ICF 국내 활용도 향상을 위한 ICF교육과 이러닝툴에 대한 제안

본 연구의 목적은 ICF 교육과 관련된 문헌검토를 통해 ICF교육이 건강과 장애분야에 어느 정도 활용되고 있는지 알아보고, WHO에서 개발한 ICF 이러닝툴과 관련된 문헌(ICF 커리큘럼 모델, KCF 책자) 비교를 통해 발전방안을 제안하는 것이다. 이를 위해 ICF 교육관련 연구들을 수집하고, 연구참여자, 교육/훈련 제공방법, 교육/훈련의 목적으로 구분하였으며, ICF 커리큘럼 모델과 ICF 책자를 기반으로 이러닝툴에 추가되어야 하는 내용을 분석하였다. 연구결과를 보면 건강과 장애분야에서 ICF 교육에 대한 관심이 2008년 이후부터 꾸준하게 증가하는 것으로 나타났다. 교육대상자들이 점차 다양화되어 가고 있으며, 온-오프라인 교육이 혼합되어 제공되며, 이론-실습교육이 함께 진행되는 경향으로 나타났다. ICF 커리큘럼 모델과 KCF 책자를 기반으로 이러닝툴을 비교한 결과 ICF 사용시 윤리적 지침, 올바른 자료수집을 위한 가이드라인, 문서화 및 코딩방법에 대한 내용이 추가되어야 하는 것으로 나타났다. 이러한 결과를 기반으로 ICF교육에 대한 효과적인 발전 방안과 차후 연구 등을 제시하였다. This study aimed to investigate how the International Classification of Functioning, Disability, and Health (ICF) has been integrated into health and disability education through a literature review of ICF education and to compare the ICF e-learning tools with the KCF (Korean version) manual and curriculum model. This study analyzed the collected literature related to ICF education by research participants, education/training methods, and purposes. Based on the ICF curriculum model and ICF manual, this study made suggestions on the information that needs to be added to the e-learning tools. The study found that the interest in ICF education relating to health and disability has been increasing since 2008. Participants in the ICF education system have gradually diversified with both online-offline and theoretical-practical education being provided together. In the comparison of ICF materials with KCF, it was observed that information, including ethical guidelines in using ICF, guidelines for appropriate data collection, documentation, and coding methods would need to be added to the e-learning tools. Based on these results, effective development plans for ICF education and future research were presented.

노인 만성질환자의 민간의료보험 가입 여부가 의료이용에 미치는 영향

최근 우리나라는 급속한 인구 고령화로 노년기가 길어지고 있으며, 대다수의 노인들이 만성질환 보유자로 지속적인 관리와 치료가 필요하다는 점에서 높은 의료수요를 유발하는 주요 원인이 되고 있다. 이러한 점에서 우리나라 국민들 중 상당수는 공적보험인 건강보험으로 보장되지 못하는 의료비 부담을 줄이기 위해 민간의료보험에 가입하고 있다. 이에 본 연구는 고령화연구패널 6차(2015-2016), 7차(2017-2018)년도의 자료를 이용하여 민간의료보험 가입 여부가 노인 만성질환자의 의료서비스 이용에 어떠한 영향을 미치는지 연령별 차이를 고려하여 분석하였다. 연구대상자는 고령화연구패널 6차, 7차 조사에 참가한 만성질환을 보유한 65~74세 노인 총 847명을 분석대상으로 하였다. 분석 결과, 만성질환 보유 65~74세 노인은 민간의료보험 가입자가 미가입자에 비해 입원 및 외래 의료이용을 더 많이 하는 것으로 나타났다. 본 연구를 통해 만성질환 보유 노인을 위한 정책적 개선에 기여하고자 한다. Recently, the Korean population has been rapidly aging and a growing number of elderly people have come to have chronic diseases that require consistent treatment. In this respect, an increasing number of Koreans rely on private medical insurance programs to reduce the burden of medical expenses that are not covered by public health insurance. In this study, we analyzed how private medical insurance affects medical service of elderly people having chronic disease by using materials from the 6th (2015-2016) and 7th (2017-2018) research panels on aging of the elderly considering age differences. The study participants were 847 elderly people aged 65 to 74 with chronic disease who had participated in the 6th and 7th research panels on aging. The analysis showed that elderly people with chronic illness had more hospitalization and outpatient use of private health insurance than non-members. Through this research, we will try to contribute to policy improvement for the elderly with chronic disease.

ICF기반 한국 장애출현율 산출에 대한 연구: Model Disability Survey를 중심으로

본 연구는 한국판 ICF를 기반으로 한국의 장애출현율과 관련된 통계작성을 시도하는 것이 목적이다. 이를 통해 장애인권리협약, 지속가능한 개발목표 수행을 위한 장애모니터링 가능성을 제시하고, 장애데이터 마련을 통해 향후 보건 의료 및 장애인재활과 관련된 기초자료를 마련하고자 한다. 본 연구는 서울시 인구분포와 유사한 성북구에 거주하는 321명에게 개별면접방식을 실시하였다. 장애출현율 조사를 위해 17개 도메인, 48개 문항으로 구성되어 있는 Model Disability Survey(MDS)를 활용하였다. 라쉬분석을 이용해 MDS가 제시하는 기준에 따라 계산한 결과 장애출현율은 20.9%로 나타났다. 이는 2017년 장애인실태조사결과가 제시한 5.39%에 비해 약 4배 정도 높은 수치이다. MDS 기준에 따라 나타난 장애정도는 ‘장애없음’ 28.0%, ‘경증’ 24.0%, ‘중등도’ 27.1%, ‘중증’ 20.9%로 나타났다. 이러한 결과는 현재 국내에서 실시하고 있는 조사결과와 수행능력에 초점을 둔 MDS의 장애출현율 측정방식과 국제적으로 통용되는 도구를 사용한 실제 출현율이 다르게 나타남을 보여준다. 연구결과를 기반으로 향후 국내 장애인복지정책을 개발하고 잠재적 수요자들의 복지욕구를 파악하기 위해 필요한 방안에 대한 논의와 제언을 하였다. The purpose of this study is to examine the prevalence of disabilities in South Korea based on the Korean version of the International Classification of Functioning, Disability and Health (ICF). This study presents the possibility for disability monitoring in accordance with the UN CRPD or Sustainable Development Goals (SDGs), and prepares basic disability data related to future health care and rehabilitation. A total of 321 persons living in Seongbuk-gu, which has a population distribution similar to Seoul, participated in a face-to-face survey exploring disability prevalence through individual interviews. To investigate the prevalence rate, we used the Model Disability Survey (MDS), which consists of 48 questions in 17 domains. According to results from Rasch analysis, the disability prevalence rate was 20.9%, which is about four times higher than a 2017 survey on disability (5.39%). Based on criteria under the MDS, the breakdown was as follows: No disability, 28.0%; Mild disability, 24.0%; Moderate disability, 27.1%; and Severe disability, 20.9%. These results imply that disability prevalence and disparity rates differed between the MDS and the 2017 disability survey in South Korea. Based on the results of this study, discussion and suggestions to identify the welfare needs of potential consumers, and ways to develop welfare policies for people with disabilities, are provided.
