고관절 및 습관절 수술전후의 폐색전증 발생빈도에 관한 연구

Background: Pulmonary embolism is common complication following orthopedic joint replacement procedures. The incidence of pulmonary embolism (PE) after elective hip or knee replacement has not been reported in Korean population. Methods: To find out the incidence of PE related to elective hip or knee replacement, we performed perfusion (QS) and ventilation scintigraphy (VS) preoperatively and postoperatively in 157 patients who underwent hip or knee replacement procedures, QS was performed preoperatively, and then again 7 days after surgery, VS was obtained within a day in patients with significant perfusion defects on QS using 99mTc-diethylenetriamine-pentacetic acid (DTPA) aerosol. PE was diagnosed either by pulmonary angiography, or by changing persusion pattern on sequential QS in case of high-probability scan. QS and VS were read according to PIOPED (Prospective Investigation of Pulmonary Embolism Diagnosis) criteria. Results: Among 157 preoperative studies, 5 high-and 6 intermediate-probablity scans were observed. Five patients with intermediate-probablity scan underwent pulmonary angiogrphy, and PE were confirmed in 3 patients. All 8 patients with PE (5 patients with high- probability scan and 3 confirmed patients by pulmonary angiography) were asymptomatic and showed improvement on QS after 7 days of heparin therapy. Of 146 patients, 12 patients (8.2%) showed significant changes from low-probability or normal scan to 5 high-and 7 intermediate-probability scans postoperatively, compared to preoperative QS. Among 7 patients with intermediate-probability, PE were confirmed in 4 patients by pulmonary angiography. Among 9 patients with PE, 7 patients showed improvement on QS after heparin therpy, and 1 patient did not undergo follow-up QS, and 1 patient died of hypoxic respiratory failure. In 9 patients with PE, 8 patients were asymptomatic. Conclusion: The incidence of PE following total hip or knee replacement was 6.1%(9/146). Preoperative prevalence of PE was 5.1%(8/157). Combination of preand past-operative scintigraphic imaging is a useful diagnostic modality for screening PE, because most patients are not clinically suspected.

Primary extramedullary plasmacytomas (E.M.P) are uncommon plasma cell neoplasms that generally occur in the submucosal tissue of the upper airway. The following illustrates a case study of a 73 year-old man with peritoneal E.M.P and subsequent ascites is described, The finding of many plasma cells in ascites and electrophoresis and immunoelectrophoresis of as- cites led to diagnosis. Similar histologic findings such as multiple myeloma and immunoblastic lymphoma were ruled out by clinical study and immunochemical studies. This malignant ascites represents the first case described caused by E.M.P.

We developed and tested the high pressure hydrogen gas cylinder(type4) for fuel cell vehicle. The working pressure is 350bar. We conducted material tests, production tests and design qualification tests on the developed cylinders according to modified NGV2-2000(hydrogen). The high pressure hydrogen gas cylinder met all the design qualification requirements of ANSI/CSA NGV2-2000 and acquired NGV2 certification from independent inspection agency.

Objetive: The measurement of cardiac output by CO2 rebreathing method is noninvasive procedure using indirect Fick equation. In order to compare the result of cardiac output measured by CO2 rebreathing method with that by thermodilution technique, this study was performed. Methods: Simultaneous measurement of cardiac output by CO2 rebreathing method and thermodilution technique was performed in 13 mitral stenosis patients. The subjects were 4 men and 9 women, with mean age of 41.15±11.01 year. The cardiac output (CO) can be calculated from indirect Fick equation using the CO2 rebreathing method, CO=CO2 production/CvCO2-CaCO2. The CO2 production was obtained by collecting expired gas and multiply its volume by CO2 concentration and the arterial PCO2 was estimated from the end tidal PCO2. The mixed venous PCO2 was obtained from rebreathing plateau during O2 and mixture gas breathing through rebreathing bag. Results: 1) The average cardiac output was 3.41±0.45(L/min) by CO2 rebreathing method and 3.45±0.37(L/min) by thermodilution technique. 2) The result of cardiac output measured by CO2 rebreathing method was highly correlated to that by thermodilution technique (r=0.82). 3) The equation relating two measurements was Y =-0.01±0.99X. (Y: CO2 rebreathing method, X: thermodilution technique) Conclusion: This study showed that the result of cardiac output by CO2 rebreathing method was correlated well with that by standard thermodilution technique, so it is thought that CO2 rebreathing method could provide valid estimate of cardiac output for evaluation of cardiac function. It is simple to perform, easily repeatable and essentially risk free. Therefore this method could be very useful for clinical use.
STELLA-2 소듐 시험 시설 고온 배관 계통의 설계 및 건전성 평가

본 연구에서는 한국원자력연구원이 개발 중인 소듐 열유동 종합효과 시험장치(STELLA-2)의 주요 배관 계통을 대상으로 고온 설계를 수행하고, 두 가지 설계기술기준에 따라 배관의 건전성 평가를 수행하였다. 배관 설계기술기준으로는 일반 압력배관에 관한 ASME B31.1과 프랑스의 원자력등급 배관 설계기술기준인 RCC-MRx RD-3600을 적용하였으며, 이들 기술기준의 보수성을 정량적으로 비교 및 분석하였다. STELLA-2 소듐시설에서는 모의 잔열제거계통(Model DHRS), 모의 중간열전달계통(Model IHTS) 및 펌프 모의계통(PSLS)에 배관이 설치되는데, 두 설계기술기준을 따라 이들 배관 계통에 대해 건전성 평가를 수행한 결과 설계 건전성이 확인되었으며, 설계 기술기준 간 비교분석 결과 유지하중에 대해서는 ASME B31.1이, 열하중에 대해서는 RCC-MRx RD-3600이 더 보수적인 것으로 평가되었다. In this study, elevated temperature design and integrity evaluation have been conducted using two different piping design codes for the high-temperature piping systems of sodium integral effect test loop for safety simulation and assessment(STELLA-2) being developed by KAERI(Korea Atomic Energy Research Institute). The design code of ASME B31.1 for power piping and French nuclear grade piping design guideline, RCC-MRx RD-3600 were applied, and conservatism of those codes was quantified based on the piping integrity evaluation results. The piping system of Model DHRS, Model IHTS and PSLS are to be installed in STELLA-2. The integrity evaluation results for the three piping systems according to the two design codes showed that integrity of the piping system was confirmed. As a code comparison result, ASME B31.1 was shown to be more conservative for sustained loads while RD-3600 was more conservative for thermal loads compared to B31.1.
Design and analysis of high-temperature piping system in the STELLA-2 sodium test facility

Elevated temperature design and integrity evaluation have been conducted for the high-temperature piping systems in the STELLA(Sodium integral effect test loop for safety simulation and assessment)-2 sodium test facility which is under development at KAERI(Korea Atomic Energy Research Institute). There are several piping systems in the STELLA-2 sodium test loop. Piping systems were designed and design evaluation were conducted for the piping systems in IHTS(Intermediate Heat Transport System) and PSLS(Pump Simulation Loop System). Piping analyses for these sodium test facility have been performed according to the piping design codes of ASME B31.1 and integrity of the piping systems were confirmed.
