        간호대학생 대상의 국제보건역량강화 프로그램 효과: 체계적 문헌고찰 및 메타분석

        Purpose: This study aimed to evaluate the effects of global health competency education on cultural competence, empathy, and cultural acceptance among nursing students in Korea. Methods: A literature review was performed until July 2021 in domestic electronic databases, including KoreaMed, RISS, KISS, and Google Scholar. Nine references that met the inclusion and exclusion criteria were selected and Meta-analysis was performed using Cochrane’s risk of bias tool and the R version 3.5.2 (Meta-analysis with R) program. Results: The findings in this study indicate that effect size of global health competency education for cultural competence was Hedges'g=1.33 (95% confidence interval, 0.78~1.89) and the effect size of empathy was Hedges'g=0.82 (95% confidence interval, 0.07~1.57), as indicated by a large effect size. The effect size of cultural acceptance was Hedges'g=0.49 (95% confidence interval, 0.06~0.92), as indicated by a moderate effect size. The differences were statistically significant. In the meta-subgroup analyzed by approach intervention type, intervention time had a significant effect. The risk of publication bias is low. This study found that the global health competency education program for nursing students had an effect on cultural competence, empathy, and cultural acceptance. Conclusion: Various global health competency programs should be developed and implemented for nursing students to improve their multicultural health competencies.

        간호대학생의 동성애에 대한 태도 영향요인 ; 성적 자기주장과 성역할 고정관념 중심으로

        본 연구는 간호대학생의 성적 자기주장, 성역할 고정관념 및 동성애에 대한 태도 정도를 확인하고, 간호대학생의 동성애에 대한 태도에 영향을 미치는 요인을 파악하기 위한 연구이다. D광역시에 소재하는 2개 대학교 간호대학생 253명을 대상으로 2019년 4월 11일부터 4월 20일까지 설문조사를 실시하였다. 분석방법은 SPSS/WIN 22.0 프로그램을 이용하여 연구대상자의 일반적 특징, t-test, ANOVA 분석, 상관분석, 다중회귀분석(multiple regression)을 실시하였다. 연구결과는 다음과 같다. 성적 자기주장은 평균 평점은 4.44±0.37점, 성 역할 고정관념은 2.08±0.58점, 동성애 태도는 2.75±0.37점로 나타났다. 일반적 특성에 따른 동성애 태도는 종교(F=13.32, p <.000), 종교생활(F=20.43, p <.000), 부모님의 양육태도(F=2.74, p <.044)가 통계적으로 유의한 차이가 있는 것으로 나타났다. 대상자의 성적 자기주장과 동성애의 태도(r=-0.13, p =.034)는 유의한 음의 상관관계가 있으며, 성역할 고정관념과 동성애의 태도(r=0.24, p <.000)는 유의한 양의 상관관계가 있는 것으로 나타났다. 대상자의 종교, 종교생활, 성역할 고정관념이 동성애 태도에 유의한 영향요인으로 나타났으며, 영향요인의 설명력은 23.1%로 나타났다. 본 연구 결과를 토대로 간호대학생의 동성애 태도와 관련된 중재전략이 필요하고, 건강한 성 정체성을 위한 실천 방안과 교육 프로그램 개발이 필요할 것이다. The purpose of this study was to determine the relevance and different factors having an impact on sexual assertiveness and sex stereotypes for the attitude among nursing students toward homosexuality. A survey was conducted on 253 nursing students in D city using a self-report questionnaire. The data was collected from April 11 to April 20, 2019 and it was then analyzed by descriptive statistics, t-tests, ANOVA, Pearson’s correlations and multiple regression using SPSS 22.0. The average of sexual assertiveness in the nursing students was 4.44±0.37, and the average of sex stereotypes was 2.08±0.58, and the average of attitude toward homosexuality was 2.75±0.37. The attitude toward homosexuality according to the general characteristics differed among religions (F=13.32, p <.000), religious life (F=20.43, p <.000) and method of parenting (F=2.74, p <.044). There was a statistically significant negative correlation between sexual assertiveness and the attitude toward homosexuality (r=-0.13, p =.034). Also, there was a statistically significant positive correlation between sex stereotypes and the attitude toward homosexuality (r=0.24, p <.000). Religion, religious life and sex stereotypes were the influential factors. Models that included the variables explained 23.1% of the variance for the attitude toward homosexuality. Thus, it is necessary to create an interventional strategy related to nursing student’ attitude towards homosexuality so as to develop a practical educational program for promoting healthy gender identity.

