융모종양에 대한 약물치료의 고찰 - Methotrexate 저항성을 중심으로

Methotrexate treatments were done for 4 cases of metastatic choriocarcinoma and one case of chorioa- denoma destruens, with the result that the complete clinical healing was gained in one, while in 4 the resistance to methotrexate treatment developed. The followings are the summary of the result: 1)It seems that delayed initiation of treatment is followed with more liable resistance formation. 2)Effect of methotrexate was more pronounced on the vaginal metastatic growth than the pulmonary metastasis. The vaginal new growth could not be observed even after the resistance developed, whereas in the lung new growth or exacerbation of the old tumor could be observed. 3)The repeated cures or increments of the durg were not effective to reduce the resistance to meth- otrexate. Review of the literature and athor`s own clinical experience showed taht treatment in meth- otrexate-resistant cases should be switched on other anticarcinogenic agent. 4)Formation of resistance to methotrexate can reasonably be judged by Friedman rabbit unit, when the unit remains unchanged even after 2 cycle treatment of methotrexate or the suppressed unit shows increase with cessation of treatment or even under methotrexate regimen.

96 cytohormonal patterns of 57 cases of physiological menopause, 22 cases of men- opause after both side adnexectomy and 17 cases of menopause after Wertheim operation and radiation theraphy were examined by Papanicolaou stain and following results were obtained; 1) Of the 96 cases, proliferation was high in the 14 cases(14.6%), moderate in 44 cases(46,9%) and absent in 38 casesa(39.5%) 2) During the early period of menopause moderate proliferation was common finding, but after 10 year absence of proliferarion was prominent. 3) From the study of 57 physiological menopause it was noted that stimulation of sex jorm- one was more active during the first 3 year than after 3 year. 4) From the study of 62 cases which were under the 3 year of manopausal age, physiologcial menopausal group being 6 to 8 year older than other groups showed high proliferation of 3 to 6 times than other 2 groups. It might be suggested that menopausal ovary also produced the endogenous estrogen. 5) Frequency of Doderlein bacillus detection was influenced by the effect of esteogen as foll- ow; 6 cases(42.85%) in high proliferation(38 cases). 5 caese (11.36%) in moderate proliferation (44 cases). None in absent proliferation(38 cases). 6) Atrophic change of vaginal mucosa and external genitalia was examined in each group of cytohormonal pattern. None in high proliferation. 7 cases(15.9%) in moserated proliferation. 33 cases (86.84%) in absent proliferation. 7) Vaginitis was found in 28.57% of high proliferation, 36.36% of moderate proliferation and 60.52% of absent proliferation. 8) It was cleared that senile vaginitis showing the absent proliferation needed the treatment with estrogen.
