        Analysis of the Activities of the Website “Question and Answer about Radiation in Daily Life” after the Accident at the Fukushima Daiichi Nuclear Power Plant and Some Lessons Learned from It: To Pass on This Experience to the Future

        After the accident at the Fukushima Daiichi Nuclear Power Plant, artificial radionuclides such as radioactive cesium and iodine were released into the environment. It caused great anxiety not only in the vicinity of the Fukushima Daiichi Nuclear Power Plant but also in other regions of the world. Some members of the Japan Health Physics Society (JHPS), a leading academic soci- ety in Japan in the field of radiation protection, volunteered to establish a website called “Ques- tion and Answer (Q&A) about radiation in daily life” shortly after the accident to help reduce the residents’ anxiety about the health effects of radiation. In August 2011, “Committee for Q&A about radiation in daily life” was established in JHPS, making the website-related activities a responsibility of JHPS. The Q&A website continued to respond to the questions from the gen- eral public with expertise and sincerity until February 2013 when the Committee members de- cided to end the activities because the number of questions received had gradually decreased with the passage of time. This paper aims to introduce the following: the activities of the Q&A website during the two years (2011–2013), the stance chosen for the activities, the information related to the website activities and the analysis of Twitter data. Building on the experience and the knowledge obtained from the activities, it also discusses issues and experiences that can be utilized in the initial response to emergencies for radiation protection experts as well as those in other fields.

        Optimal First-Line Antibiotic Treatment for Pediatric Complicated Appendicitis Based on Peritoneal Fluid Culture

        Purpose: Consensus is lacking regarding the optimal antibiotic treatment for pediatric complicated appendicitis. This study determined the optimal first-line antibiotic treatment for pediatric patients with complicated appendicitis based on peritoneal fluid cultures. Methods: This retrospective study examined the cases of pediatric patients who underwent appendectomy for complicated appendicitis at our institution between 2013 and 2019. Peritoneal fluid specimens obtained during appendectomy were cultured for the presence of bacteria. Results: Eighty-six pediatric patients were diagnosed with complicated appendicitis. Of them, bacteria were identified in 54 peritoneal fluid samples. The major identified bacteria were Escherichia coli (n=36 [66.7%]), Bacteroides fragilis (n=28 [51.9%]), α-Streptococcus (n=25 [46.3%]), Pseudomonas aeruginosa (n=10 [18.5%]), Enterococcus avium (n=9 [16.7%]), γ-Streptococcus (n=9 [16.7%]), and Klebsiella oxytoca (n=6 [11.1%]). An antibiotic susceptibility analysis showed E. coli was inhibited by sulbactam/ampicillin in 43.8% of cases versus cefmetazole in 100% of cases. Tazobactam/piperacillin and meropenem inhibited the growth of 96.9-100% of the major identified bacteria. E. coli (100% vs. 84.6%) and P. aeruginosa (100% vs. 80.0%) were more susceptible to amikacin than gentamicin. Conclusion: Tazobactam/piperacillin or meropenem is a reasonable first-line antibiotic treatment for pediatric complicated appendicitis. In the case of aminoglycoside use, amikacin is recommended.

        Associations between serum 25-hydroxyvitamin D3 level and skeletal muscle mass and lower limb muscle strength in Japanese middle-aged subjects

        Objectives: One of the important risk factors of falling is decreased muscle mass and muscle strength. Recently, there has been an increasing concern on the role of vitamin D in muscle strength and physical activity. Aim of our study is to examine the relationships between vitamin D status and muscle mass and muscle strength in middle-aged healthy adults. Methods: Subjects were 40 healthy volunteers aged 42.0 ± 10.6 years old. Evaluation was made for serum vitamin D3 metabolites including 25- hydroxyvitamin D3 [25(OH)D3] and 24,25-dihydroxyvitamin D3 [24,25(OH)2D3] concentrations, lower limb muscle strength, and dietary intake by food frequency questionnaire. Body composition was measured by dual-energy X-ray absorptiometry (DXA), and appendicular skeletal mass index (ASMI) was calculated as skeletal muscle mass/squared height. Results: 70% of the subjects had vitamin D insufficiency/deficiency (serum total 25(OH)D < 20 ng/mL), and female subjects had significantly lower serum total 25(OH)D level compared with males. Vitamin D insufficiency/deficiency group had significantly higher body fat, lower SMI and muscle strength, probably reflecting higher percentage of female subjects. Serum vitamin D3 metabolites levels were significantly correlated with whole and site-specific ASMI, and lower limb muscle strength, except for the correlation between serum 24,25(OH)2D3 concentration and lower limb muscle strength. In addition, serum 25(OH)D3 level was a positive significant predictor for both ASMI and lower limb muscle strength, while serum 24,25(OH)2D3 level was not their significant predictor. Conclusions: Serum 25(OH)D3 level was significantly correlated with both skeletal muscle mass and lower limb muscle strength.
