      • Clinician Survey on Speech Pathology Services for People with Aphasia in Hong Kong

        This study aims to investigate aphasia practices by speech-language pathologists working with clients suffering from acquired neurogenic communication disorders through an online survey. Questions pertaining to content and intensity of aphasia management, service adequacy, and barriers to effective service provision to persons with aphasia were asked to understand the current service-provision situation in Hong Kong. A web-based questionnaire containing 38 questions was electronically distributed to speech-language pathologists in Hong Kong. A total of 41 respondents (equivalent to about 5% of the body of clinicians) completed and returned the survey. Compared to recommendations in existing best practice guidelines of aphasia from western countries, the results indicated that intensity and frequency of current aphasia services in Hong Kong fell short of expectations. Major challenges perceived by speech-language pathologists that contributed to dissatisfaction of aphasia service included limited manpower, resources, funding, and accessibility. It was also found that barriers to effective services could be related to clinicians’ restricted adoption of intervention approaches, lack of active involvement of related healthcare professionals, and insufficient effort of referring persons with aphasia to support or self-help groups. Finally, factors to consider in order to improve existing aphasia management were discussed.

      • Proposing a quick, objective, comprehensive analysis of impaired spoken discourse in post-stroke aphasia

        Purpose: Aphasia is an acquired language disorder, most commonly caused by a stroke, that adversely affects one’s ability to understand, speak, read, and write. Diagnosis of aphasia is typically done through administration of standardized aphasia batteries, many of which lack a detailed and adequate evaluation of spoken discourse. The aim of this paper was to propose a quick, objective, and comprehensive analytic system that addressed the micro- and macro-linguistic aspects of oral narratives in persons with aphasia (PWA). Methods: Using a subgroup of unimpaired native speakers of English from the public corpus of AphasiaBank, task-specific normative data of three narrative tasks were constructed. Scoring criteria on the production of events, use of corresponding lexical items in these events, and the order of sequence of presenting the events were also developed. Twelve PWA were recruited and their spoken discourse was quantified using this newly proposed analytic system. Results: Significant correlations were found between PWA’s performance on formal aphasia batteries and selected indices on the number of events produced as well as informative words used. Fluent and non-fluent PWA performed significantly differently in terms of their use of informative words and event sequence. The analytic system also demonstrated excellent intra- and inter-rater reliabilities. Conclusions: The clinical feasibility and diagnostic values of this proposed approach to quantify PWA’s spoken output are confirmed. The present investigation also offered more evidence in supporting the needed psychometric properties of spoken discourse analysis that can supplement traditional formal aphasia batteries.

      • Main Concept Analysis for Acquired Deficits of Spoken Narratives: Preliminary Data on Inter-rater Agreement and Potential Application to the Korean-Speaking Population

        This study aims to investigate the inter-rater agreement of the Main Concept Analysis (MCA), a proposition-based system for analyzing the presence, accuracy, completeness, and efficiency of content in spoken narratives of speakers with aphasia. Twenty-one MCA assessments were administered to thirteen participants recruited from an intensive aphasia treatment program. Six MCA indices were applied to the language samples, which were cross-checked to determine discrepancies of results across raters. The present results were consistent with similar studies in the literature, thus indicating that the Main Concept Analysis is a reliable assessment battery. Given the simple, quick, but objective procedures for language quantification, it is argued that the Main Concept Analysis can easily be adopted to the Korean-speaking population for clinical analysis of discourse.

      • Impact of COVID-19 on college seniors’ learning and performance in communication sciences and disorders: Students’ self-reflections

        Purpose: A recent summary of ASHA surveys (fielded in March and April 2020) has delineated the pandemic-related challenges in education of communication sciences and disorders (CSD). This paper aimed to estimate how COVID-19 affected college seniors’ learning and course performance, and discuss how a threat is posed to workforce entering. Methods: Ninety-eight written reflections were analyzed using a descriptive analysis to determine how sudden termination of face-to-face instruction affected learning, and subsequently impacted seniors’ completion of course assignments and test performance. Results: It was found that students’ loss of motivation, concentration, and/or self-discipline when adjusting and adapting to the pandemic posed the greatest challenge to knowledge acquisition and studying. Losing class engagement and face-to-face clinical exposure greatly hindered skill application; both elements lowered preparedness and satisfaction of college experience, and might reduce seniors’ competitiveness and readiness to launch a new job. Conclusions: As the coronavirus era is evolving, student comments and reflections become particularly valuable for gaining insights into improving online teaching and learning for future career.

      • Development and preliminary psychometric analysis of a quick screening form for referrals of individuals with neurogenic communication disorders to speechlanguage pathologists by laypersons

        Purpose: The study aimed at developing and determining the preliminary reliability and validity of a new screening form for use by laypersons to refer individuals with neurogenic communication disorders to speech-language pathologists. Methods: Thirty-two native Cantonese speakers with neurogenic communication disorders were tested with the proposed screening form by fourteen volunteer raters and well-established speech and language tests for aphasia, dysarthria and apraxia of speech (AOS) by ten speech-language pathology students. Results: Statistical analyses showed that the internal consistency and inter-rater reliability were high in all the tasks for aphasia and most of the tasks for dysarthria and AOS of the proposed screening form. It was further identified that some of the tasks for aphasia, dysarthria, and AOS would be valid for laypersons to identify individuals with communications disorders. Conclusions: The results indicated that the tasks for screening aphasia, dysarthria, and AOS should be carefully selected so that untrained laypersons would be able to make correct judgements. Further investigations involving a larger sample size are needed to determine areas to enhance the sensitivity and discriminating power of the current screener to be used by laypersons without a clinical background.
