        흡연이 하부 식도 괄약근에 미치는 영향

        N/A To determine the influence of cigarette smoking on the lower esophageal sphincter(LES), LES pressure was studied by the stationary technique of esophageal manometry in 19 chronic healthy smokers who had no symptoms of gastroeophageal disease. Using a Dent sleeve catheter, LES pressure was recorded continuously in four consecutive 10-minute periods-basal, puffing an unlit cigarette, during smoking, and after smoking. LES pressure was measured from the gastric baseline at end-expiration. The mean basal LES pressure was 13.8(±4.7) mmHg, mean pressure during puffing an unlit cigarette 13.4(±5.0) mmHg, mean pressure during smoking 10.0(±4.2) mmHg, and mean pressure after smoking was 12.8 (±6.3) mmHg. Smoking caused a statistically significant reduction(27.5%) in mean LES pressure compared with the basal period. The drop of LES pressure began 2 min 40 sec(±1 min 50 sec) after the initiation of cigarette smoking, and the pressure remained low until smoking was stopped. The pressure then returned to the basal level 4 min 10 sec(±4 min) after finishing cigarette smoking. In 13 subjects, hypercontraction of the LES was noted after smoking and mean peak LES pressure rose to 19.1(±7.1) mmHg. It is concluded that cigarette smoking causes a reduction in LES pressure, After the reduction in LES pressure, the LES pressure rises higher than the baseline pressure, and this may be explained by rebound excitation.

        N/A To evaluate the relationship between symptoms and esophageal motility disorders, esophageal motility tracings were performed in patients with symptoms of chest pain, dysphagia, lump sense and dyspepsia. Fifty-two (56 of 93 patients with chest pain had abnormal motilities, as did 39 (67%) of 58 patients in the dysphagia group, and 16 (30%) of 53 patients in the lump sense and dyspepsia group. Nutcracker esophagus, nonspecific esophageal motility disorders and hypertensive lower esophageal sphincter were the more frequent esophageal motility disorders than the classic motility disorders such as achalasia and diffuse esophageal spasm. Nutcracker esophagus is the most common single motility disorder in Korea. The esophageal manometric test is a useful technique to evaluate esophageal symptoms of unknown origin.
