        노인 환자에 있어서 치아 재식술 및 이식술의 임상적 고찰

        The purpose of this article is to discuss the effect of aging on the treatment of replantation and transplantation of teeth in old patients. 1 case of replantation and 2 cases of transplantations in old patients are reported in this article. Patients aged 65 and over are included. They had several problems such as periapical lesion, crack or root fractures. In those cases, the replantation and transplantation were treatment of choice. White ProRoot MTA was used in apical retrograde filling and perforation repair. After replantation and transplantation of teeth, follow-up visits showed signs of healing in 3 cases. Considerations and indications for replantation and transplantation are not much affected by age. Old patients are aware that replantation and transplantation can be performed comfortably and that age is not a factor in prediction success.

        Antimicrobial choice of severe endodontic infection

        Objectives : The purpose of our study was to evaluate penicillin as a still drug of choice for severe endodontic infection, by analyzing the antimicrobial susceptibilities from endodontic infections with swelling to figure out appropriate antibiotics as empirical treatment. Materials and methods : This study involved 18 patients who attended for emergency treatment because of facial or periapical swelling associated with root canal infections. Identification and antimicrobial susceptibility test of each pathogen were performed by Vitek2 Systems (bioMerieux, Marcy l’Etoile, France). Results : The most frequent bacteria was Streptococcus spp.(77%), and the resistance against penicillin was 35% in overall patients, followed by clindamycin and erythromycin (17%), which was much higher than previous studies. Conclusions : In our study, the higher resistance made penicillin alone not to be chosen as the first antibiotic drug for severe endodontic infections. Combinations with other drug, penicillin with wider spectrum of activity, or changing to other antibiotics was considered while remembering the increased risk of resistant microorganism.

        외상으로 파절된 치아의 치료에 대한 증례보고

        The traumatic accident could cause various damages to the teeth and surrounding tissues. It makes dentist embarrass because it not only requires the dentist to make quick decision, but also delayed decision affect the prognosis of the treatment. Depending on the location of the fracture, the fractured teeth are divided into crown fracture without pulpal involvement (Uncomplicated crown fracture), crown fracture with pulpal involvement (Complicated crown fracture), crown-root fracture, and root fracture. When the crown was fractured, treatment was performed with reattachment of the fragment or direct composite resin restoration depending on the presence or absence of the crown fragment. In the case of crown-root fracture, the direction of treatment can be set depending on whether the moisture control for final restoration is properly performed. In the case of root fracture, follow-up was observed after fixation using resin wire splint fixation, and if necessary, endodontic treatment of the crown fragment is recommended. The cases reported here have led to positive treatment outcome in fractured tooth for various traumatized situation.

        Glide path 형성용 니켈티타늄 회전 파일의 효율

        Objectives: This preliminary study compared the effects of glide path establishing instruments prior to substantial root canal preparation. Materials and Methods: Glide path was established by enlargement of the 2nd mesiobuccal root canal of Dentalike by using three kinds of glide path preparation nickel-titanium file; PathFile, One G and ProGlider. The pre- and post-instrumented Dentalikes were weighed in the resolution of 1/10㎎. In addition, after glide path preparation, torque generated during shaping using the WavoOne file was measured. The data were analyzed by one-way ANOVA and Tukey post-hoc test at a significance level of 95%. Results: The ProGlider had the significantly larger amount of reduced weight than other instrument groups (p<0.05). There was no significant difference between group of glide path preparation with ProGlider and without glide path preparation in maximum torque and total stress generation during the shaping with WaveOne. Conclusions: Glide path preparation instruments may have different efficiency according to their geometries. The Dentalike artificial teeth were revealed to have discrepancies in the size of root canals by microCT examination. It is impossible to make a meaningful judgment of the results due to the reliability or resolution problem of the root canal size of the artificial tooth selected as the standardized tooth.

