1990년대 후반부터 공립요양병원이 설립되었고, 2018년 기준, 79개소로 전체 요양병원의 5.1%를 차지하고 있다. 문재인 정부에서 치매 국가책임제를 발표하면서 공립요양병원을 치매안심병원으로 지정하여 중증치매환자에 대한 전문치료를 수행하는 기관으로 육성하고자 다양한 정책을 펼치고 있다. 이와 관련하여 2018년에 치매관리법을 개정하여 공립요양병원 운영평가를 위한 법적 근거가 마련되었으며, 2020년부터 본 평가가 시작되었다. 본 연구는 2020년도 공립요양병원 운영평가를 시행하기 위한 연구의 일환으로 공립요양병원을 치매환자의 전문치료기관 및 회복기 재활환자를 치료할 수 있는 역량 있는 기관으로 육성할 수 있도록 기능을 정립하였으며, 양질의 진료서비스, 공공성, 책무성 등 공공기관으로서의 행태를 평가할 수 있도록 평가체계를 개발하였다. 또한 개발된 평가지표가 적용가능한지 검증하기 위해 공립요양병원 79개소 중 20개소를 대상으로 시범 평가를 시행하였다. 본 평가의 결과를 바탕으로 교육 및 피드백이 진행되고 지속적으로 관리 할 수 있는 모니터링시스템을 구축함으로써 전체 요양병원의 기준(standard)으로서의 기능과 역할을 수행할 수 있을 것으로 기대한다. 구강건조증이 구강건강관련 삶의 질에 미치는 관계에서 심리적 요인의 매개효과연구

Objectives: The aim of this study was to investigate the role of psychological factors as mediators in the casual relationship between mouth dryness and oral health-related quality of life of adults and to explore gender differences in what psychological factors play an important role in mediating the path of mouth dryness to quality of life. Methods: Self-administered questionnaires were distributed to 321 adults who are above 20 and under65 years of age. Collected data were analyzed via statistical packages using a multi-group analysis through the SEM (Structural Equation Model). Results: Based on the statistical analysis, stress was a major psychological factor in male subjects. While depression served as a main mediator in female subjects. We discovered that the path coefficients of the following paths showed significant gender differences: mouth dryness → depression, depression→ quality of life, stress → quality of life. Conclusions: A gender-specific approach regarding effective prevention of stress and depression maybe important for understanding the relationship between mouth dryness and quality of life.

이 연구는 최근 노인들의 스트레스와 우울, 자살생각 등 정신건강 폐해의 심각성을 인지하고 이에 영향을 미칠 것으로 예상되는 삶의 질, 건강행태, 건강수준 등과 정신건강 간의 구조적인 관계를 검증하고자 수행하였다. 연구 자료는 전 국민을 모집단으로 체계적인 표본조사를 실시한 2009년 국민건강영양조사의 원자료에서 65세 이상 노인들 중 연구에 적합한 1,490명을 최종 연구대상자로 선정하였다. 연구결과 첫째, 노인들의 삶의 질이 높을수록 스트레스, 우울, 그리고 자살생각 등을 감소시키는 효과가 있었다. 또한 삶의 질은 건강수준을 강화시켜 정신건강을 증진시키는 효과가 있었다. 둘째, 흡연, 문제음주, 비만, 적당한 운동과 수면 등으로 측정한 건강행태가 정신건강에 직접적인 영향은 없었으나, 건강수준을 매개로 한 간접효과를 확인하였다. 이 연구결과를 기반으로 노인의 정신건강 증진을 위해서는 삶의 질 제고를 우선적으로 고려하면서, 건강행태를 변화시키려는 체계적인 노력이 중요하다 하겠다. This study recognizes the seriousness of mental health damage in the aged these days including stress, depression, suicidal ideation and intends to verify structural relationship between mental health and those expected to affect it such as health behavior, quality of life, and subjective consciousness of health. According to the results of this study, first, as the aged’s quality of life is higher, it has more effect to reduce stress, depression, or suicidal ideation. Also, quality of life enhances subjective consciousness of health and improves mental health. Second, although health behavior measured by smoking, problematic drinking, obesity, or moderate exercise and sleep did not show direct effect on mental health, it has indirect effect on it with subjective consciousness of health as mediation. Based on the result of this study, in order to improve the aged’s mental health, it is needed to consider the enhancement of their life quality in advance and make systematic efforts to change their health behavior.

지역사회 돌봄 보건인적자원 관리 방안: 한국, 일본, 영국의 사례

In response to environmental changes driven by population ageing, the emergence of integrated care systems, and a notable shift towards home-based service delivery, dedicated efforts are underway to advance the Korean-style community care system. Acknowledging the importance of education, training, coordination, and collaboration among diverse health human resources involved in this initiative, this study conducts a comparative analysis of cases in Korea, Japan, and the UK to explore effective health human resources management strategies. The analysis findings underscore the imperative to redefine the presently evolving and ambiguous roles among health human resources, emphasizing the critical need for legal frameworks and systems to integrate and manage these human resources seamlessly. This study suggests reinforcing education and retraining programs to nurture health human resources capable of assuming pivotal roles bridging various disciplines, drawing inspiration from successful models such as Japans care managers and the UKs care coordinators. Furthermore, it proposes enhancing networks that foster a deeper understanding of each others roles and expertise, promoting more effective intervention strategies. To cultivate high-caliber health human resources and elevate their capabilities, the study recommends improvements to the working conditions in community care, the establishment of motivating reward systems, and the development of practical training programs designed to contribute to improved job performance directly.
