Objectives: This study was designed to investigate the effects of Lumbricus extract(LE) on the regional cerebral blood flow(rCBF) in ischemic rats, further to determine the mechanism of action of LE, and the effects that LE inhibits lactate dehydrogenase(LDH) activity in brain cells. Methods and materials :This study, ischemic rats were divided into total four group: control group(n=6), experimental group Ⅰ(LE treated group)(n=6), experimental group Ⅱ(LE treated group after pretreatment with indomethacin)(n=6), experimental group Ⅲ(LE treated group after pretreatment with methylene blue)(n=6). And the measurement that LE inhibits LDH activity in the damage to brain cells to N-methyl-D-aspartic acid(NMDA). The changes of rCBF were determinated by laser-doppler flowmetry(LDF), and LDH activity was determinated by microplate reader in vitro. Results :1. The rCBF was significantly improved by LE(10 ㎎/㎏, i.p.) during the period of cerebral reperfusion, compared with the control group. 2. The rCBF was significantly increased by LE after pretreatment with indomethacin(1 ㎎/㎏, i.p.), an inhibitor of cyclooxygenase, during the period of cerebral reperfusion, compared with the LE group, and rCBF was accelerated by LE after pretreatment with methylene blue(10 ㎍/㎏, i.p.), an inhibitor of guanylate cyclase during the period of cerebral reperfusion, compared with the control group. 3. LE significantly inhibited LDH activity in vitro in a dose-dependent manner. Conclusions :From the above results, these were suggested that Lumbricus had anti-ischemia action in connection with cyclooxygenase and might prevent the brain cells death through inhibited LDH activity.
요추 경막외 차단술 후 발생한 하지마비의 한의학적인 치료 효과 -증례 보고-

Objectives: This study was performed to report the effect of oriental medical treatment in the paraplegia and pain after epidural nerve block. Methods: A 39-year-old woman who underwent epidural nerve block at a local clinic was admitted with motor weakness of lower limbs, severe lower radiating pain and decreased sensation when voiding and defecating. We treated her by acupuncture, a herbal medicine, a bee venom injection, moxibustion and cupping treatment and physical theraphy from 11th July 2008 to 14th October 2008. Results: After treatment, most symptoms decreased, VAS(Visual Analog Scale) score changed from 9 to 2. and examinated muscle power changed from 2-3 to 4-5. Conclusions: Our study suggested that oriental medical treatments are significantly effective in the paralplegia and pain after epidural nerve block. And further studies will be aid to identify underlying mechanism of treatment.

Objective : Mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke like episodes (MELAS) syndrome is a progressive neurodegenerative disorder. The typical presentation of patients with MELAS syndrome includes features such as mitochondrial encephalomyopathy, lactic acidosis, and stroke like episodes. Other features, such as seizures, diabetes mellitus, hearing loss, cardiac disease, short stature, endocrinopathies, exercise intolerance, and neuropsychiatric dysfunction are clearly part of the disorder. Approximately 80% of patients with the clinical characteristics of MELAS syndrome have a genetic mutation. This progressive disorder is reported to have a high morbidity and mortality. This case report is intended to estimate clinical effects of oriental meedicinal treatment of MELAS syndrome. Methods : A 44 year old female patient diagnosed as MELSA syndrome was treated with general oriental medicinal therapy including acupuncture, moxibustion, cupping, pharmacupunture and herbal medication in 20XX in Wonkwang Medical Center Gwangju. And the changes in symptoms and signs were evaluated as time dependently. Results : Although there is currently no curable treatment and MEALS syndrome is tend to progress, our treatment showed improvement in general weakness, gait disturbance and pain in the patient. Conclusion : Our case report suggests that various oriental medicinal treatment could be effective for improvement of MELAS syndrome and may represent a new potential therapeutic approach to control the disease. It could be applied to improve general condition, prevent relapse, enhance the quality of life and reduce complaints in the patient.
교통사고로 발생한 외상성 대퇴부 피하혈종의 한의학 치료 효과

Objectives: This study was performed to report the effect of oriental medical treatments in one patient with traumatic thigh subcutaneous hematoma by traffic accident. Methods :The patient was treated by acupuncture, herbal medicine, pharmacopuncture and cupping for 3 months. Results :After treatments, VAS(visual analog scale) score changed from 10 to 0. MR Imaging showed that the size of subcutaneous hematoma has decreased from 5×11×5.8 cm to 2.4×3.2×2.6 cm. And the palpation results also showed that the size of subcutaneous hematoma has gotten smaller. Conclusions :Our study suggests that oriental medical treatments such as acupuncture, herbal medicine, pharmacopuncture and cupping may be effective in traumatic subcutaneous hematoma. More cases of controlled treatments are needed to define the effect of oriental medical treatments for traumatic subcutaneous hematoma.

