        Predictive Value of Molecular Subtyping for Locoregional Recurrence in Early-Stage Breast Cancer with N1 without Postmastectomy Radiotherapy

        Purpose: This study was designed to investigate the relationship between molecular subtype and locoregional recurrence (LRR) in patients with early-stage breast cancer with 1–3 positive axillary lymph nodes (ALNs) and improve the individualized indications for postmastectomy radiotherapy (PMRT). Methods: The records of 701 patients with pT1-2N1M0 breast cancer who did not undergo PMRT were retrospectively analyzed. Tumors were subclassified as follows: luminal A, luminal B, human epidermal growth factor receptor 2 (HER2)-enriched, and basal-like subtypes. Multivariate Cox analysis was used to determine the risk of LRR associated with the different subtypes and to adjust for clinicopathologic factors. Results: Luminal A, luminal B, HER2- enriched, and basal-like subtypes accounted for 51.2%, 28.0%, 8.1%, and 12.7% of cases, respectively. The median follow-up duration was 67 months (range, 9–156 months). Univariate analysis revealed that, compared with the luminal A subtype, the HER2-enriched and basal-like subtypes were associated with significantly higher 5-year LRR rates (5.6% vs. 21.6% and vs.15.7% respectively; p=0.002 each), lower 5-year LRR-free survival (LRFS) rates (90.6% vs. 73.8% and 78.5%, respectively; p=0.001 each), and poorer 5-year breast cancer-specific survival (BCSS) rates (93.7% vs. 82.2% [p=0.002] and 84.9% [p=0.001], respectively). Multivariate analysis revealed that the HER2-enriched and basal-like subtypes, age ≤35 years, a medial tumor, and pT2 stage were poor prognostic factors for LRR and LRFS; furthermore, 2 to 3 positive ALNs represented an independent prognostic factor affecting LRR. The 10-year LRR rates of patients with 0, 1, 2, 3, and 4 risk factors were 1.0%, 6.9%, 14.3%, 30.4%, and 54.3%, respectively (p<0.001); the 10-year BCSS rates were 86.6%, 88.5%, 84.4%, 79.7%, and 38.8%, respectively (p<0.001). Conclusion: Molecular subtyping allows for individualized evaluation of LRR risk in patients with pT1-2N1M0 breast cancer. PMRT should be recommended for patients with ≥3 LRR risk factors.

      • Building up the Church in a Socialist Country : An Examination of Bishop K. H. Ting’s Chinese Social Theology

        Bishop K. H. Ting lives in a difficult transitional period to handle the relationship between Christianity and socialism in China. Ting has made great achievements throughout his life in terms of education, church leadership, scholarship, and ecumenism. He has led the Chinese churches through the most difficult periods especially when the ultra-leftist political power dominated, which tried to eradicate all religions in China. Out of his own life experience, Ting develops his socio-political consciousness in his theological reflections. He remains calm, objective and controlled to analyze the civil aspect of Christianity in the socialist country, and tries his best to sinolize and socialize Christianity in the Chinese context through his social theology which covers his theology of society, his theology for society, and his theology of the church in society. His notion of the all-embracing cosmic Christ becomes his theological foundation that enables him, with a realistic attitude, to enter into dialogue with different Christian groups and non-Christians, not the least humanitarian atheists. From the contemporary perspective, however, Ting’s social theology is inevitably challenged by globalization and a Chinese civil society in the making.

      • Clinical Application of the Adenosine Triphosphate-based Response Assay in Intravesical Chemotherapy for Superficial Bladder Cancer

        Objective: To investigate correlations between adenosine triphosphate chemotherapy response assay (ATP-CRA) and clinical outcomes after ATP-CRA-based chemotherapy for drug selection in patients receiving intravesical chemotherapy to prevent recurrence of superficial bladder cancer after surgery. Methods: The chemosensitivities of 12 anticancer drugs were evaluated, including 5-Fu ADM, and EPI, using ATP-CRA and primary tumor cell culture in 54 patients. In addition, a further 58 patients were treated according to clinical experience. Differences in post-chemotherapeutical effects between drug sensitivity assay and experience groups were compared. Results: The evaluable rate of the test was 96.3%, the clinical effective rate was 80.8%, the sensitivity rate was 97.6% (41/42), the specificity was 20%, the total predicting accuracy was 74.3%, the positive predictive value was 83.7% (41/49), the negative predictive value was 66.7% (2/3); in the drug sensitivity test group, the clinical effective rate was 80.8%, the experience group response rate was 63.8%, with a significant difference in clinical effects between the ATP-based sensitivity and experience groups (${\chi}^2$=7.0153, P<0.01). Conclusion: ATP-CRA is a stable, accurate and potentially practical chemosensitivity test providing a predictor of chemotherapeutic response in patients with superficial bladder cancer.

