Objectives : The purpose of this study was to suggest a new method for measurement of children's growth development Ultrasound transonic velocity of inferior radiocarpal joint has some advantages that it is harmless to human body and apply a new analysis algorithm, it can be used for the diagnosis of bone age. Methods: This clinical study have been carried out with the 117 case(male 68, female 49) of the children aged 5 to 16 years old who visited in Department of Pediatrics, ${\bigcirc}{\bigcirc}$ university Oriental Hospital. Bone maturity is measured by Ultrasound transonic velocity and X-ray image of growth plate in inferior radiocarpal joint. This study were designed to investigate the relationship of the bone age by Ultrasound transonic velocity and X-ray image. Result & Conclusion : The maturity of growth plate of inferior radiocarpal joint was correlated with age, height, weight, BMI in this study(P<0.001). The Ultrasound transonic velocity of inferior radiocarpal joint are sufficient diagnostic worth as an index to predict bone maturity.

Clinical Assessment of Efficacy, Safety and Usefulness of Traditional Herbal Medicine (THM) on Atopic Dermatitis

목적 : 본 연구를 통하여 아토피 피부염 환아의 한약치료 후 임상 효능, 안전성을 평가하고, 나아가 효능과 안전성을 종합적으로 평가함으로써 아토피 피부염에 대한 한약치료의 유용성을 평가하고자 한다. 방법 : 경희대학교 한의과대학 부속한방병원 한방소아과에 내원한 아토피 피부염 환아 39명의 한약치료 전후의 의무기록을 검토하였다. 아토피피부염 치료에 대한 한약의 효능을 평가하기 위하여 치료 후 아토피 피부염의 개선정도에 대한 부모의 평가를 VAS를 사용하여 분석하였다. 아토피 피부염에 패한 한약치료 후 안정성의 평가를 위하여 치료 전후 혈중 AST, ALT, BUN, creatinine 변화를 검토하여 한약치료의 간신독성 여부에 대해 조사하였고, 보호자에 대한 설문조사를 통하여 한약치료 후 발생한 부작용 및 불편한 증상의 종류 및 정도를 평가하였다. 나아가 한약의 효능 및 안정성을 종합적으로 평가하여 아토피 피부염에 대한 한약 치료의 유용성을 분석하였다. 결과 및 결론 : 환아 39명 중 27명 (69%)의 부모가 한약치료가 아토피 피부염의 개선에 효과가 있다고 보고하였다. 안정성의 평가에 있어서, 39명의 환자에게서 한약 치료 후 간장과 신장에 미치는 독성이 발견되지 않았다. 2명의 환아가 일시적이고 경미한 복부 불쾌감을 호소하였으나, 한약투여와의 관련성은 관찰되지 않았다. 1명의 환아에서 경미한 복통이 발견되었으나, 의학적 치료 없이 증상은 소실되었으며, 치료 과정의 중단 없이 계획되었던 모든 치료 과정을 종료하였다. 치료 후 상기 환아는 아토피 피부염의 증상 개선 뿐 아니라 변비증상의 완화, 만성적 어지러움의 개선 등 제반 신체 조건의 호전을 보고 하였다. 임상적 효능과 안전성을 종합적으로 고려할 때, 환아 39명 중 27명 (69%)의 사례에서 한약치료가 아토피 피부염의 유용한 치료수단으로 평가되었다. 이를 근거로 한약치료가 아토피 피부염에 대하여 안전성을 가진 유용한 치료방법이라 사료되며, 향후 임상적 효능과 안전성을 객관적으로 평가할 수 있는 대규모의 임상연구가 필요하리라 사료된다.