Objectives :This study was performed to report the effect and safety of oriental medical treatment in the pregnant woman diagnosed to herniation of nuclues pulposus(HNP) at L5-S1 with central protrusion and left S1 nerve root. Methods :A 32-year old pregnant woman diagnosed HNP at L5-S1 at local hospital was admitted with low back pain and left leg rad pain. We treated her by acupuncture, a herbal medicine, cupping treatment and measured visual analogue scale(VAS), Oswestry disability index(ODI) and Roland-Morris disability qustionnaire(RMDQ) from 5th March 2010 to 26th March 2010. Results :After treatment, most symptoms decreased, VAS score changed 10 to 2, ODI changed 488 to 188, RMDQ changed 13 to 6. Conclusions :In this study, oriental medical treatment was effective and safe in pregnant woman with HNP at L-spine. But the rigorous studies will be needed to define clearly that oriental medical treatment is effective and safe in pregnant women with HNP at L-spine.
반월상 연골 손상에 대한 관절강내 봉약침 치료 증례보고 4례

According to previous reports, intra-articular bee venom injection is very effective for synovitis, intra-articular lesions. In this article, we report 4 cases of well-treated meniscal tear with intra-articular bee venom injection. We used intra-articular bee venom injection on meniscal tear diagnosed by MR imaging and prescribed herbal medication, physiotherapy if necessary. Outcomes were measured by Visual Analogue Scale(VAS) and Lysholm Knee Scoring Scale. Patients who are treated by intra-articular bee venom injection had a significant effect on the pain reduction, improved range of motion and knee function. Further well-designed, controlled studies and more cases are needed to define the effect of intra-articular bee venom injection on knee.

Objectives: The aim of this study is to suggest approach of oriental medical management and necessity by research of eastern and western medical literatures for Complex Regional Pain Syndrome(CRPS). Methods: We reviewed the clinical and experimental literatures of eastern and western concerned with CRPS which is related causation, signs and symptoms, diagnosis, etiology and management. Results: 1. CRPS is divided into type 1 and 2 by nerve injury. Two types of CRPS have been recognized: type 1, corresponds to RSD and occurs without a definable nerve lesion, and type 2, formerly called causalgia refers to cases where a definable nerve lesion is present. These conditions can be charaterized clinically by the sensory abnormalities, vascular abnormalities, oedema, sweating abnormalities, motor or trophic changes. 2. CRPS are well known to patients and physicians relatively, but the pathophysiology, causation and treatments are still unclear. 3. CRPS is needed to take the early diagnosis and multidisciplinary approach for significant effect. 4. CRPS can be regarded for obstruction syndrome of Ki and blood(痺證), blood stasis(瘀血), Wei symptom(痺證), numbness(痲木) in the oriental medical management of CRPS. Conclusions: Above the results, it is suggested that further studies and active approach of management of CRPS will be conducted precisely in oriental medicine.
부정항암탕(扶正抗癌湯) 추출액이 Diethylnitrosamine과 CCl<sub>4</sub>로 유발된 흰쥐의 간암 형성 및 간경화에 미치는 영향

Objective : Bujeonghangam-tang(BH) has been used for cure of tumor as a traditional medicine. This study was carried out to clarify the effect of BH extract on hepatocellular carcinogenesis and hepatic cirrhosis induced by Diethylnitrosamine(DENA) and $CCl_4$ in Rats. Method : Experimental groups were divided into two, 8th and 12th week groups, and subdivided into four; normal group(Nor), cirrhosis and hepatic cancer inducing control group(Con), and BH extract 320 mg/kg/day(BHA) or 640 mg/kg/day(BHB) administered groups to Con. Results: In the 8th week group: The body weight decreased significantly in Con compared with the Nor. The activities of transaminase, alkaline phosphatase(ALP), and lactacte dehydrogenase(LDH) were significantly increased(p<0.05) in the Con compared with Nor, but decreased in the BHA and BHB compared with Con. Alpha fetoprotein(AFP) were the most increased in the Con compared to BHA and BHB. The results of light microscopical observation, a number of hepatocytes were damaged in the Con compared with Nor and BH extract administerd groups. The number of hepatic p53 positive cells was reduced in the BH extract administered groups. According to the electron microscopical observation, hepatocarcinoma cells were observed distinctly in the Con compared with BH extract administered groups. In the 12 weeks group: The results of body were similar to 8th week groups. The activities of transaminase and ALT were significantly increased(p<0.05) in the Con compared with Nor. LDH was significantly(p<0.05) increased in the Con compared with Nor but significantly (p<0.05) decreased in the BHB. Alpha fetoprotein(AFP) were the most increased in the Con among ex perimental groups. The activities of superoxide dismutase(SOD) were significantly (p<0.05) increased in the Con, but the activities of catalase were not increased(p<0.05) compared with Nor. The number of hepatic p53 positive cells was increased in the Con. The results of electron microscopical observation were similar to 8th week groups. Conclusion : These results suggest that ad ministration of BH extract suppress or retard DENA and $CCl_4$-induced hepatocellular carcinogenesis and hepatic cirrhosis in rats.

Objectives : The purpose of this study was to investigate the effects of modified fasting therapy on body composition, blood cell count and blood chemistry in inpatients. Methods : The modified fasting therapy program consists of three phases, a period of reducing food intakes, fasting and refeeding. Body weight, body composition, and the degree of obesity were estimated in before fasting therapy and after refeeding. Blood cell count and blood chemistry were estimated in before fasting therapy, before refeeding and after refeeding. Results : After modified fasting therapy, body weight, body fat mass, percent body fat, body mass index(BMI), body matabolic rate(BMR) and muscle mass was decreased significantly. WBC count and platelet count decreased significantly, RBC count and hemoglobin increased significantly and hematocrit was not changed significantly but within normal limit. AST and ALT increased significantly, especially patients group over BMI 25 within normal liver function. This study was not founded correlation between decreased muscle mass and increased liver function test. ALP, BUN, Total-cholesterol, and triglyceride decreased significantly. Creatinine increased significantly, but within normal limit. Sodium was not changed significantly, potassium decreased significantly but within normal limit. Conclusions : These results suggest that modified fasting therapy will be beneficial if several measures complement.
