Background: Due to the risk of viral transmission during in-person training, a shift towardonline platforms is imperative in the current pandemic. Therefore, we compared the effectivenessof an in-person interactive course with a structurally similar online course designedto improve cognitive skills among clinical health professionals in arterial blood gas analysis,management of electrolyte imbalances, and approaches to mechanical ventilation in criticallyill patients. Methods: In an observational, outcome assessor-blinded, cohort trial, group A included participantsenrolled prospectively in an online course, while group B included those who tookpart in an in-person course (retrospective arm). The primary objective was comparison ofcognitive skills through a pre and post-test questionnaire. Statistical analysis was performedusing Student t-test. Results: In total, 435 participants were analyzed in group A, while 99 participants were evaluatedin group B. The mean pre-test score was 9.48±2.75 and 10.76±2.42, while the meanpost-test score was 11.94±1.90 (passing rate, 64.6%) and 12.53±1.63 (passing rate, 73.3%)in groups A and B, respectively. Group B scored significantly higher in both pre-test (P=0.001)and post-test evaluations (P=0.004). The improvement in post-test score was significantlygreater (P=0.001) in group A (2.46±2.22) compared to group B (1.77±1.76). The medicalspecialties fared better in group B, while surgical specialties scored higher in group A. Thepre-test vs. post-test scores exhibited a moderate correlation in both groups (P<0.001). Thefeedback survey showed a Likert score >3.5 for most points in both groups. Conclusions: The online teaching module exhibited a significant benefit in terms of participantsensitization and knowledge sharing.

Purpose: Uncertainty exists about whether early laparoscopic cholecystectomy (LC) is an appropriate surgical treatment for acute calculous cholecystitis. This study aimed to compare early vs. late LC for acute calculous cholecystitis regarding intraoperative difficulty and postoperative outcomes. Methods: This was a prospective randomized study carried out between December 2015 and June 2017; 60 patients with acute calculous cholecystitis were divided into two groups (early and delayed groups), each comprising 30 patients. Thirty patients treated with LC within 3 to 5 days of arrival at the hospital were assigned to the early group. The other 30 patients were placed in the delayed group, first treated conservatively, and followed by LC 3 to 6 weeks later. Results: The conversion rates in both groups were 6.7% and 0%, respectively (p = 0.143). The operating time was 56.67 ± 11.70 minutes in the early group and 75.67 ± 20.52 minutes in the delayed group (p = 0.001), and both groups observed equal levels of postoperative complications. Early LC patients, on the other hand, required much fewer postoperative hospital stay (3.40 ± 1.99 vs. 6.27 ± 2.90 days, p = 0.006). Conclusion: Considering shorter operative time and hospital stay without significant increase of open conversion rates, early LC might have benefits over late LC.

Background: Data on the prevalence of programmed death-ligand 1 (PD-L1) expression and tumor-infiltrating lymphocytes (TILs) in non–small cell lung cancer (NSCLC) and their clinical significance in Indian patients are limited. Methods: Newly diagnosed NSCLC cases (adenocarcinoma or squamous cell carcinoma [SqCC] histology) were included in the present study. The TILs were evaluated based on morphology on hematoxylin and eosin–stained slides. PD-L1 expression in tumors was assessed using immunohistochemistry with rabbit monoclonal antibody (SP263) on the Ventana automated immunostainer. Tumors with PD-L1 expression > 50% on tumor cells were considered PD-L1–positive. Tumors in which TILs occupy > 25% of stroma were considered to have high TILs. The association of PD-L1 expression and TILs with various clinical parameters including overall survival (OS) was investigated. Results: The present study included 128 cases of NSCLC (67 adenocarcinoma, 61 SqCC). PD-L1 positivity was observed in 17.2% of the patients with NSCLC. Baseline characteristics of PD-L1–positive subjects were similar to PD-L1–negative subjects except for a higher prevalence of liver metastasis (18.2% vs. 2.8%; p = .018) and a higher probability of diagnosis from extrapulmonary biopsies. High TILs were observed in 26.6% of the subjects. However, PD-L1 expression and high TIL did not affect OS. Conclusions: PD-L1 positivity and high TILs were observed in 20% and 25% of the patients with NSCLC, respectively, however, neither were predictors of survival in SqCC.
Design and Implementation of Router for NOC on FPGA