근전도상 불량한 예후를 보였던 Ramsay Hunt Syndrome 환아 1례에 대한 임상적 고찰

Objective : To evaluate the effect of Oriental medical treatment on a patient with Ramsay Hunt syndrome with poor prognosis on EMG. Methods : We applied various methods of Oriental Medical treatment including Acupuncture, Electroacupuncture, Herb medicine, Vesiculation therapy using Mylabris and Crontonis Fructus, Massage, Self-excercise of face muscles. Results : 1. Herb medicine was applied on the basis of Differentiation of Syndromes(辨證): We tried Herb medicine to improve the function of Digestive System. The patient's stool condition, subjective sensation related with abdominal pain and general condition improved. 2. We used the Vesiculation therapy using Mylabris which was used traditionally to treat Facial Palsy. We powdered Mylabris and mixed it with Crontonis Fructus to make a paste. We put it on Ye-poong(?風), Nae-gwan(內關) at left side. It induced vesicles at the applied area, but it disappeared in 48 hours without any scar. 3. We applied acupuncture, electroacupunture, massage and self-excercise of face muscles. On gross scale assessment, there was some improvement. And assessment with regional scale also showed changes. The patient and the guardian were satisfied with the result. Conclusion : We experienced a case of Ramsay Hunt syndrome with poor prognosis on EMG. She also showed poor response to conservative treatment. Through various modalities of Oriental medical treatment, we attained some clinical improvement.

Objective The purpose of this study was to investigate health and life style of pediatric outpatients who visited the oriental hospital. Methods The study was composed of 363 students from elementary school, middle school and high school who visited the pediatrics department in $\bigcirc\;\bigcirc$ university oriental medicine hospital between 2005 and 2006. Results 1. The group wasconsisted with 56% of male and 44% of female student and for the age distribution, 7.4% were elementary school students, 55.6% were middle school students, and 36.9% were high school students. 2. Usually the eldest child tends to visit hosipital more than the younger ones. 3. More than a half of those students' parent had University education or beyond that. 4. There were more students who had below the level of the height-weight curve than in the higher level. 5. The average study hours of the students were 3.67 hours. An average sleep hours was 6.18 hours. An average exercise hour was 1.16 hours and an average time for watching TV was 1.71 hours. 6. Many students had hard time to concentrate on the studying for a long time. 7. For the question about the reason why they got the poor grade, they answered because of the lower concentration, and they didn't put much effort on the studying. The most they concern about was their grade. 8. Most of the students who visited the clinic said they did not feel refreshed when they woke up in the morning, and, many of them said that they don't feel okay. 9. More than an half of the students wear glasses. 10. More than an half of the students answered that they often catch a cold when the weather changes a lot. 11. A lot of them had some digestive problems. 12. As they getting older, they said they often feel back pain and shoulder pain. 13. Many students felt irritated and got mad easily. Many of them felt bored about their study and stressed out because of their tests and the university entrance exams. 14. Most of the female students answered that they have irregular period or cramps. 15. 21% of them usually skip breakfast. 16. Many students enjoyed snacks. Most of them enjoyed snacks after school. As they go into higher grade, they would like to eat at night especially cookies, bread and fruits. Conclusions Further studies with larger sample size of students will be neededfor accurate results, and it would be better if we can compare the conditions of the students before treatments and after those.

저산소성 허혈성 뇌손상 환아의 치험 1례

Objective: There were few reports on the treatment of Hypoxic ischemic encephalopathy. We treated a hypoxic ischemic encephalopathic patient after accident with Oriental medical approach, and get a significant result. this treatment shows the possibility of healing Hypoxic ischemic encephalopathy, So we are reporting that case. Method : The acupuncture, herb medication. rehabilitation therapy was applied for treting patient's chife symptom(involuntary movement, dystonia, aphasia, dysuria, constipation) Results : 1. Generally patients with Hypoxic ischemic encephalopathy show language impairment and cognition disorder for several weeks to months. After coma stage, first they recover consciousness but have various degree of confusional mentality, visual agnosia, extrapyramidal stiffness and motor disturbance. 2. Consciousness loss in acute stage of Hypoxic ischemic encephalopathy can be considered as Mental Confusion due to Phlegm(Dammisimgyu) in veiw point of Oriental medicine. 3. After oriental medical treatment, patient's chief symptoms were improved. Conclusion: We treated a hypoxic ischemic encephalopathic patient with Oriental medical approach and patient's chief symptoms were improved. this treatment shows the possibility of healing Hypoxic ischemic encephalopathy.

