The Effect of Gender and Foot Landing Type on Lower Extremity Biomechanics During Single-Leg Landing

The purpose of this study was to examine the effect of gender and foot landing type (forefoot vs. rearfoot landing) on kinematics, kinetics, and energy absorption of lower extremity joint. Twenty males and twenty females performed single-leg landing with two different foot landing types: forefoot landing and rearfoot landing. Three-dimensional kinematic and kinetic parameters were measured using motion capture system. Greater knee valgus angle at peak vertical ground reaction force (p = 0.034) during rearfoot landing increased the risk of anterior cruciate ligament (ACL) injury in females as increasing valgus positioning from neutral alignment could increase the load on ACL. Greater contribution of ankle joint and less contribution of hip joint in energy dissipation were found in females during both forefoot (p = 0.029 and p = 0.016, respectively) and rearfoot landing (p = 0.003 and p = 0.016, respectively). These results suggest that increasing muscular activity of ankle plantarflexor could reduce shock transmission to the proximal joint in females. In addition, greater hip joint’s contribution to total negative work in males induced lower hip flexion angle found in both forefoot and rearfoot landing by elevated activation of the hip extensor. In conclusion, landing strategy differs between genders in both forefoot and rearfoot landing.

The purpose of this study is to suggest novel surgical technique for irreparable rotator cuff tear patients; superior pylon reconstruction (SPR) by comparing its surgical effect with superior capsular reconstruction (SCR) by a simulation study. We constructed total 4 models; intact, tear, SCR and SPR models based on MRI. Total 10 male subjects (age, 25±4 year old; weight, 66±10 kg; height, 170±5cm) implemented shoulder abduction in scapular plane. Range of motion (ROM), acromio humeral distance (AHD), and superior-inferior (SI) translation of humerus of SPR and SCR models were compared. There’s no significant difference in ROM between two models, however in AHD, SPR model increased by 0.4 mm (p = 0.001) compared with SCR model. SPR models showed decreased maximum superior translation of humerus by 1.0 mm (p = 0.001) compared with SCR model. These results implied that SPR could restore shoulder joint kinematics of massive irreparable rotator cuff tear patients as effectively as SCR.

Rotator cuff refers group of muscles surrounding shoulder joint which are supraspinatus, infraspinatus, subscapularis and teres minor. Tear of these muscles results in abnormal kinematics of shoulder joint. Previous study reported anterosuperior tear patients did not recover range of motion (ROM) despite rehabilitation. Purpose of this study is to find out why rehabilitation failed on anterosuperior tear patients. Two types of shoulder models were constructed: intact and anterosuperior (supraspinatus and infraspinatus) tear model. Anterosuperior tear model’s elevation ROM was observed in 3 different conditions: enhancing deltoids, teres minor and both muscles. Results showed that enhancing only teres minor increased ROM. However, enhancing only deltoids and enhancing both deltoids and teres minor did not, and impingement occurred. These results implied strengthening deltoids and teres minor in equal ratio could not be appropriate rehabilitation for anterosuperior tear patients.

Dynamic balance is an ability to maintain stable position while performing various tasks and it has been reported that dynamic balance associated with athletic performance and injury risks. The purpose of study was to examine the effect of core strength on dynamic balance. Nineteen males completed 10-week core training interventions. To test the core strength, a trunk endurance test was used. During this test, the plank and side-plank positions were maintained for as long as possible. The star excursion balance test (SEBT) were conducted before and after core training to measure the dynamic balance. Plank records and SEBT parameters were compared using a paired Student’s t-tests at a significance level of 0.05. Greater plank and left/right side plank record were found in post-training (p=0.008, p=0.049, and p=0.007 respectively), thus the core muscle strength increased after training. As the SEBT results, greater anterior, anteromedial, medial, posteromedial, posterior, posterolateral reach distances were found between pre- and post-training, but there was no significant differences in lateral and anterolateral distances. These results indicate that strengthening of core muscle increase the dynamic balance, thereby it positively affected in lower extremity injury risk.
