Neuralgic amyotrophy is a clinical syndrome of unknown etiology characterized by the acute onset of severe shoulder girdle pain followed by motor weakness or paralysis, and variable sensory loss. Up to 90 percent of patients eventually recover within the third years and the treatment is symptomatic. Neurologic findings and electrodiagnostic studies are only useful diagnostic methods of neuralgic amyotrophy and other laboratory testing are frequently normal or nonspecific. We report two cases of neuralgic amyotrophy with paraspinal muscles involvement with review of literature.

Three sheet steels were uniaxially prestrained and tension tested in 15˚intervals to the prestrain axis In the subsequent tests after prestrains higher than about 7%, the residual uniform strains were very small in orientations beyond 30˚ to the prestrain axis. Total strains to failure also decreased in these noncoaxial orientations but the tendency was not as marked as in the uniform strain. The subsequent flow curves in the noncoaxial directions skewed no sharp yield points. The work-hardening rate was initially very high but decreased rapidly, which resulted in a large decrease in uniform strain.

Several new test procedures were tried to measure flow behavior of sheet metals under combined stress stales utilizing a normalized aluminum-killed steel which was nearly isotropic. Equibiaxial stress condition was obtained From a through-thickness compression test on a disk-slacked circular cylinder. Plane strain and pure shear conditions were obtained by plane strain compression tests, in which one of in-plane strains or the thickness strain is zero. Experimental Flow stresses of the steel were slightly higher in the equibiaxial loading path, and slightly lower in the plane-strain path than the expected values from the Mises theory. Experimental flow stresses in the pure shear path fitted well with the theoretical values.

Physical effects resulting from strain-path changes from cold rolling to uniaxial tension were studied on two sheet steels; a aluminum-killed steel and a normalized aluminum-killed steel. Cold rolling caused a unusually large decrease in uniform strain and in total strain to failure in all orientations in subsequent tension. The prestrained sheets showed three stages of deformation; initial rapid work hardening, work softening and low-work hardening stage.
저산소환경이 일차 배양된 대뇌겉질 신경세포의 ubiquitin-proteasome system에 미치는 영향

저산소환경에 만성으로 노출된 경우 신경세포는 적응하기 위하여 세포 내의 다양한 신호 경로의 활성화를 통해 반응한다. 그 중 ubiquitin-proteasome system은 세포의 생존을 위한 핵심적인 기전으로 proteasome이 억제될 때 비정상적인 단백질이 세포 내에 축적되어 세포자멸사와 신경세포의 퇴행이 일어난다. 이번 실험에서 저자들은 ubiquitin-proteasome system을 억제하는 proteasome 억제제와 저산소환경이 일차 배양한 대뇌 겉질 세포에 미치는 영향을 관찰하였으며, 도파민생성 세포에서의 결과와 비교하였다. 저산소환경이나 합성된 억제제인 PSI (Z-lle-Glu(OtBu)-Ala-Leu-al)를 투여한 신경세포의 자멸사는 증가하였으며, PARP와 caspase 3의 활성도 증가하였다. 특히 NF-κB의 발현이 PSI를 처리한 경우와 저산소환경에서 증가하였다. 저산소환경에서 배양한 신경세포에 PSI를 처리한 경우에는 이들 단백질의 발현이 현저하게 증가하였다. α-synuclein의 발현 역시 증가하였는데 특히 저산소환경에서 현저하게 증가하였다. 이 실험을 통해 저산소환경과 proteasome 억제제를 처리했을 경우 신경세포의 자멸사가 증가하였을 뿐 아니라, α-synuclein의 생성 및 NF-κB의 발현이 증가하였음을 확인하였다. 이를 통해 저산소환경이 ubiquitin-proteasome system을 교란할 수 있음을 제시하였다. 이러한 결과는 저산소환경이 신경퇴행성질환의 원인이 될 수 있음을 제시하는 것이다. The ubiquitin-proteasome system is crucial in maintaining cellular growth and metabolism. Dysfunction of this system may contribute to neurodegenerative diseases, such as Parkinson’s disease. But its effects on primary neurons are largely unknown. In the present study, we investigated the effects of proteasome inhibitor and hypoxia on primary neuronal cultures to determine whether proteasomal malfunction induces neuronal death. Neuronal apoptosis increased in primary cultured cortical neurons with treatment of proteasomal inhibitor in normoxic condition and in the presence or absence of proteasomal inhibitor in hypoxic condition. Also expression of PARP and activated caspase 3 increased. NF-κB, a key transcription factor in this system expression increased in hypoxic condition and proteasomal inhibition. Interestingly, hypoxic condition induced an expression and accumulation of α-synuclein in neuron, one of components of Lewy body in Parkinson’s disease. Our findings determine that hypoxic condition may affect the ubiquitin-proteasome system. Furthermore, it suggests that hypoxic condition and proteasomal inhibitors are involved, at least in parts, in neurodegeneration of mouse model for Parkinson’s disease.
정중 감각신경 전도검사에 의한 당뇨병성 신경병증의 조기진단

Background To evaluate the diagnostic value of median sensory nerve conduction study in early phase of diabetic neuropathy. Methods The onset latency, duration, peak amplitude, and area of sensory nerve action potential (SNAP) of median nerve stimulated at wrist and elbow, respectively on 20 normal adults and 20 diabetic patients who revealed normal findings in conventional electrodiagnostic procedure were checked and compared. Results 1) The onset latency, peak amplitude, and area of SNAP stimulated at wrist of diabetic patients were significantly different than those of control group (p<0.05). 2) The conduction velocity, peak amplitude, and area of SNAP stimulated at elbow of diabetic patients were significantly different than those of control group (p<0.05). 3) The amplitude of SNAP stimulated at elbow on diabetic patient was most accurate parameter and next, the amplitude of median sensory nerve action potential stimulated at wrist. Conclusion The amplitude of median sensory nerve action potential stimulated at elbow are good quantitative indices of diabetic neuropathy who revealed normal findings in conventional electrodiagnostic procedure with positive neurologic symptom and physical examination.

Objective To investigate the clinical factors associated with successful gastrostomy tube weaning in patients with prolonged dysphagia after stroke. Methods This study involved a retrospective medical chart review of patients diagnosed with prolonged dysphagia after stroke who underwent gastrostomy tube insertion between May 2013 and January 2020. Forty-seven patients were enrolled and consequently divided into gastrostomy tube sustaining and weaning groups. The numbers of patients in the sustaining and weaning groups were 31 and 16, respectively. The patients’ demographic data, Korean version of Mini-Mental State Examination (K-MMSE) score, Korean version of the Modified Barthel Index (K-MBI), Functional Dysphagia Scale (FDS) score, and Penetration-Aspiration Scale (PAS) score were compared between the two groups. A videofluoroscopic swallowing study was performed before making the decision of gastrostomy tube weaning. The clinical factors associated with gastrostomy tube weaning were then investigated. Results There were significant differences in age; history of aspiration pneumonia; K-MMSE, FDS, and PAS scores; and K-MBI between the groups. In the multiple logistic regression analysis, the FDS (odds ratio [OR]=0.791; 95% confidence interval [CI], 0.634–0.987) and PAS scores (OR=0.205; 95% CI, 0.059–0.718) were associated with successful gastrostomy tube weaning. In the receiver operating characteristic curve analysis, the FDS and PAS were useful screening tools for successful weaning, with areas under the curve of 0.911 and 0.918, respectively. Conclusion In patients with prolonged dysphagia, the FDS and PAS scores are the only factors associated with successful gastrostomy tube weaning. An evaluation of the swallowing function is necessary before deciding to initiate gastrostomy tube weaning.