      • BRCA1 Gene Exon 11 Mutations in Uighur and Han Women with Early-onset Sporadic Breast Cancer in the Northwest Region of China

        The prevalence of BRCA1 gene mutations in breast cancer differs between diverse ethnic groups. Relatively little information is known about patterns of BRCA1 mutations in early-onset breast cancer in women of Uighur or Han descent, the major ethnic populations of the Xinjiang region in China. The aim of this study was to identify BRCA1 mutations in Uighur and Han patients with early-onset (age <35 years), and sporadic breast cancer for genetic predisposition to breast cancer. For detection of BRCA1 mutations, we used a polymerase chain reaction single-stranded conformation polymorphism approach, followed by direct DNA sequencing in 22 Uighur and 13 Han women with early-onset sporadic breast cancer, and 32 women with benign breast diseases. The prevalence of BRCA1 mutations in this population was 22.9% (8/35) among early-onset sporadic breast cancer cases. Of these, 31.8% (7/22) of Uighur patients and 7.69% (1/13) of Han patients were found to have BRCA1 mutations. In 7 Uighur patients with BRCA1 mutations, there were 11 unique sequence alterations in the BRCA1 gene, including 4 clearly disease-associated mutations on exon 11 and 3 variants of uncertain clinical significance on exon 11, meanwhile 4 neutral variants on intron 20 or 2. None of the 11 BRCA1 mutations identified have been previously reported in the Breast Cancer Information Core database. These findings reflect the prevalence of BRCA1 mutations in Uighur women with early-onset and sporadic breast cancer, which will allow for provision of appropriate genetic counseling and treatment for Uighur patients in the Xinjiang region.

      • Radiotherapy Alone is Associated with Improved Outcomes Over Surgery in the Management of Solitary Plasmacytoma

        Background: A moderate dose of radiation is the recommended treatment for solitary plasmacytoma (SP), but there is controversy over the role of surgery. Our study aimed at comparing different treatment modalities in the management of SP. Materials and Methods: Data from 38 consecutive patients with solitary plasmacytoma, including 16 with bone plasmacytoma and 22 with extramedullary plasmacytoma, were retrospectively reviewed. 15 patients received radiotherapy alone; 11 received surgery alone, and 12 received both. The median radiation dose was 50Gy. All operations were performed as radical resections. Local progression-free survival (LPFS), multiple myeloma-free survival (MMFS), progression-free survival (PFS) and overall survival (OS) were calculated and outcomes of different therapies were compared. Results: The median follow-up time was 55 months. 5-year LPFS, MMFS, PFS and OS were 87.0%, 80.9%, 69.8% and 87.4%, respectively. Univariate analysis revealed, compared with surgery alone, radiotherapy alone was associated with significantly higher 5-year LPFS (100% vs 69.3%, p=0.016), MMFS (100% vs 51.4%, p=0.006), PFS (100% vs 33.7%, p=0.0004) and OS (100% vs 70%, p=0.041). Conclusions: Radiotherapy alone can be considered as a more effective treatment for SP over surgery. Whether a combination of radiotherapy and surgery improves outcomes requires further study.

        A method for nonlinear aerostatic stability analysis of long-span suspension bridges under yaw wind

        By using the nonlinear aerostatic stability theory together with the method of mean wind decomposition, a method for nonlinear aerostatic stability analysis is proposed for long-span suspension bridges under yaw wind. A corresponding program is developed considering static wind load nonlinearity and structural nonlinearity. Taking a suspension bridge with three towers and double main spans as an example, the full range aerostatic instability is analyzed under wind at different attack angles and yaw angles. The results indicate that the lowest critical wind speed of aerostatic instability is gained when the initial yaw angle is greater than 0°, which suggests that perhaps yaw wind poses a disadvantage to the aerostatic stability of a long span suspension bridge. The results also show that the main span in upstream goes into instability first, and the reason for this phenomenon is discussed.

