정신장애인의 사회관계망 특성이 사회적응에 미치는 영향 = The Effects of Social Network Characteristics on the Mental Illness People's Social Adjustment

국문 초록 (Abstract)

본 연구는 정신장애인 사회관계망의 긍정적, 부정적 특성이 사회적응에 미치는 영향을 파악하고자 한 연구이다. 정신장애인이 지역사회 내에서 생활하도록 방향의 전환이 이루어지고 있는 정신보건 영역에 있어 ‘사회관계망’은 정신장애인의 치료와 재활을 위한 새로운 접근방법으로 많은 관심을 증가시키고 있다. 그러나 선행 연구 대부분은 사회관계망의 일부 기능인 사회적 지지가 정신장애인의 치료 및 재활과 사회적응을 증진시키는 긍정적인 요인임을 밟히고 있을 뿐 취약계층에 있어 심리적 디스트레스를 악화시키고 하나의 스트레스 원으로 작용할 수 있는 사회관계망의 부정적 기능에 대해서는 설명을 하고 있지 못하다.
따라서 본 연구는 사회적 지지와 같은 일부 특성만으로 사회관계망을 분석한 선행연구들의 한계점을 보완하고자 사회관계망의 긍정적 기능인 ‘사회적 지지’와 부정적 기능인 ‘사회적 갈등’ 모두를 포괄하여 살펴보았다. 구체적으로 사회적 지지와 사회적 갈등이 정신장애인의 사회적응에 미치는 영향을 각각 파악하였고 마지막으로 이 둘의 상대적인 영향력을 파악하였다.
이를 위해 서울, 경기 및 충남, 경남과 전북 지역에 소재한 사회복귀시설. 정신보건센터, 낮병원 등의 19개 정신장애인 재활기관에 설문을 우편 발송하였고 현재 해당 기관을 이용하고 있거나 과거 이용하였던 정신장애인을 대상으로 조사한 190부를 최종분석에 활용하였다.
연구결과를 요약하면 다음과 같다.
첫째, 사회관계망의 긍정적 기능인 사회적 지지가 정신장애인의 사회적응에 미치는 영향을 검증한 결과, 사회적 지지가 높을수록 사회적응 정도가 높게 나타났다. 구체적으로 사회적 지지가 사회적응의 하위영역 각각에 미치는 영향을 분석한 결과 사회관계 및 여가선용, 핵가족 내의 역할, 확대가족 내의 역할, 직업상 역할, 자아감 및 만족도의 5가지 영역에 유의한 정적 영향을 미치는 것으로 나타났다. 즉 정신장애인이 그를 둘러싼 사회관계망 구성원으로부터 사회적 지지와 같은 긍정적 형태의 반응을 받는 경우 핵가족을 비롯한 확대가족 내에서 가족구성원으로서 역할을 잘 수행할 수 있게 되며, 사회관계와 여가선용과 같은 사회적 역할도 잘 수행할 수 있게 됨을 알 수 있다. 이와 같은 가족 내에서와 사회생활 모두의 역할수행에 긍정적인 영향을 받음으로서 자아감과 삶에 대한 만족도 향상에도 도움을 받을 수 있음을 확인하였다.
둘째, 사회관계망의 부정적 기능인 사회적 갈등이 정신장애인의 사회적응에 미치는 영향을 검증한 결과, 사회적 갈등이 높을수록 사회적응 정도가 낮게 나타났다. 구체적으로 사회적 갈등이 사회적응의 하위영역 각각에 미치는 영향을 분석한 결과 핵가족 내의 역할과 확대가족 내의 역할에 유의하게 부적 영향을 미치는 것으로 나타났다. 핵가족 및 확대가족 구성원으로서 역할은 정신장애인들이 비교적 잘 수행하는 역할임에도 불구하고 사회관계망 구성원으로부터 부정적 형태의 반응을 받는 경우 어려움을 겪는 다는 것을 알 수 있다. 이는 정신장애인의 대체능력 및 문제해결능력이 취약하다는 것을 확인할 수 있는 결과이기도 하다. 따라서 정신장애인을 둘러싼 여러 사회관계망의 부정적 기능에 주의를 기울일 필요가 있으며 부정적 기능의 감소 및 긍정적인 방향으로의 전환 가능성에 실천적 노력이 필요함을 알 수 있다.
셋째, 사회관계망의 긍정적, 부정적 기능이 사회적응에 미치는 상대적인 영향력을 살펴본 결과 사회관계망의 부정적 기능인 사회적 갈등보다는 긍정적인 기능인 사회적 지지가 정신장애인의 사회적응에 더 큰 영향력을 갖는 것으로 밝혀졌다.
결론적으로, 본 연구는 정신장애인 사회관계망의 긍정적, 부정적 기능 특성에 대한 실증적 조사를 수행하여 그 동안 사회관계망의 긍정적 기능인 사회적 지지가 정신장애인의 사회적응에 영향을 미친다는 선행연구들의 결과를 재검증해 볼 수 있었다. 또한 아직 연구가 활발하게 진행되지 않았던 사회관계망의 부정적 기능이 사회적응에 미치는 영향을 분석하여 사회관계망의 부정적 기능이 긍정적 기능인 사회적 지지보다 사회적응에 상대적으로 미치는 영향력은 작지만 정신장애인에게 있어 일차적 보호제공자이며 지지자원인 가족 및 확대가족 태도의 중요성을 시사한다는 결론을 도출할 수 있었다.
본 연구는 사회관계망의 부정적인 측면을 검증하는데 있어 정신장애인이라는 새로운 대상으로 그 연구영역을 넓혀 조사하였다는 연구방법론상의 의의가 있다. 실천적으로는 정신장애인의 사회적응을 예측하는데 가족과 확대 가족의 태도가 중요한 요인임을 확인하였다데 의의가 있다. 즉 정신장애인에 대한 가족의 부정적인 태도가 감소되고 질병에 대한 이해를 높이고 정신장애인에 대한 보다 현실적인 기대를 가지고 지지적인 태도를 유지할 수 있도록 가족을 대상으로 한 교육 및 상담의 확대가 필요함을 알 수 있다.