        자궁양성종양 환자의 수술 사전 교육이 불안, 불확실성, 통증에 미치는 효과: 체계적 문헌고찰 및 메타분석

        본 연구는 자궁양성종양 환자의 수술 사전 교육이 불안, 불확실성, 통증에 미치는 효과를 분석하기 위해 수행된 체계적 문헌고찰 및 메타분석 연구이다. 문헌검색을 위해 PubMed, OVID MEDLINE, CINAHL, Google Scholar 및 학술연구정보서비스(RISS), 한국학술정보(KISS)등의 데이터베이스를 활용하였으며, 2020년 1월까지 검색하였다. 총 401개의 연구가 확인되었으며 선택배제 과정을 거쳐 최종적으로 13편이 분석에 포함되었다. 문헌의 비뚤림 위험은 RoB(Risk of Bias)와 Robans를 이용해 평가하였으며, 전반적으로 비뚤림 위험은 낮은 것으로 평가하였다. 연구결과 수술사전 교육은 불안 (Hedges’s g= -0.89, 95 % CI –1.39, -0.47)과 통증 (Hedges‘s g= -0.49, 95 % CI –0.95, -0.02)에는 통계적으로 유의한 차이가 있는 것으로 나타났으나, 불확실성( Hedges’s g = -1.38, 95 % CI –3.98, 1.23)은 통계적으로 유의한 차이가 없는 것으로 나타났다. 하위그룹 분석결과 중재방법에 따라 인지행동프로그램, 시범교육, 이완요법이 효과가 있는 것으로 나타났으며, 출판 편향의 위험은 낮았다. 이를 토대로 향후 자궁양성종양 환자의 수술 사전교육에 적용할 수 있을 것이라 사료된다. This study aimed to evaluate the impact of education and counselling by nurses on anxiety, uncertainty, and pain in patients with benign uterine tumors. Electronic databases, including PubMed, OVID, CINAHL, Google Scholar, and Korean databases and were searched during January 2020. A total of 401 studies were identified of which 13 were suitable for meta-analysis. Cochrane’s risk of bias tool and the R version 3.5.2 (Meta-analysis with R) program were used for analysis. The authors performed a meta-analysis of the 13 trials that met eligibility criteria. The findings in this study indicate that the effect size of nursing education for anxiety was Hedges’ g=-0.89 (95% CI:-1.39 to -0.47), as indicated by a “large effect size” and the effect size of pain was Hedges’ g=-0.49 (95% CI:-0.95 to -0.02) as indicated by a "moderate effect size". The effect size of uncertainty was Hedges’ g=-1.38 (95% CI:-3.98 to 1.23), and it was not statistically significant. In the meta-subgroup analyses by approach intervention type, cognitive-behavioral programs, demonstration intervention, and relaxation therapy had a significant effect. The risk of publication bias was low. It may therefore be concluded that pre-operative education by nurses for patients with benign uterine tumors would affect their levels of anxiety, uncertainty, and pain.

        간호대학생 대상의 고충실도 시뮬레이터를 이용한 분만 간호 교육 프로그램의 개발 및 효과

        Purpose: This study was conducted to investigate the effects of an education program using a high-fidelity simulator of labor and delivery on nursing knowledge, critical thinking, and clinical performance among nursing students who had not yet experienced clinical practicum. Methods: The development of a 5-week maternity nursing education programs using high-fidelity simulators included modules containing case-oriented scenarios, knowledge, and skills required for maternity care. A randomized controlled study was conducted to verify the effects of the developed program. Data were collected from October 21 to December 9, 2019. The experimental group (n=36) participated in a 5-week high-fidelity simulation program on care for the woman in labor, whereas the control group (n=36) received standard education as lecture and practice with delivery model. The collected data were analyzed using descriptive statistics (frequency, percentage, mean, and standard deviation), the Chi-square test, Fisher exact test, and t-test. Results: For participants who received education using the high-fidelity simulation program, nursing knowledge (t=2.33, p=.011), critical thinking (t=3.73, p<.001), and clinical performance (t=2.53, p=.006) were significantly higher than in the control group. Conclusion: Even for students with no clinical experience, high-fidelity simulation-based nursing education was effective in improving nursing knowledge, critical thinking, and clinical performance among nursing students. Nurse educators will be able to use this high-fidelity simulator effectively, especially in situations where direct clinical practicum may not be feasible.