        COMPARISON OF SCREW-IN EFFECT FORSEVERAL NICKEL-TITANIUM ROTARY INSTRUMENTS IN SIMULATED RESIN ROOT CANAL

        Screw-in effect is one of the unintended phenomena that occurs during the root canal preparation with nickel-titanium rotary files. The aim of this study was to compare the screw-in effect among various nickel-titanium rotary file systems. Six different nickel-titanium rotary instruments (ISO 20/.06 taper) were used: K3 (SybronEndo. Glendora. CA, USA). M_(two) (VDW GmbH. Mu''nchen. Germany). NRT with safe-tip and with active tip (Mani Inc.. Shioya-gun. Japan). ProFile (Dentsply-Mai1lefer. Ballaigues. Switzerland) and ProTaper (Dentsply-Mai1lefer. Ballaigues. Switzerland). For ProTaper, S2 was selected because it has size 20. Root canal instrumentations were done in sixty simulated single-curved resin root canals with a rotational speed of 300 rpm and single pecking motion. A special device was designed to measure the force of screw-in effect. A dynamometer of the device recorded the screw-in force during simulated canal preparation and the recorded data was stored in a computer with designed software (LCV-USE-VS. Lorenz Messtechnik GmbH. Alfdorf. Germany). The data were subjected to one-way ANOVA and Tukey' s multiple range test for post-hoc test. P value ofless than 0.05 was regarded significant. ProTaper produced significantly more screw-in effects than any other instruments in the study (p <0.00 1). K3 produced significantly more screw-in effects than M_(two). and ProFile (p <0.001). There was no significant difference among M_(two). NRT. and ProFi1e (p>0.05). and between NRT with active tip and NRT with safe one neither (p> 0.05). From the result of the present study. it was concluded. therefore. that there seems significant differences of screw-in effect among the tested nickel-titanium rotary instruments. The radiallands and rake angle of nickel-titanium rotary instrument might be the cause of the difference. Screw-in effect는 니켈-티타늄 전통 파일을 사용한 근관형성시 나타나는 현상으로 근관형성을 어렵게 한다. 이 연구의 목적은 다양한 니캘-티타늄 전동 파일들 사이의 screw-in effect를 비교하고자 하는 것이다. 본 연구에서는 여섯 가지의 다른 니켈 티타늄 전동 파일 기구들, 즉, K3 (SybronEndo, Glendora, CA USA), M_(two) (VDW GmbH, Mi.inchen, Germany), 삭제능이 있는 첨단을 가진 것과 삭제능이 없는 첨단을 가진 NRT (Mani Inc, Shioya-gun, Japan) , ProFile (Dentsply-Maillefer, Ballaigues, Switzerland), 그리고 ProTaper (Dentsply-Maillefer, Ballaigues, Switzerland) 가 사용되었다. 기구가 통일한 경사도와 크기를 가지게 하기 위해, 각 기구는 경사도 0.06, size 20을 선택하였으며, ProTaper의 경우에는 이와 유사한 S2를 사용하였다. 각 기구당 10개씩 총 60개의 투명레진 블록의 모조 단일 만곡근관 (REF A0177, Dentsply-Maillefer, Ballaiguez, Switzerland) 에서, 전동파일의 회전속도는 분당300회 전으로 하고, 단일 pecking 동작이 되게 하여 기구조작을 하였다. 장치를 고안하여 일정한 힘의 pecking 동작을 재현하고 screw-in effect의 힘을 측정하였다. 고안한 장치의 dynamometer 근관형성 과정동안 screw-in force를 측정하였고, 기록된 data는 고안된 소프트웨어를 이용하여 컴퓨터에 저장되었다. 데이터는 one-way ANOVA로 통계처리를 하였고, Tukey's multiple range test를 사용하여 95% 수준에서 유의성을 검정하였다. ProTaper가 가장 큰 screw-in effect를 나타내었다 ( p<0.001). K3는 M_(two)와 ProFile 보다 큰 screw-in effect를 나타내었다 (p <0.001). 그러나 Mtwo, NRT와 ProFile 사이에서는 유의한 차이가 나타나지 않았고 (p >0.05), 삭제능이 있는 첨단을 가진 NRT와 삭제능을 가지지 않는 첨단을 가진 NRT사이에도 유의한 차이를 나타내지 않았다. 이상의 연구결과를 통해 볼 때, 실험에 사용된 수종의 Ni-Ti 전동파일들 사이에서 screw-in effect의 차이가 나타날 것으로 보이며. 특히, Ni-Ti 전동파일의 radial lands와 rake angle이 screw-in effect의 차이를 나타낼 수 있을 것으로 생각된다.

        Influence of root canal curvature on the screw-in effect of nickel-titanium rotary files in simulated resin root canal

