본원에서 1975년 9월 1일부터 1978년 8월 31일까지 수술한 자궁외임신 228례중 확진된 난소임신은 3례로서 자궁외임신의 1.31%였다. 3례 모두 2~5회의 인공유산이나 자연유산의 과거력이 있었고 2례는 미경산, 1례는 경산으로 자궁내 피임장치 중에 발생한 것으로 사료되며, 2례가 우측 1례가 좌측난소임신이였으며, 3례는 모두 육안적, 현미경적 소견으로 Spiegelberg의 진단기준을 충족하였기에 문헌적 고찰과 함께 보고하는 바이다. The primary ovarian pregnancy is an extremely rare form of ectopic gestation and is fullfilled Spiegelbergs criteria for the diagnosis. Authors report 3 cases of primary ovarian pregnancy among 228 cases of ectopic pregnancy which were operated upon from Sept. 1 1975 to Aug. 31 1978 at Eul-Ji General Hospital. One of them was suspected primary ovarian pregnancy during operation. The other two were suspected bleeding corpus luteum and ruptured follicular cyst. All of them were confirmed primary ovarian pregnancy by postoperative pathology. We present these 3 cases of primary ovarian pregnancy with brief review of literatures.

본 증례는 Spiegelberg씨 조건에 일치한 난소임신으로서 그 치험 1예를 보고하는 바이다. A case of ovarian pregnancy which fulfilled Spiegelberg’s criteria for the diagnosis of ovarian. Pregnancy in a 31 years old woman is presented.

복강경 불임시술후 발생한 자궁외 임신에 관한 임상통계적 고찰

저자들은 1984년 1월1일부터 1987년 12월31일까지 청주 서울병원에서 기왕 복강경 난관불임시술력을 가진 자궁외임신 43예를 분석, 고찰하여 다음과 같은 결론은 얻었다. 1. 복강경 불임시술 기왕력을 가진 자궁외임신 환자는 총 자궁외임신 환자의 13.9%였다 2. 복강경 불임시술후 자궁외 임신의 연령분포는 27세부터 42세까지였으며 호발연령군은30-34세였다. 3. 최다 임신횟수는 8회, 최소임신횟수는 2회로서 평균 임신회수는 4.5회였다. 4. 최대 생존아수는 5명이었고, 최소 생존아수는 1명이었으며 평균 생존아수는 2.7명이었다. 5. 대부분의 경우 하복부 동통, 이상질출혈, 무월경, 오심 및 구토, 현기증 등의 증상이 복합적으로 나타났으며, 심한 경우는 쇼크상태를 보이는 경우도 있었다. 6. 임신반응검사의 양성율은 57.6%이었으며, 더글라스 와 천자소견에서는 95.0%의 양성율을 보였다. 7. 최종 월경 제 1일부터 증상 발생시까지의 기간은 최저 3주에서 최고 13주가지의 분포를 보였으며 평균 임신기간은 6.2주이었다. 8. 복강경 불임수술후 자궁외 임신이 발생되기까지의 기간은 최소 3개월에서 최고 10년까지분포되었으며 평균 3.9년이었다. 9. 복강경 불임시술방법에는 난관소작법이 86.0%이었고, falope ring 시술법이 5예, 원인대 소작이 1예 있었다. 10. 자궁외임신 발생부위에서는 원위부가 79.1%이었고, 근위부가 20.9%이었으며 최다 호발부위는 팽대부로서 69.8%의 빈도를 나타냈다. 11. 수헐하지 않은 경우도 7예가 있었으나 평균 3.1pint의 수혈을 한 것으로 나타났다. 1. The incindence of ectopic pregnancy with laparoscopic tubal sterilization history was 13.9% 2. The highest incindence occured in 30-34 year group . 3. Majourity of patients (79.8%) have two to three children and average child number was 2.7. 4. The main symptoms were lower abdominal pain (93.0%) vaginal spotting (77.0%) and amenorrhea (55.8%) 5. The positive rate of urinary HCG test was 57.6% (19cases ) and positive rate of culdocentesis was 95.0% (40cases) 6. The mean gestational period was about 6.2weeks. 7. Ectopic pregnancy after laparoscopic tubal sterilization occured in 55.9% within 4 years, with the shortest interval being 3 months and the longest 10 years. 8. The sterilization method was bilateral tubal fulguration in 37 cases and Yoon`s falope ring application in 5 cases. 9. The location of ectopic pregnancy were in order of pregnancy 30 patients in the ampullar 5 patients in the isthmic 4 patients in the interstitial and 4 patients in the fimbrial portion. 10. The intraperitoneal blood loss was 500-1500cc in 27 patients and mean blood tranfusion volume was 3.1 pints. 11. The operation method were bilateral salpingectomry in 14 patients and unilateral salpingectomy with contralateral tubal ligation in 11 patients
