R&D performance in Korea has been grown remarkably owing to the government support policy. However, the revenue of technology transfer to private enterprise by public research institutions in Korea is insignificant compared to the foreign developed countries. Korean government has reviewed with multilateral on the efficient technology commercialization, and then legislated research institute company law in 2006 as a support policy to maximize technology commercialization success rate. A lot of research institute companies have been operating these days. However, the business performances of research institute companies are still not so prominent. In order to suggest methods to increase the success rate of research institute companies, the research on the research institute company is needed. This paper is to suggest right directions to activate technology commercialization in public research institutions. And it suggests methods to lead the research institute companies as prominent companies based on the case study of the four representative technology investment institutes and four research institute companies.

Wolter 금속판을 이용한 견봉 쇄골 관절 탈구의 수술적 치료 결과 및 술중 오구 쇄골 인대 봉합술의 임상적 유의성

목적: Wolter plate를 이용한 견봉 쇄골 관절 완전 탈구의 술후추시 관찰을 통하여 술식의 임상적 결과를 분석하고 오구 쇄골 인대를 봉합한 환자군과 봉합하지 않은 환자군 사이의 임상결과를 비교해오구 쇄골 인대 봉합술의 필요성을 평가하고자 한다. 대상 및 방법: 2003년 1월부터 2005년 9월까지 두 의료기관에서 Rockwood 분류 III형 이상의 견봉쇄골 관절 탈구로 Wolter plate를 이용하여 수술한 환자 중 6개월 이상 추적관찰이 이루어진 23명을 대상으로 하였다. 평가는 술후 6개월째 Constant-Murley의 평가 지표로 관절 기능을 점수화하고 금속판 제거 후 부하 방사선 사진에서 견봉 쇄골 관절의 정복 상태 및 오구 쇄골 간격을 관찰하였으며 환자를 오구 쇄골 인대를 봉합한 10명과 봉합하지 않은 13명으로 재구분하여 상기 임상결과를 비교하였다. 합병증이 나타난 환자에 대해서는 합병증의 종류를 분석하였다. 결과: 견봉 쇄골 관절 탈구에서 Wolter plate를 이용한 내고정 시행후 6개월째 합병증이 발생한 3예를 제외한 20예의 경우에서 Constant-Murley 평가상 `양호` 이상의 결과를 보였으며 오구 쇄골인대의 봉합군과 비봉합군의 점수는 환자군별로 우수 7예/6예, 양호 2예/5예, 보통 및 불량 1예/2예였으며 술후 6개월째 추시 방사선 사진상 원위 쇄골의 전위는 봉합군에서 술전 평균 194%에서 술후 9%로, 비봉합군에서는 술전 188%에서 28%로 감소하였다. 오구 쇄골 인대 봉합군과 비봉합군의 Constant-Murley score는 통계적 차이가 없었으며 (p=0.072) 추시 부하방사선 사진상 쇄골 원위부의 전위도 통계적 유의성이 없었다 (p=0.067). 결론: Wolter 금속판을 이용한 견봉 쇄골 관절 탈구의 고정술 후 단기 추시에서 비교적 만족스러운 임상 결과를 경험하였으며 오구 쇄골 인대를 봉합한 군과 봉합하지 않은 군 사이의 임상적, 방사선학적 차이에는 통계적 유의성이 없었다. Purpose: To evaluate the clinical result of the Wolter plate fixation for the acromioclavicular joint dislocation and the necessity of coracoclavicular ligament repair with the operation. Materials and Methods: Twenty three patients operated between January 2003 to September 2005 with over 6 months of follow-up period were studied. The Constant-Murley scoring system was administered on 6 months postoperatively and stress films were taken for the surveillance of acromioclavicular joint and coracoclavicular distance after plate removal. All patients were classified into two groups in that coracoclavicular ligament was repaired (10 cases) or not (13 cases) and the clinical indices described above were compared. Results: With the Wolter plate fixation for the acromioclavicular joint dislocations, 20 cases of Constant-Murley scores were more than `good` except complicated 3 cases. The scores of the repaired group were 7 cases of excellent, 2 cases of good and 1 case of moderate to poor, and that of not-repaired group were 6 cases, 5 cases and 2 cases respectively. With mean coracoclavicular interspace on x-ray at postoperative 6 months, repaired group showed residual 9% of displacement from initial 194% but not-repaired group showed 28% from initial 188%. There`s no statistically significant difference in clinical scores between two group (p=0.072) and neither was residual coracoclavicular interspace displacement (p=0.067). Conclusion: Short term follow-up of Wolter plate fixation for the acromioclavicular dislocation showed acceptable clinical results and there was no statistically significant difference between two groups of repaired coracoclavicular ligaments and not repaired.