In today’s technological era, SOC has undergone rapid evolution and is still processing at a swift pace. But due to this explosive evolution of semiconductor industry, the devices are scaling down at a rapid rate and hence, SOC today have become communication-centric. However, the existing bus architectures comprising of wires for global interconnection in SOC design are undergoing design crises as they are not able to keep up with the rate of scaling down of devices. To overcome bottleneck of communication system, NOC is an upcoming archetype. In on-chip network, router is considered as an important component. This paper proposes router, its components and parameters which affects the entire design. Thus, to validate the functioning of NOC on hardware, router has been designed in VHDL and simulated in Xilinx ISE 14.1 targeting Xilinx XC5VLX30-3 FPGA.

A layer of compressible, rotating, elastica-viscous fluid heated & soluted from below is considered in the presence of vertical magnetic field to include the effect of Hall currents. Dispersion relation governing the effect of viscoelasticity, salinity gradient, rotation, magnetic field and Hall currents is derived. For the case of stationary convection, the Rivlin-Erickson fluid behaves like an ordinary Newtonian fluid. The compressibility, stable solute gradient, rotation and magnetic field postpone the onset of thermosolutal instability whereas Hall currents are found to hasten the onset of thermosolutal instability in the absence of rotation. In the presence of rotation, Hall currents postpone/hasten the onset of instability depending upon the value of wavenumbers. Again, the dispersion relation is analyzed numerically & the results depicted graphically. The stable solute gradient and magnetic field (and corresponding Hall currents) introduce oscillatory modes in the system which were non-existent in their absence. The case of overstability is discussed & sufficient conditions for non-existence of overstability are derived.

Aluminium metal matrix composites (AMMCs) are the fastest developing materials for structural applications due to their high specific weight, modulus, resistance to corrosion and wear, and high temperature strength. Carbon nanotubes (CNTs) is known as the material of the twenty-first century for its various applications in structural components for their high specific strength as well as functional materials for their exciting thermal and electrical characteristics. The present study comprise a systematic literature review of Al/CNT nanocomposites fabricated through a solid state friction stir processing. The present review is primarily focussed on the dispersion and survivability of CNTs in the Al matrix because these are the key factors in deciding the mechanical properties of the fabricated composite. Additionally, the formability, weldability and machinability of the FSPed fabricated composites reinforced with CNTs are also summarised here. Based on the detailed literature review, following research gaps are identified which require a critical and more focussed attention of the scientific community working in this research area: (i) the presence of agglomeration or clustering of CNTs in the composite, (ii) survivability and shortening of CNTs during FSP, (iii) interfacial reactions or the formation of reaction products (such as Al4C3) between Al matrix and CNTs, and (iv) the unidirectional alignment of CNTs in the fabricated composite. Important suggestions for further research in effective dispersion of CNTs with its preserved structure by FSP are also provided.

A pectin zirconium (IV) selenotungstophosphate (Pc/ZSWP) nanocomposite was prepared via sol–gelmethod. The material was characterized using different techniques such as FTIR, SEM and TEM. Theadsorptional/photocatalytic activity of Pc/ZSWP was investigated for the removal of methylene blue andmalachite green dyes in two different conditions. It was revealed that the synergistic adsorptional/photocatalytic degradation of dyes was more efficient than mere adsorption in dark followed byphotocatalysis. The photocatalytic degradation percentage of MB and MG dyes onto Pc/ZSWPnanocomposite was found out to be 89.21 and 79.27% in 3 h of photo irradiation which is higherthan for adsorption in dark followed by photocatalysis. The antimicrobial activity of Pc/ZSWP was alsoascertained against Escherichia coli.