다국어 초록 (Multilingual Abstract)

The purpose of this study is to recognize what the positive and negative social network characteristics of the mental illness people influences on the social adjustment. In the field of the Public Mental Health which has been changed into the way for the mental illness people to live in the community, there have been many interests in the 'Social network' as a new approaching method for the therapy and rehabilitation for the mental illness people. Nevertheless, most of preceding studies just clarify that the social support, a function of the Social Network, is the positive factor which promotes the therapy, rehabilitation and social adjustment for the mental illness people but those studies don't explain at all for the negative function which might make the psychological stress of delicate levels deteriorate and be able to act as the one of stressful reasons.
Hence, this study examined both Social Support and Social Conflict which was the positive and negative functions of the Social Network inclusively to complement the limitations of preceding studies which analyzed the Social Network with only particular characteristics such as the social support. Concretely, this study understood each effect what the social support and conflict influenced on the social adjustment of the mental illness people and finally understood the relative influence of both functions.
Data for this analysis were collected from 19 mentally handicapped rehabilitation organization and totally 190 data were use to the final analysis.
The summary for the result of study is as follows.
First, the social support which is the positive function of the social network influenced on the social adjustment for the mental illness people, the higher social support resulted in the higher social adjustment. And as a result of that this study examined concretely about what each social support influenced on the subordinate field, it did the significant influence on five category, that is, the social and leisure time role, the role in a nuclear and extended family, the role for work, the sense of self and the satisfaction. In other words, it is recognizable that if the mental illness people got some positive types of response like the social support from the social network members surrounded them, they could perform their roles faithfully as the member of extended family including nuclear one as well as the social and leisure time roles. Finally as they got some positive influence from both their family and society, they could get a help to improve the senses of own selves and satisfactions with their lives.
Second, the social conflict which was the negative function of the social network influence on the social adjustment for the mental illness people, the higher social conflict resulted in the lower social adjustment. In addition, as a result of hat this study examined concretely about what each social conflict influenced on the subordinate field, it did the significant negative influence on the role in a nuclear and extended family. In other words, it is recognizable that if the mental illness people got some negative types of response from the social network members, they might be in trouble although they perform their roles faithfully as the member of a extended family including nuclear one. So, it could be the recognizable result that their ability to cope with situations and solve problems of the mental illness people is quite low. As the following of that, we need to pay attention to the negative function of several social networks surrounded the mental illness people and try to practice about reducing the negative function and turning ability to the positive ways.
And finally, the positive and negative functions of the social network influenced on the social adjustment, it was recognizable the social support which was the positive function influenced on the social adjustment of the mental illness people more than the social conflict which was the negative one.
In conclusion, this study examined the positive investigation about the positive and negative function characteristic of the social network for the mental illness people and could verify again for the preceding results that the social support which was the positive function of social network influenced on the social adjustment for the mental illness people. Furthermore, the negative function of the social network influenced on the social adjustment which had been progressed well and came to the conclusion that the very negative one suggested the importance of the first protecting providers and supporters, extended and nuclear family's attitude for the mentally handicapped though its influence was comparatively less than the social support which was the positive function to the social adjustment.
On the methodological significance, this study is enlarged the study field to a new subject, the mental illness people, to verify the negative aspects of the social network. And practically there is significance that the attitude of a nuclear and extended family is the essential factor to predict the social adjustment for the mental illness people. So to speak, we try to not only reduce the family's negative attitude for the mental illness people and elevate to understand the diseases correctly but also enlarge the education and counsel of the family to be able to keep an supportive attitude with more real expectation for the mental illness people.