        Objectives: Nickel-titanium (Ni-Ti) rotary instruments have some unexpected disadvantages including the tendency to screw-in to the canal. The purpose of this study was to evaluate the influence of root canal curvatures on the screw-in effect of Ni-Ti rotary files. Materials and Methods: A total of 80 simulated root canals in clear resin blocks were used in the study. Canals with curvature of 0, 10, 20 and 30 degrees were instrumented with ProTaper instruments SX, S1, S2 and a ProFile of #25/0.06 to 1.0-2.0 mm beyond the initial point of root curvature. The screw-in force was measured with a specially designed device while canal was instrumented with a ProFile of #30/0.06 at a constant speed of 300 rpm. The data were subjected to one-way ANOVA and Scheffe multiple range test for post-hoc test. Results: Larger degree of canal curvature generated significantly lesser screw-in forces in all groups (p < 0.001). Conclusions: More attention needs to be paid when using rotary instruments in canals with less curvature than canals with more curvatures to prevent or reduce any accidental overinstrumentation. 연구목적: 근관의 만곡도가 니켈-티타늄 전동파일의 screw-in effect에 미치는 영향을 평가하고자 하였다. 연구 재료 및 방법: 0, 10, 20, 그리고 30도의 만곡을 가지는 80개의 레진모형근관 (Nissin Dental Prod. Inc.)에서 0.06 경사도 30번 전동화일 (ProTaper, Dentsply-Maillefer)을 분당 300회전, 단일 pecking 동작으로 기구조작하였으며 dynamometer를 이용하여 발생한 screw-in force를 측정하였다. 결과치는 one-way ANOVA로 통계처리 하였고, Scheffe multiple range test를 사용하여 95% 수준에서 유의성을 검정하였다. 결과: 근관의 만곡도가 작을수록 screw-in force가 높게 나타났으며 (p < 0.001), 모든 그룹 간에 유의성 있는 차이를 보였다. 결론: 이 결과로 미루어 볼 때, 근관의 만곡도가 작을수록 screw-in force가 많이 발생하는 것으로 보여지며, 따라서 기구조작 동안의 screw-in force를 방지 또는 억제하기 위해서는 만곡도가 작은 근관을 성형할 때 더 많은 주의가 요구될 것으로 생각된다.

        Influence of taper on the screw-in effect of nickel-titanium rotary files in simulated resin root canal

        Objectives: The introduction of nickel-titanium alloy endodontic instruments has greatly simplified shaping the root canal systems. However, these new instruments have several unexpected disadvantages. One of these is tendency to screw into the canal. In this study, the influence of taper on the screw-in effect of the Ni-Ti rotary instrument were evaluated. Materials and Methods: A total of 20 simulated root canals with an S-shaped curvature in clear resin blocks were divided into two groups. ProFile .02, .04, .06 (Dentsply-Maillefer) and GT rotary files .08, .10, .12 (Dentsply) were used in Profile group, and K3 .04, .06, .08, .10, and .12 (SybronEndo, Glendora) were used in K3 group. Files were used with a single pecking motion at a constant speed of 300 rpm. A special device was made to measure the force of screw-in effect. A dynamometer of the device recorded the screwin force during simulated canal preparation and the recorded data was stored in computer with designed software. The data were subjected to one-way ANOVA and Tukey's multiple range test for post-hoc test. p value of less than 0.05 was regarded significant. Results: The more tapered instruments generated more screw-in forces in Profile group (p < 0.05). In K3 group, 0.08, 0.10. and 0.12 tapered instruments showed more screw-in force than 0.04 tapered one, and 0.08 and 0.12 tapered instruments showed more screw-in force than 0.06 tapered one (p < 0.05). Conclusions: The more tapered instruments seems to produce more screw-in force. To avoid this screw-in force during instrumentation, more attention may be needed when using more tapered instruments. 연구목적: 니켈-티타늄 금속을 근관치료기구에 도입함으로써 근관형성과정이 매우 단순화되었다. 그러나 이러한 새로운 기구는 단점도 함께 가지고 있는데 그 중 하나가 근관내로 기구가 빨려 들어가는 경향이다. 본 연구에서는 니켈-티타늄 전동파일의 경사도가 screw-in effect에 미치는 영향을 평가하고자 하였다. 연구 재료 및 방법: 총 20개의 S-자형 투명레진블록 (Dentsply-Maillefer) 근관에서 두 군으로 나누어 실험하였는데 Profile군에서는 0.02, 0.04 그리고 0.06 경사도의 Profile (Dentsply-Maillefer)과 0.08, 0.10 그리고 0.12의 경사도의 GT Rotary file (Dentsply-Maillefer)을 사용하였고 K3군에서는 같은 경사도의 K3 file SybronEndo)을 사용하였으며, 분당 300회전의 일정한 속도에서 단일 pecking 동작으로 기구조작을 하였다. 특수한 장치를 고안하였으며 장치내 dynamometer를 이용하여 screw-in effect를 측정하여 one-way ANOVA로 통계처리하고 Tuckey's multiple range test로 사후검정하였다. 결과: Profile 군에서는 큰 경사도의 기구가 더 큰 screw-in force를 나타내었고 (p < 0.05), K3군에서는 0.08, 0.10. 및 0.12 경사도의 기구가 0.04 경사도의 기구에 비해, 0.08 및 0.12 경사도의 기구가 0.06 경사도의 기구에 비해 더 큰 screw-in force를 나타내었다 (p < 0.05). 결론: 기구의 경사도가 클수록 screw-in force가 많이 발생하는 것으로 생각되며, 경사도가 큰 전동화일을 사용시에는 screw-in force를 억제하기 위해 더 많은 주의가 요구될 것으로 생각된다.
