Rowland Universal Dementia Assessment Scale (RUDAS)의 국내 적용을 위한 번역 연구

목적: 고령화로 인해 노인인구가 증가하고 치매환자의 수도 증가하고 있기 때문에 치매환자 관리에 관심이 많아지고 있다. 조기발견은 임상적 결과와 관련성이 있어 치매선별도구의 중요성이 더 높아질 것이다. 본 연구에서는 교육수준, 언어, 문화에 영향을 받지 않을 뿐만 아니라 전두엽기능 및 시공간구성능력도 평가할 수 있는 한국어판 롤랜드 유니버셜 치매평가척도(RUDAS-K)를 개발하고자 번역하였다. 연구방법: 본 번역 과정은 총 9개의 단계로 진행되었다. 1단계에서 작업치료사 3명이 원 도구를 한국어로 각자 번역하였고, 2단계로 작업치료사 1명이 3개의 번역본을 취합하였다. 3단계는 재활전문가 1인이 원문과 번역 사이의 불일치를 검사하였다. 4단계에서 검증위원회를 열어 한국어 번역본이 원문과 얼마나 흡사한지 설문하였다. 5단계에서는 검증위원회의 설문 결과를 기반으로 번역본을 수정한 후, 6단계로 번역본의 이해도에 대한 설문 조사를 시행하였다. 7단계에서는 3명으로 구성된 번역 검증위원회에서 한글번역의 적합성을 검증한 후, 8단계로 번역전문가를 통해 역번역을 하였다. 마지막으로 영어를 모국어로 하는 외국인 1명과 작업치료사 1명이 원문과 역번역본 사이의 의미전달의 유사성을 검증하였다. 결과: 1차 및 2차 번역 후 1차 합의과정을 거친 RUDAS-K는 적합성 설문지 조사결과 직역한 문장을 간단하게 수정하였고 의역하며 누락된 내용을 추가하여 수정하였다. 이해도 설문조사에서 낮은 점수를 받은 항목들과 적합성 설문 시행 이후 수정된 부분을 표시하여 검증위원회와 2차 합의과정을 거쳤다. 그 결과 영어교육과 교수와 국어국문학과 교수는 한글표기법 의견을 제시하였고 작업치료학과 교수는 평가도구의 특성을 고려한 단어선택 의견을 제시하였는데 이러한 의견들을 중점으로 수정하였다. 역번역 및 원본 비교 시 “tea”를 한국어에서 이중적인 뜻을 갖은 “차”가 아닌 “녹차”로 번역한 부분 외에 별다른 의미의 차이는 없어 RUDAS-K에 추가적인 수정을 하지 않고 최종 RUDAS-K를 개발하였다. 결론: 체계적인 번역 과정을 거친 RUDAS-K는 치매선별 평가도구로 유용하고 다양하게 사용될 수 있을 것으로 기대된다. Objective: Interest in the management of dementia is expanding due to an increase in the elderly population and consequently an increase in the number of dementia population. Seeing that early detection closely interferes with clinical outcomes, selection of screening tools is critical. The purpose of this study was to develop a Korean version of the Rowland Universal Dementia Assessment Scale (RUDAS-K), which is unaffected by education level, language, and culture, along with assessing frontal function and visuospatial aspects. Methods: The translation process was carried out in nine stages. In the first stage, three occupational therapists translated the original RUDAS into Korean, followed by another occupational therapist collecting three translations. For the third stage, one rehabilitation specialist examined the discrepancy between the original and the translated version. In the fourth stage, verification questionnaires (each question consisting of one sentence from the original RUDAS in English followed by one sentence translated into Korean) were surveyed to find out the similarity betweenthe original text and the Korean translation. In the fifth stage, the Korean translation was revised based on the questionnaire results, and comprehension questionnaires were conducted. In the seventh stage, the three-person translation verification committee confirmed the suitability of Korean translation and subsequently had a professional translator provide a back translation of RUDAS-K. Lastly, an English native speaker and a bilingual occupational therapist verified the similarity of semantics between the original and the back translation. Result: After the first and second stage of translation, RUDAS-K underwent the first consensus verification process, revised the sentences with the lowest scores. Sentences with literal translation were simply revised. Missing texts were paraphrased and added to the liberal translation. Indicating the revised sentences and ones with the lowest score on the comprehension questionnaire, RUDAS-K was sent to the verification committee. As a result, the English education professor and the Korean language professor provided advices on the official Korean language notation while the occupational therapy professor suggested some revision based on vocabularies often found in evaluation used by healthcare professionals. There were no changes made to RUDAS-K after comparing original RUDAS to the back translation. There was one difference in meaning for the translation of “tea” in Korean; it was changed to “green tea" instead of “tea" for the direct translation of “tea” in Korean hold two different meanings. Conclusion: RUDAS-K, which has undergone a systematic translation process, is expected to be a useful and versatile screening tool for those suspected with dementia.
