상완골 간부 골절(AO Type A, B1, B2)에서 최소 침습적 금속판 고정술과 고식적인 관혈적 금속판 고정술의 비교

목적: 상완 간부 단순 골절에서 관혈적 금속판 고정과 최소 침습적 금속판 고정의 결과를 비교하고자 하였다. 대상 및 방법: 2010년 9월부터 2015년 2월까지 상완골 간부 단순 골절로 최소 침습적 금속판 고정술을 시행한 26예(MIPO군)와 관혈적 금속판 고정술을 시행한 41예(OPEN군)를 대상으로 하였다. 수술시간, 출혈량, 방사선 노출시간, 골유합 시기와 각형성 등을 조사하였다. 최종 추시상 견관절, 주관절의 임상적 결과와 합병증을 조사하였다. 결과: M IPO군과 OPEN군에서 수술시간은 각각 평균 82±23분, 119±20분(p=0.007), 출혈량은 238±67 ml, 303±48 ml (p=0.003), 방사선 노출시간은 201±85초, 20±5초로(p=0.000) 통계적으로 유의하였다. 골유합 시기, 각형성, 임상적 결과는 통계적으로 유의하지 않았다. 합병증은 MIPO군에서 의인성 요골신경마비 2예, 불유합 1예였고 OPEN군에서는 불유합 2예, 연부조직 감염 2예였다. 결론: 상완골 간부 단순 골절에 있어서 정확한 해부학적인 지식 및 적절한 고정물의 선택과 술기가 바탕이 되어 있다면, 최소 침습적 금속판 고정술은 유용한 수술적 치료법으로 생각된다. Purpose: The purpose of this study is to evaluate the efficacy of minimally invasive plate osteosynthesis (MIPO) by comparing the results between open plating and MIPO conducted by simple humeral shaft fractures. Materials and Methods: From September 2010 to February 2015, we evaluated humeral shaft fractures that 26 cases underwent MIPO and 41 cases underwent open plate fixation (OPEN). Operation time, amount of blood loss, and radiative exposure time were examined. Radiographically, bone union time and angulation were compared. At last, UCLA shoulder score and MEPI were used to compare the clinical results of shoulder and elbow and complications were examined. Results: The average operation time 82±23 minutes in MIPO, 119±20 minutes in OPEN (p=0.007) and amount of bleeding 238±67 ml in MIPO, 303±48 ml in OPEN (p=0.003), radiation exposure time 201 ±85 seconds in MIPO, 20±5 seconds in OPEN (p=0.000) were statistically significant. Bone union time and angulations, clinical results were not statistically significant. In Complication, iatrogenic radial nerve paralysis occurred 2 cases, nonunion occurred 1 case in MIPO. Nonunion and soft tissue infection occurred 2 cases each in OPEN. Conclusion: MIPO in simple humeral shaft fractures gave us radiologically and clinically satisfactory results, and may be useful by understanding the anatomical knowledge and using appropriate implants and skills.