        Wind tunnel investigation on flutter and buffeting of a three-tower suspension bridge

        The Maanshan Bridge over Yangtze River in China is a new long-span suspension bridge with double main spans of 2×1080 m and a closed streamline cross-section of single box deck. The flutter and buffeting performances were investigated via wind tunnel tests of a full bridge aeroelastic model at a geometric scale of 1:211. The tests were conducted in both smooth wind and simulated boundary layer wind fields. Emphasis is placed on studying the interference effect of adjacent span via installing a wind deflector and a wind separating board to shelter one span of the bridge model from incoming flow. Issues related to effects of mid-tower stiffness and deck supporting conditions are also discussed. The testing results show that flutter critical wind velocities in smooth flow, with a wind deflector, are remarkably lower than those without. In turbulent wind, torsional and vertical standard deviations for the deck responses at midspan in testing cases without wind deflector are generally less than those at the midspan exposed to wind in testing cases with wind deflector, respectively. When double main spans are exposed to turbulent wind, the existence of either span is a mass damper to the other. Furthermore, both effects of mid-tower stiffness and deck supporting conditions at the middle tower on the flutter and buffeting performances of the Maanshan Bridge are unremarkable.

        An iterative approach for time-domain flutter analysis of bridges based on restart technique

        This paper presents a restart iterative approach for time-domain flutter analysis of long-span bridges using the commercial FE package ANSYS. This approach utilizes the recursive formats of impulse-response-function expressions for bridge's aeroelastic forces. Nonlinear dynamic equilibrium equations are iteratively solved by using the restart technique in ANSYS, which enable the equilibrium state of system to get back to last moment absolutely during iterations. The condition for the onset of flutter instability becomes that, at a certain wind velocity, the amplitude of vibration is invariant with time. A long-span suspension bridge was taken as a numerical example to verify the applicability and accuracy of the proposed method by comparing calculated results with wind tunnel tests. The proposed method enables the bridge designers and engineering practitioners to carry out time-domain flutter analysis of bridges in commercial FE package ANSYS.

        Aerostatic instability mode analysis of three-tower suspension bridges via strain energy and dynamic characteristics

        Multispan suspension bridges make a good alternative to single-span ones if the crossed strait or river width exceeds 2-3 km. However, multispan three-tower suspension bridges are found to be very sensitive to the wind load due to the lack of effective longitudinal constraint at their central tower. Moreover, at certain critical wind speed values, the aerostatic instability with sharply deteriorating dynamic characteristics may occur with catastrophic consequences. An attempt of an in-depth study on the aerostatic stability mode and damage mechanism of three-tower suspension bridges is made in this paper based on the assessment of strain energy and dynamic characteristics of three particular three-tower suspension bridges in China under different wind speeds and their further integration into the aerostatic stability analysis. The results obtained on the three bridges under study strongly suggest that their aerostatic instability mode is controlled by the coupled action of the anti-symmetric torsion and vertical bending of the two main-spans’ deck, together with the longitudinal bending of the towers, which can be regarded as the first-order torsion vibration mode coupled with the first-order vertical bending vibration mode. The growth rates of the torsional and vertical bending strain energy of the deck after the aerostatic instability are higher than those of the lateral bending. The bending and torsion frequencies decrease rapidly when the wind speed approaches the critical value, while the frequencies of the anti-symmetric vibration modes drop more sharply than those of the symmetric ones. The obtained dependences between the critical wind speed, strain energy, and dynamic characteristics of the bridge components under the aerostatic instability modes are considered instrumental in strength and integrity calculation of three-tower suspension bridges.

      • Prediction of Power Generation in China Using Process Neural Network

        The power generation prediction problem can be seen as a time series prediction problem in nature. The traditional time series prediction methods based on regression analysis do not take account into the time accumulation effect existed in the time series due to their discrete input values. This limitation causes the low prediction accuracy of the time series prediction methods based on regression analysis. To solve this problem, a power generation time series prediction model based on the process neural network is proposed. The inputs of the proposed prediction model can be continuous time-varying functions. The time accumulation effect existed in the power generation time series can be expressed and computed by the integration operator of the process neural network. The proposed prediction model is trained and the efficiency of the proposed prediction model is tested by the month power generation data form January 2001 to April 2012, and the comparison experiment results indicate that the process neural network performs better than the auto regression analysis.
