Fracture of Hamate Hook의 유용한 방사선 단순촬영법에 대한 고찰

Purpose : As racket sports have been recently popular, hamate hook fracture has frequently occurred. As the case stands this study is to ascertain which simple radiography is effective for hamate hook fracture. Materials and Method: On patients with carpal injuries who were admitted to the emergency room, simple radiography was performed from the anteroposterior view, the lateral view, the both oblique view, the carpal tunnel view and the cup view, respectively. The cup view indicates what the patient cups the hand while undergoing radiography, and in this case the CR angle is applied to cephalad side. On this wise, patients with hamate hook fracture were analytically compared with the normal in radiological depiction in order to simple radiography effective for hamate hook fracture. Results : It was ascertained which simple radiography is most effective to depict the hamate bone, as above. From the anteroposterior view, the lateral view and the both oblique view, the hamate bone and the carpal bone seemed to be observed. But from the carpal tunnel view and the cup view, the hamate bone was well depicted. In the case of patients with harnate hook fracture, the cup view was more effective than the carpal tunnel view because it was difficult for them to take the posture. Conclusion : In this study it was found that hamate hook fracture was depicted best in cases where simple radiography was performed from the cup view. In addition, the simple posture was effective to mitigate patients’ pains, to prevent them from moving while undergoing radiography, and to enhance the quality of radiographic images. Purpose : An increment in waiting time is one of the factors making patients inconvenient because of the overall improvement of service of medical institutions and the standard of public service. This is a critical factor in assessment by institution and customer on the service of the hospital and has been direct influence on the customer satisfaction survey. The purpose of this study is to increase the level of service satisfaction by minimize the inconvenient of patients through the improvement of process for the reduction of waiting time in general projection. Materials and Methods : The subjects were the visiting patients of this hospital. The waiting time before and after the process improvement of each projection room were measured and compared The way of improvement was determined by considering the staffs and equipments using the order of process improvement possibilities. The practice was carried out on the work and equipment separately. As a result, the data compiled on the general projection completion the number of cases before and after the process improvement of each projection room and compared and analyzed using the tool contained in 6-Sigma. Results : The improvements of waiting time derived from the improvement of both work and equipment were observed. The improvement of work efficiency was achieved by solving the problem of work overload imbalance through the improvement the effective assignment of projection room and staffs and continuous work training. The bottle neck phenomenon was released by the reduction in projection time due to operation of projection room only for whole spine or long bone, the improvement of productivity due to replacement of DR Type equipment, and the change of projection room only for IVP into general projection room The reduction of waiting time during peak time was reduced effectively. The change of waiting time were from 12min. to 11min. and from 23min. to 11min both for the chest projection and general projection, respectively. Conclusion : The most of general projection department of diagnostic radiology inspection are performed by the needs of medical treatment, therefore it is impossible to predict the amounts of work It was common, accordingly, for the patients of peal< time to wait long time due to the concentration of patients. This has negative effect on the level of customer satisfaction, therefore the reduction of waiting time is important in improving the level. It was shown that the improvement of process was most effective. It is concluded that the improvement of process leads to the productivity increment and effective management on staffs, ultimately, to the improvement of customer satisfaction.
