Kenberg는 ‘전이 초점 심리 치료(Transference Focused pysychotherapy, TFP)’를 개발하였다. 경계성 심리의 장애가 되는 심리구조의 취약성을 변화시키고 통합하는 것이 전이초점심리치료이다. 본 연구는 경계성 성격장애 사례를 Kenberg의 전이초점치료 전략에 따라 분석하였다. 전이초점 심리치료의 관점으로 경계성 성격장애를 가진 내담자의 사례를 제시하였다. 그리고 이 사례를 Kernberg의 전이해석을 위한 3단계 전략의 구조에 맞추어 분석을 시도하였다. 여기에서 사례분석의 1단계는 만성적인 공허감, 혼란스러운 사고와 감정표현, 왜곡된 정서가 활성화되는 원시적 부분적 대상관계의 성격을 재구성하도록 하는 것과 가장 지배적인 정서를 파악하고 평가한 회기까지를 범주에 넣었다. 사례분석의 2단계는 자기 이미지와 대상 이미지의 측면에서 전이 안에서 형성되는 지배적인 대상관계를 상담자가 평가한 부분이다. 대상관계와 자기와의 상호작용을 명료화 한 회기까지를 2단계의 범주로 분석하였다. 사례분석 3단계에서는 경계성 성격장애를 갖는 내담자가 진정한 자기와 그의 내적 대상에 대한 개념이 통합되어가는 부분이다. 방어적으로 해리된 대상과의 통합을 돕고, 내재화된 대상관계, 자기, 그리고 대상들이 전체 세계를 통합하도록 개입한 범위까지를 3단계의 범위로 넣었다(컨버그, 2003). 전체적으로 경계성 성격장애와 전이치료에 대한 이론적 고찰, 내담자의 호소문제, 초기경험, 임상평가와 진단, 그리고 상담 진행 과정과 전이초점치료의 한계점까지 알아보았다. 이 사례는 기본적으로는 전이초점 심리치료로 진행되었으나 다른 기법들이 통합적으로 사용되었다. 이 연구를 통해 Kernberg의 전이초점 치료와 임상현장의 치료적 역동성을 함께 살필 기회가 될 것이다. 또한, Kernberg의 전이해석을 위한 3단계 전략의 구조에 맞추어서 사례를 분석한 것을 보고 실질적으로 임상 현장에서 적용해 볼 수 있을 것이다. Kernberg developed ‘Transference Focused Psychotherapy (TFP)’. TFP is aimed at changing and integrating the vulnerability of the psychological structure which causes borderline personality disorder. This study offers an analysis of a client with borderline personality disorder according to Kernberg’s schema of TFP: Three-stage strategy for transference analysis. The sessions of the first stage are directed to transform the primitive partial object relations of the client in which chronic emptiness, confused thoughts, and distorted emotions are activated, and grasp and evaluate the most dominant emotions. The sessions of the second stage are aimed at evaluating dominant object relations formed in transference in terms of self-image and object image and clarifying the interaction between the object and the self. The sessions of the third stage of analysis are intended for the client to integrate the concept of not only her true self but also her inner object. I aided her to integrate defensively dissociated objects, internalized object relations, and her self in her whole inner world (Kerberg, 2003). Overall, I in this study explain the theoretical review of borderline personality disorder and TFP, the client’s complaints and initial experiences, clinical evaluation and diagnosis of her, the course of analysis, and the limits of TFP. The analysis basically proceeded based on TFP, but other techniques were used in combination. This study will provide an opportunity to examine Kernberg’s TFP and the therapeutic dynamics that occur in the clinical field. I believe that the case analysis according to Kernberg’s three-step strategy should be a help for other psychoanalysts in the clinical field.