        자궁양성종양 환자의 고강도 초음파 집속술(HIFU) 치료 경험

        본 연구의 목적은 현상학적 접근을 통하여 고강도 초음파 치료를 받은 자궁양성종양 환자에 대한 경험의 본질을 심도 있게 파악하기 위해 이루어졌다. 연구 대상자는 자궁양성종양을 진단받고 고강도 초음파 집속술 치료를 받은 5명의 환자였다. 본 연구의 자료 수집은 2018년 2월부터 3월까지 이루어졌으며, 고강도 초음파 집속술을 시술하고 있는 D시 여성전문병원에서 시행되었다. 연구방법은 일대일 심층 면담을 통해 이루어졌으며, 자료는 비구조적이고 개방된 질문형태를 사용하여 자신의 의견과 경험을 자유롭고 유연하게 표현할 수 있도록 하였다. 인터뷰 자료는 Colaizzi의 현상학적 분석 방법을 사용하였다. 연구결과 5명의 대상자로부터 얻은 고강도 초음파 집속술 치료에 따른 건강문제는 총 3개의 범주로 구성되었고, 각각의 범주는 다시 하부 요인으로 구성되었다. 3개의 범주로는 ‘불편감’, ‘양가감정", ‘의지와 요구" 로 나타났다. 본 연구는 고강도 초음파 치료를 받은 자궁양성종양 환자들의 경험을 이해하는데 도움을 줄 것이다. 나아가 본 결과는 향후 여성건강을 위한 새로운 치료법의 개발과 발전에 따라 올바른 간호학적 이해 및 고강도 초음파 치료를 받는 환자에 대한 지속적인 연구와 효과적인 간호 교육프로그램 개발이 이어져야 할 것이다. The purpose of this study was to identify the experience of patients with uterine benign neoplasms in response to high-intensity focused ultrasound treatment. The study participants were patients who received high-intensity focused ultrasound treatment after diagnosis of benign uterine neoplasms and were contacted through purposive techniques. Five patients agreed to participate in this study. Data were collected from February, 2018 to May, 2018 at Women`s hospital in D city. The interview data were analyzed by the phenomenological analysis method developed by Colaizzi using open-ended questions during in-depth interviews. Three categories emerged from the analysis using Colaizzi"s method: 1) Discomfort. 2) Ambivalence. 3) Will and need. These study results contribute to a deeper understanding of the experience of patients with uterine benign neoplasms in response to high-intensity focused ultrasound treatment. Therefore, it should be developed as part of an effective nursing education program to provide evidence-based guidelines and nursing interventions that alleviate health problems in patients receiving high-intensity focused ultrasound treatment and proper nursing understanding of the development of new treatment methods for woman"s health.

      • 유방암 환자 대상의 운동 중재 프로그램이 기능회복과 삶의 질에 미치는 효과: 체계적 문헌고찰 및 메타분석

        Purpose: This study was carried out to evaluate the efficacy of exercise program on recovery of functional and quality of life in breast cancer patients through systematic literature review and meta-analysis. Methods: The following databases were used to search the literature: RISS, KISS, KMbase, and KoreaMed. Keywords included 'breast cancer', 'mastectomy' 'exercise', 'rehabilitation', 'intervention' and the evaluated articles were published from 2009 to 2018. The statistical was used R program, the effect size of shoulder functionality, grip strength, upper limb, pain and QOL were calculated by random-effects model. Results: 7 RCT studies were meta-analyzed and were evaluated for the risk of bias by the RoB of Cochrane Collaboration; the overall risk of bias was low. The effect sizes of exercise program was flexion (g=1.11), abduction (g=0.97) as indicated by a "large effect size". Conclusion: It is necessary to study the develop and apply a exercise programs to improve the function and quality of life in breast cancer patients.

      • 모유 수유 개별 방문 프로그램의 효과: 체계적 문헌고찰과 메타분석

        Purpose: This study was a systematic review and meta-analysis designed to evaluate the effects of breastfeeding individual visit program. Methods: Based on the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA), a systematic search was conducted using four core electronic databases from October 28, 2018 to November 14, 2018. The topics of breastfeeding individual visit program were analyzed using descriptive analysis and the effects of intervention were meta-analyzed using the Review Manager 5.3 software. Results: A total of 13 studies were included in the review and 12 were included for meta-analysis. Breastfeeding individual visit programs were found to be more effective in breastfeeding rates, attitude, skill and self-efficacy compared to group education or non-education. Conclusion: Effective breastfeeding education can help mothers to start and sustain breastfeeding. Breastfeeding individual visits programs that are more effective than group education to maintain breastfeeding for pregnant women should be developed and applied.
