      • Tuning anti-microbial activity of poly(4-vinyl 2-hydroxyethyl pyridinium) chloride by anion exchange reactions.

        <P>A series of new bioactive polymers with pendant choline analogous group was prepared by anion exchange reaction direct at the quaternary nitrogen of the polycation. Poly(4-vinyl 2-hydroxyethyl pyridinium) chloride was prepared in situ by simultaneous polymerization and quaternization of 4-vinyl pyridine with 2-chloroethanol that also acts as catalyst. The counter anion (Cl(-)) of the polycation was exchanged by anion exchange reaction with Br(-), (-)OH, (-)SH, NO(3) (-), BF(4) (-) or CF(3)COO(-). Evidence of anion exchange was obtained by the characterization of the resultant polymers. The nature of the counter anion has profound effect on their properties including strong anion-dependent anti-microbial activity against bacteria and fungus. Polymer containing (-)OH was observed to be the most potent anti-microbial agent with the lowest minimum inhibitory concentration against both the classes of microbes studied.</P>

        The paper includes the growth of tourism since years in Maldives. The paper examines tourist development in Maldives. This will include the tourism indicators and impacts of tourism economically, socially and culturally. Although social/cultural impacts from coastal tourism development have been widely recognized in a great number of cases, it is relatively, difficult to develop technical criteria, for evaluating these impacts in quantitative terms. According to the Ministry of Tourism website, the emergence of tourism in 1972 transformed the economy of the Maldives, moving rapidly from the dependence on the fisheries sector to the tourism sector. Just in three and a half decades, the industry has become the main source of income and livelihood of the people of the Maldives. Tourism is also the country's biggest foreign currency earner and the single largest contributor to the GDP. Today, there are 89 resorts in the Maldives with a bed capacity of over 17,000, providing world class facilities for tourists whose annual arrival figure exceeds 600,000.

      • Vaginal Dose, Toxicity and Sexual Outcomes in Patients of Cervical Cancer Undergoing Image Based Brachytherapy

        Background: The aim of the study was to evaluate the vaginal dose and toxicity in patients of cervical cancer treated with image guided brachytherapy at our institute. Materials and Methods: Thirty-five patients treated with image based brachytherapy for cervical cancer were included. Vaginal contouring was done on MRI at brachytherapy and with CT scans of subsequent brachytherapy fractions. Dose volume parameters (DVH) were reported in accordance with the GEC-ESTRO guidelines. These were correlated with vaginal toxicity (assessed by CTCAE version 3) and quality of sexual life assessed at one year of completion of treatment. Results: Vaginal shortness was observed in 22 out of 30 (62.8%) patients, Nine (25.7%) had vaginal dryness and in 10 (28.5%) patients, there was contact bleeding. No association could be demonstrated between the dose volume parameters and vaginal toxicity in the present study. Conclusions: The lack of association between dose volume parameters of vagina with vaginal morbidity may be due to uncertainties involved in the delineation of vaginal wall and dosimetry. Future research is required to accurately define vaginal dose distribution to study its correlation with vaginal morbidity. Vaginal morbidity needs to be documented in order to improve the sexual outcome in these patients.

        Plasma cell leukemia in North India: retrospective analysis of a distinct clinicohematological entity from a tertiary care center and review of literature

        BackgroundPlasma cell leukemia (PCL) is a rare and aggressive plasma cell neoplasm. In PCL, clonal plasma cells comprise ≥20% of the peripheral blood (PB) leukocytes and/or the absolute clonal PB plasma cell count is ≥2×109/L. Primary PCL (PPCL) originates de novo, whereas, secondary PCL (SPCL) evolves from pre-existing multiple myeloma.MethodsClinicohematological features, immunophenotypic profile, and survival of PCL patients were analyzed retrospectively.ResultsBetween January 2007 and December 2014, ten PPCL and four SPCL patients were inves-tigated (8 PPCLs and 3 SPCLs had complete clinical data). All were North Indians, sharing common geography and ethnicity. Our cohort showed less frequent renal failure, more frequent hepatomegaly, and non-secretory type disease. In contrast to western literature, flow cytometric immunophenotyping of our cohort revealed altered expression of CD138 (67%), CD56 (33%), and CD20 (0%). With novel therapeutic agents, these PPCL patients had a median overall survival of 15 months.ConclusionWe highlight that our PPCL patients from North India had distinct clinicohematological and immunophenotypic profiles. The significance of our findings must be tested in a larger patient cohort and must be supported by molecular and cytogenetic investigations to un-mask possible significant effects on pathogenesis.
