        Pulsatile Extracorporeal Membrane Oxygenation(ECMO) can mitigate the heart load and raise the patient's blood perfusion. But If the ECMO pulsate the blood flow during the systolic period, It can burden to the patient's heart. To avoid the heart injury, we have to consider the relation between output of ECMO, hemodynamic states and heart movement. To raise the efficacy of the pulsatile ECMO, we investigated the coronary perfusion, cardiac muscle tension and hemodynamic states during the ECMO perfusion by using the mathematical model of human blood circulatory system and ECMO. The outflow data of the pulsatile ECMO(T-PLS, Bioheartkorea, Korea) was obtained in vitro experiments. According to the phase and pumping rate of the ECMO, the heart's load and coronary perfusion could be adjusted to the proper levels. The results of the human- ECMO lumped parameter model showed that the synchronizing operation of the pulsatile ECLS can be helpful at stabilizing the patient's hemodynamic states.

      • 경동맥 자극을 통한 혈압조절 기전의 분석

        A recent research shows that blood pressure tends to decrease temporarily: when artificial electronic simulation is generated over baroreceptors within carotids. This proves that the baroreflex can be controlled by using external stimulation. The objective of this paper is to build a new type of baroreflex control system that contributes to reduce the set point of an autonomic nervous system in patients with high blood pressure. A hemodynamic model was designed to control blood pressures in both the central nerve system and the peripheral nerve system, as well as a stimulating system in carotids consisting of sensor, stimulator and control system. It was produced not only to control the intensity and the frequency of a stimulator implanted under the skin by using Zigbee, but also to enable the stimulator to run for a long period of time. Preliminary results indicate that approximately 30 ~ 50 ㎜Hg decrease in blood pressure is observed through electronic simulation in carotids.

      • 박동형 생명구조장치의 동기구동 방법에 대한 연구

        Pulsatile ECLS (Extracorporeal Life Support System) maintains the blood pressure of patients and replace oxygen and carbon dioxides by supplying pulsatile blood flow to the body of a patient in emergency such as acute heart failure or cardiac arrest. In this system, the pulsation and stroke volume of supplying blood flow are the essential performance factors that must be considered to reduce the load of heart and to supply sufficient blood flow to organs. Based on the previous studies, this study developed the new control algorithm of the Pulsatile ECLS that the pulse wave of the pulsatile ECLS delivered to the patients' heart when the aortic valve is closed. To measure the pulsation of the heart, the Pulsatile ECLS used ECG (Electrocardiogram) and PPG(Photoplethysmograph). And, the synchronous operations and performances of the Pulsatile ECLS obtained from these two signals were compared through an extracorporeal experiment.

        In pulsatile artificial heart, the abrupt change in blood pressure and flow cause the blood cell damage. So, to mitigate the abrupt blood pressure and flow change, it is vary important that the actuator should be controlled to reduce blood flow output change as well as the design of an actuator. We controlled the moving position of the actuator and it is enable to have various locus and different from the previous moving velocity control scheme. In in-vitro experiments, we compared the effect on the blood pressure and flow according to each position locus.