인터넷 게임 중독으로 발생하는 신체증상과 중독성 사이의 상관성 연구

Objectives : As the side effect brought up by internet game addiction, many reports come out, but most of past articles are for middle or high school students. There is seldom a poisoning game treatise to a primary school student, if there is treatise, it is real condition that treatise for lead pipe with psychological reasons is seldom discussion about physical symptoms. This research searches high grade elementary students' game use actual conditions and when divide to general user group, latent danger use group, high dangerous use group, according to serious illness degree of game poisoning, three groups and game poisoning happens, recognized about body symptoms that happen much. Methods : Target population of these questionnaire are fifth and sixth grade elementary student of Seoul city 2 school. Chose 263 elementary students, 144 men, 119 women who understand purpose of research and admit participation is 25 October, 2006 since 1 September, 2006 Results : As poisoning serious illness degree is high, there are many number of game times, much more boys are addicted to internet game than girs, insomnia is apt to happen frequently in game poisoning danger group, and correlation is high by megrim, shoulder ache, digestive trouble. Insomnia, symptoms that show next even if some symptoms show first because there is high correlation between megrim, shoulder ache, digestive trouble is high possibility to happen among four symptoms. Insomnia, megrim, shoulder ache, digestive trouble that happen by internet game poisoning, is apt to increase by increasing addiction. Symptoms treatment can divide by internal treatment that treat insomnia, megrim, digestive trouble, external treatment that is typical symptoms of VDT syndrome shoulder ache, headache, fatigue persimmon. Root treatment is mind symptoms treatment. Game addiction treatment is relationship method 'seven emotions injury treatment' in oriental medicine. 'promote the normal flow of qi' and 'adjustment of suitable emotion' can talk as pivotal point of treatment.

심폐소생술후 발생한 저산소성 허혈성 뇌손상 환아(患兒) 치험 1례(例)

Objective : To evaluate the effect of Oriental Medical Treatment on a patient with Hypoxic Ischemic Encephalopathy followed by Cardiopulmonary Resuscitation Method : We applied various methodology of Oriental Medical Treatment including Acupuncture, Electroacupuncture, Physical treatment, Herbal Medicine, Moxibustion treatment and Western medication as well. Result: Herbal medicine was applied on the basis of the patient's history. We applied formular to remove phelgm as a pathogenic factor after Hypoxic Ischemic Encephalopathy. At the same time, considering the patient spent more than a month in ICU lacking appropriate nutrition, we used formuli on the basis of 'Deficiency of Spleen' focusing to vitalize the function of digestive system. As the condition of the patient changed, we also adapted formular accordingly. We prescribed Herbal medication to strengthen Yin and Yang equally as she got hospitalized for long time. Also we applied Acupuncture treatment and Moxibustion treatment to control Qi flow. The general condition of the patient got better with successful removal of Foley catheter and elevated Glasgow Coma scale. We used Electroacupuncture, Physical treatment and Western medication at the same to get maximized effect on relaxing the contracted muscle. According to the Modified Ashworth Scale (MAS), we have some changes in muscle spasticity but later, the effect was not that significant. Conclusion : We had a patient with Hypoxic Ischemic Encephalopathy followed by Cardiopulmonary Resuscitation. In the management of Hypoxic Ischemic Encephalopathy, Conservative treatments are the mainstream but there are not many alternatives. Therefore, We suggest that Oriental medical approach may contribute to the management of Hypoxic Ischemic Encephalopathy.

과체중 또는 비만인 소아청소년의 체중 감량을 위한 가미태음조위탕의 임상활용: 다기관, 후향적 차트 리뷰

Objectives The purpose of this study is to evaluate the effects of Gamitaeeumjowi-tang on the weight changes and adverse events in overweight or obese prediatric-adolescents. Methods A multicenter and retrospective chart review was performed on 425 overweight and obese pediatric-adolescents who took Gamitaeeumjowi-tang from May 2022 to November 2023. Outcomes were measured in terms of total weight loss and weight loss percentage, as well as the percentage of patients who lost at least 5% and 10% of their baseline weight. Patient-reported adverse events were collected and analyzed for frequency of occurrence, and whether they were serious enough to require discontinuation. Results A total of 47 patients were included, with a mean (±standard deviation) age of 15.00±1.90 years, weight of 85.59 ±19.24 kg, and body mass index of 30.81±4.98 kg/m2; 74.47% were female. The mean (±standard deviation) weight loss was 8.84±4.93 kg and the mean weight loss rate was 10.23±4.28%. The percentage of patients who lost at least 5% and 10% of their baseline weight was 95.74% and 54.20% respectively. No serious adverse events were reported. Conclusions Gamitaeeumjowi-tang was shown to help pediatric-adolescents who were overweight or obese to lose weight, and no serious adverse events were associated with discontinuation of the medication. Further well-designed clinical studies are recommended.
