        코어 안정화 운동이 길랭바래증후군 환자의 몸통 근력, 근활성도 및 폐기능에 미치는 영향: 증례보고

        PURPOSE: This study examined the effects of core stability exercise on the strength, activation of the trunk muscle, and pulmonary function in a Guillain-Barre syndrome (GBS) patient. METHODS: A 38-year-old male with GBS was enrolled in the study. A core stability exercise program was implemented for four weeks with a duration of 30 min/day and a frequency of three days/week. The program consisted of abdominal crunch, Swiss ball crunch, bicycle crunch, medicine ball sit-up with a toss, medicine ball rotational chest pass, raised upper body and lower body, and dead bug. Measurements of the strength of the trunk muscle (trunk flexion and hip flexion), activation of trunk muscles (rectus femoris; RA, external oblique abdominal; EOA, internal oblique abdominal; IOA, erector spinae; ES), and pulmonary function (forced expiratory capacity; FVC, forced expiratory volume at one second; FEV1) were taken before and after four weeks of core stability exercise. RESULTS: The strength of trunk muscles increased in the trunk and hip flexion after four weeks of core stability exercise, respectively, compared to the baseline levels. Activation of the trunk muscles increased in RA, EOA, and IOA after four weeks of core stability exercise compared to baseline levels, but decreased in ES after four weeks of core stability exercise compared to the baseline levels. The pulmonary function increased in FVC and FEV1 after four weeks of core stability exercise compared to the baseline levels. CONCLUSION: These results suggest that core stability exercise improves strength, Activation of the trunk muscle, And pulmonary function in patients with GBS.

        양측 및 편측 체중부하운동이 근활성도, 근피로도 및 근긴장도의 비대칭지수에 미치는 영향

        이 연구에서는 양측 및 편측 체중부하운동이 근활성도, 근피로도 및 근긴장도의 비대칭지수(AI)에 미치는 영향을 알아보는데 그 목적이 있다. 건강한 성인 남성 12명을 대상으로 양측의 스쿼트와 편측의 스플릿스쿼트를 무선할당 교차설계방식으로 각각 실시하였다. 우세측과 비우세측의 안쪽넓은근(VM), 가쪽넓은근(VL) 및 큰볼기근(GM)의 근활성도, 근피로도 및 근긴장도를 각각 측정하였다. 근활성도 및 근피로도는 양측 및 편측 체중부하운동 중에 5회, 10회, 15회 및 20회의 반복횟수를, 근긴장도는 체중부하운동 전과 후에 각각 측정하였다. 모든 측정값은 AI 공식을 이용하여 우세측과 비우세측 근육의 AI를 산출하였다. VM, VL 및 GM에서 근활성도의 AI는 체중부하운동에서 유의한 상호작용 효과가 나타났으며(p<.05), 독립표본 t-검정 결과, VM 및 VL에서 근활성도의 AI는 양측과 비교해서 편측 체중부하운동의 20회에서, GM에서 근활성도의 AI는 양측과 비교해서 편측 체중부하운동의 15회 및 20회에서 각각 유의하게 높게 나타났다(p<.05). 또한, VM, VL 및 GM에서 근긴장도의 AI는 반복횟수, 체중부하운동, 반복횟수 및 체중부하운동 간에 유의한 상호작용 효과가 나타났으며(p<.05). 대응표본 t-검정 결과, VM, VL 및 GM에서 근긴장도의 AI는 체중부하운동 전과 비교해서 체중부하운동 후에 편측에서 유의하게 증가하는 것으로 나타났고(p<.05), 독립표본 t-검정 결과, VM, VL 및 GM에서 근긴장도의 AI는 양측 체중부하운동과 비교해서 편측 체중부하운동 후에서 유의하게 높게 나타났다(p<.05). 이상의 결과, 이 연구에서 VM, VL 및 GM의 근활성도 및 근긴장도의 AI는 양측과 비교해서 편측 체중부하운동에서 높게 나타났다는 사실을 증명하였다. The purpose of this study was to examine the effects of bilateral weight-bearing (BWB) and unilateral weight-bearing (UWB) exercises on asymmetry index (AI) of muscle activity, muscle fatigue and muscle tone in healthy men. Twelve healthy men were randomly crossover design assigned to squat of BWB and split-squat of UWB. The activity, fatigue and tone of muscle were measured vastus medialis (VM), vastus lateralis (VL) and gluteus maximus (GM) of dominant and non-dominant. The muscle activity and fatigue were measured repetition of 5, 10, 15 and 20 during BWB and UWB, and the muscle tone were measured of before and after the both weight-bearing exercise. All measured values were calculated AI of dominant and non-dominant muscle using AI formulas. AI of muscle activity in VM, VL and GM showed significant interaction in weight-bearing exercise (p<.05). Independent-sample t-test produced a result, AI of muscle activity in VM and VL were significantly higher in UWB than that BWB during repetition of 20, and AI of muscle activity in GM were significantly higher in UWB than that BWB during repetition of 15 and 20 (p<.05). AI of muscle tone in VM, VL and GM showed significant interaction with repetition and weight-bearing exercise (p<.05). Paired-sample t-test produced a result, AI of muscle tone in VM, VL and GM were significantly increased at unilateral after weight-bearing exercise compared to before weight-bearing exercise (p<.05). Independent-sample t-test produced a result, AI of muscle tone in VM, VL and GM were significantly higher in the after UWB than that BWB (p<.05). This results indicate that AI of muscle activity and muscle tone in VM, VL and GM were remarkably higher in the UWB exercise than that the BWB exercise.

        일회성 경사 트레드밀 걷기운동이 건강한 성인의 운동자각도, 에너지소비량, 근활성도 및 근피로도에 미치는 영향

        Purpose: The purpose of this study is to examine the short-term effects of sloped treadmill walking exercise on rating of perceived exertion, energy expenditure, activity and fatigue of muscles in healthy adults. Method: Sixteen healthy adults (male: 8, and female: 8) were randomly crossover designed and assigned to the level (LV, 0%), uphill (UH, 10%) and downhill (DH, -10%) treadmill walking exercise. Rating of perceived exertion (RPE), energy expenditure, activity and fatigue of muscles (Erector spinae; ES, Gluteus maximus; GM, Biceps femoris; BF, Gastrocnemius; GCM, Vastus medialis; VM, Vastus lateralis; VL, Tibialis anterior; TA) were measured in repetition (T1~T5) during sloped treadmill walking exercise (1.2 m/sec). Results: RPE and energy expenditure were significantly increased at UH compared with those of LV and DH (p<.05, respectively). The muscle activity in the VL was significantly higher at T1, T2, T3 and T4 of DH compared with that of LV (p<.05, respectively). The muscle activity in ES, BF, GCM, VM and TA were also significantly increased at T4 compared with that of T1 of LV, UH and DH (p<.05, respectively). In addition, the muscle fatigue in GM and VM were significantly decreased at T3 and T4 of UH compared with that of LV (p<.05, respectively), and the muscle fatigue in ES, BF, GCM and VM were significantly increased at T3 and T4 compared with that of T1 (p<.05). Conclusion: These results confirmed that RPE, energy expenditure, and muscle fatigue in the VM increased with the time of UH treadmill walking exercise, and the muscle activity in VL was always remarkably higher in DH treadmill walking exercise.
