        Patient reported outcomes in elective laparoscopic cholecystectomy

        Backgrounds/Aims: Traditional outcome measures (e.g., length of hospital stay, morbidity, and mortality) are used to determine the quality of care, but these may not be most important to patients. It is unclear which outcomes matter to patients undergoing elective laparoscopic cholecystectomy (ELC). We aim to identify patient-reported outcome measures (PROM) which patients undergoing ELC valued most. Methods: A 45-item questionnaire with Four-point Likert-type questions developed from prior literature review, prospectively administered to patients treated with ELC at a tertiary institution in Singapore. Results: Seventy-five patients participated. Most essential factors were technical skill and experience level of a surgeon, long-term quality of life (QoL), patient involvement in decision-making, communication skill of a surgeon, cleanliness of the ward environment, and standards of nursing care. Least important factors were hospitalization leave duration, length of hospital stay, a family’s opinion of the hospital, and scar cosmesis. Employed patients were more likely to find hospitalization leave duration (p<0.001) and procedure duration (p=0.042) important. Younger patients (p=0.048) and female gender (p=0.003) were more likely to perceive scar cosmesis as important. Conclusions: Patients undergoing ELC value long-term QoL, surgeon technical skill and experience level, patient involvement in decision-making, surgeon communication skill, cleanliness of the ward environment, and nursing care standards. Day-case surgery, medical leave, family opinion of hospital, and scar cosmesis were least important. Understanding what patients value will help guide patient-centric healthcare delivery.

      • Geotextile Bags and Tubes for Beach and Riverbank Restoration

        There is an increasing popularity on the use of geotextile tubes and bags for different sizes and shapes for coastal, beach and river bank application in this region over the last 10 years. These geotextile bags, tubes, and containers are basically functioned as either a gravity mass as part of containment bunds; or massive surface protection for beach stabilization and restoration, or vegetated erosion control slope protection for river bank and riverfront; or a mean of dewatering of soft sediments contained inside. This paper summarizes the two recent projects in Singapore and Malaysia: (a) Beach restoration using specially designed geotextiles bags, and (b) Riverfront restoration using combination of geotextiles tubes and bags. It highlights the innovative and cost effective solution in these applications. For the beach restoration project in Singapore, stacked geotextile bags have been used to reconstruct the eroded shoreline due to the wave action on the beaches along the east coast of Singapore. For the riverfront project that involved reclamation and slope protection in Malaysia, geotextile tubes and bags are used as an alternative design to the conventional armor rock system and anchored bulkhead.

        유사 공진형 물체에 대한 TM 전자파의 산란계산을 위한 MLFMA방법

        The method of moments has been widely used in the analysis of TM scattering problems. Recently, significant advances in the development of fast and efficient techniques for solving large problems have been reported. In such methods, iterative matrix solvers are preferred by virtue of their speed and low memory requirements. But for near resonant and strong multiple scattering problems, e.g., involving an aircraft engine inlet, a large number of iterations is required for convergence. In this paper, an efficient approximate inverse based preconditioner is used to reduce this number of iterations. By using the matrix partitioning method, the computational is used to reduce this number of iterations. By using the matrix partitioning method, the computational cost for obtaining the approximate inverse is reduced to O(N). We apply this preconditioner to an O(NlogN) algorithm, the multilevel fast multipole algorithm, for the aircraft engine inlet problem. The numerical results show the efficiency of this preconditioner. 모멘트법은 전자파산란문제에 널리 사용되고 있는데, 최근에 대용량의 문제를 빠르고 효율적으로 풀 수 있는 기법들에 대한 연구가 많이 진행되고 있다. 대부분의 이런 기법에는 계산속도나 기억용량을 효율적으로 이용할 수 있는 반복법을 사용해서 행렬방정식을 풀게 되는데, 유사공진특성을 갖는 물체에 대한 산란은 물체 내부에서 전자파가 공진하는 특성을 가지므로 반복해법올 이용하여 적분방정식을 풀 경우 수렴이 잘 되지 않거나, 수렴되기까지 많은 반복회수를 필요로 한다. 본 논문에서 사용된 MLFMA(Muli-level Fast Multipole Algorithm)는 FMM(Fast Multipole Method)을 다층으로 확장한 알고리듬으로 반복회수당 계산시간을 O(NlogN)으로 줄일 수 있다. 이 MLFMA를 유사공진형구조에 적용하고, 또한 행렬식을 블록밴드행렬 전처리를 하여 반복회수를 감소시켰다. 여기서 사용된 전처리행렬은 행렬분할법을 이용하여 O(N)의 계산시간으로 구할 수 있으므로, 미지수가 많을 때는 전처리행렬을 구하는데 드는 추가계산시간을 무시할 수 있다. 여기서 제안된 방법을 비행기의 공기유입구에 대한 TM전자파산란 계산에 적용하여 효율성을 보였다

        Effect of sintering holding time on the properties and low-temperature degradation behaviour of manganese oxide-doped Y-TZP ceramic

        The sintering of yttria-tetragonal zirconia polycrystals (Y-TZP) is commonly carried at a predefined temperature using a long holding time of 2 hours. This often has resulted in grain coarsening which affects the mechanical properties of the sintered body and low temperature degradation (LTD) behaviour of the ceramic when exposed to moist environment. In this research, the effect of using a short holding time of 1 min. during sintering coupled with small amounts (0.5 and 1 wt%) of manganese oxide, MnO2 as dopant on the mechanical properties and LTD behaviour of 3 mol% Y-TZP were investigated. The results showed that the mechanical properties improved significantly with the addition of 0.5 wt% MnO2 for the 1-min. and 12-min. holding time samples when compared to 2 hours holding time sintered at the same temperature of 1350 ℃. Nevertheless, regardless of the holding time employed, the 1 wt% MnO2 addition was not effective in improving the mechanical properties and ageing resistance of Y-TZP ceramic.
