelphia chromosome in Pakistani adults with ALL with an increase in frequency with age (p=0.003). The cumulative prevalence of Philadelphianegative poor cytogenetic aberrations in different age groups was not significant (p=0.6).

        Safety and efficacy of bendamustine in the conditioning regimen for autologous stem cell transplantation in patients with relapsed/refractory lymphoma

        BackgroundBendamustine is an attractive option for the management of both de novo and relapsed lymphomas. It is being increasingly used in the conditioning regimen for autologous stem cell transplantation (SCT) and can be an alternative to the traditionally-used carmustine. In this study, we aimed to determine the safety and efficacy of bendamustine in the con-ditioning regimen for autologous SCT in refractory/relapsed lymphomas.MethodsWe designed a descriptive study to evaluate bendamustine in combination with etopo-side, cytarabine, and melphalan (BeEAM) in the conditioning regimen for autologous SCT.ResultsFourteen patients (median age, 28 yr) with Hodgkin’s lymphoma (HL) (N=8), non-Hodgkin’s lymphomas (NHL) (N=5), or peripheral T-cell lymphoma, not otherwise specified (PTCL NOS) (N=1) were included in the study. A median number of 5.95×106CD34+ cells/kg were transfused. Median times to absolute neutrophil count and platelet engraftment were 17 days and 24 days, respectively. The 100-day transplantation mortal-ity rate was 28% (4 patients). Eight patients (57.14%) had GII-III acute kidney injury, four patients (28.5%) had GIII-IV hyperbilirubinemia, and twelve patients (85%) had GII-III diarrhea. After 3 months, 37% (5 patients) and 21.4% (3 patients) demonstrated complete response and partial response, respectively. The median follow-up was 5.5 months (15 days‒19 mo). At the final follow-up, 7 patients (50%) were alive and in CR.ConclusionOur study showed that bendamustine is a potentially toxic agent in the conditioning regi-men for autologous SCT, resulting in significant liver, kidney, and gastrointestinal toxicity. Further studies are required to assess its safety and efficacy at reduced doses.
