제2경추 반측 후궁 일괄 절제술 및 후궁 나사를 이용한 재부착술

제2경추 부위의 경막내 종양을 제거하기 위한 고식적인 후방 접근법은 제2경추 극돌기에 부착되는 신전근들을 분리(detach)하고 후궁 절제술을 시행하는 것이다. 제2경추의 극돌기에 부착된 신전근은 경추에서 가장 중요한 신전근이기 때문에, 수술중에 이 신전근을 분리하게 되면 술 후 경부 통증, 운동 제한, 후만 변형 및 불안정증 등의 후유증이 발생할 수 있다. 저자들은 제2경추 신전근을 보존하면서 제2경추 부위에 발생한 경막내 종양을 제거하기 위한 새로운 접근 및 재건 술기를 보고하는 바이다. 제2경추 신전근을 세심하게 보존하면서 반측 후궁 일괄 절제술을 시행하여 제2경추부의 경막을 적절히 노출시켜서 종양을 제거하고, 2개의 제2경추 후궁 나사를 이용하여 후궁을 재부착하였다. Conventional posterior approach for resection of an intradural tumor at C2 involves C2 lamincectomy and detachment of extensor muscles from C2 spinous process. Being major extensors of the cervical spine, the destruction of C2-attached muscles may lead to detrimental consequences, namely, neck pain, limited neck motion, progressive kyphosis, and instability. We report a new technique on exposing and reconstructing the C2 level while preserving C2 extensors for resection of an intradural tumor at C2 level. We performed an en bloc hemilaminectomy of C2 while carefully preserving C2 extensor muscles to adequately expose the dura mater at the C2 level. After removal of the tumor, we proceeded with the reconstruction by recapping the C2 hemilamina using 2 laminar screws.
