폐경이행기 및 폐경기 여성에서 몰핀의 시상 하부-뇌하수체 축에 미치는 영향에 관한 연구

1986년 5월부터 1986년 8월까지 연세의료원 산부인과에 입원한 폐경이행기 및 폐경기 부인 9명을 대상으로하여 몰핀을 정맥투여하고 각 호르몬치의 변화 양상을 관찰하여 다음과 같은 결론을 얻었다. 1. 황체화호르몬은 몰핀 투여후 15분부터 감소하는 경향을 보였으며, 45분 후에는 의의있는 감소를 보였다. 2. 난포자극호르몬은 90분에 걸쳐 의의있는 변화가 관찰되지 않았다. 3. 프로락틴치는 몰핀 투여 후 15분에 의의있게 증가하였으며, 약 45분에 이르기까지 증가 양상은 지속되었다. 4. 성장호르몬은 8예 중 3예에서 몰핀투여 후 30분에서 의의있게 증가 되었으나, 나머지 5예에서는 의의있는 변화가 관찰되지 않았다. It has been previously documented by some that exogenous and endogenous opioids could result in some functional changes of pituitary by eay of activating opioid receptors in humans and experimental animals. In humans, as has been reported, administration of morphine or DAMME(D-alaninemetenkephaline amide) brought about the increase in the level of serum prolactin often accompanied by the rise in serum growth hormone(GH) as well; these observations sometimes lacking reproducibility were subject to controbersy. Bruni, et al. in 1974 reported that exogenous and endogenous opioids each was capable of evoking changes in the level of lutenizing hormone(LH), a gonadotropin, in humans and experimental animals. Opioids are said to regulate secretion of gonadotropin releasing hormine(GnRH) through the actions of dopaminergic and noradrenergic neurotransmitters. Prolactin release is considered to be mediated by dopamin. This study, involving nine postmenopausal women, was designed to make observations of the patterns of changes in the serum levels of LH, follicle-stimulating hormone(FSH), prolactin, and GH serially by assaying the samples of peripheral blood drawn out every 15 minutes from each patient after intravenous administration of 5mg morphine sulfate(acting principally on -receptors, partially acting on -receptors). The results of the study were as follows; 1. The serum levels of LH started to decrease at 15 minutes following morphine influsion, reaching the levels that were significantly low at 45 minutes(P<0.05). 2. The levels of serum FSH failed to change significantly for 120 minutes after the infusion. 3. The levels of serum prolactin were significantly elevated at 15 minutes and maintained this pattern of increase until around 90 minutes. 4. The levels of serum GH showed significant increase in three of the nine women at 30 minutes and further increased until 90 minutes.
