본 연구는 동기강화집단상담이 알코올중독자의 회복에 미치는 영향을 검증하고자 하였으며, 알코올중독의 회복변인으로는 변화동기와 하위요인(인식, 양가성, 실천), 단주자기효능감, 실존적 영적안녕감, 작업동맹과 하위요인(목표합의, 과제동의, 정서적 유대)을 선정하였다.
D시에 소재한 병원에서 ‘DSM-IV'의 진단기준에 의거하여 알코올남용 및 의존 진단을 받고 치료 중인 환자 120명을 대상으로 동기강화집단상담의 목적설명 후 자발적으로 집단상담에 참여하고자 한 28명을 실험집단으로, 집단상담에 참여하지 않고 설문지 작성에 동의한 24명을 통제집단으로 선정하고, 사전, 사후, 추수검사를 실시하였다. 본 연구가 진행되는 중에 퇴원을 하거나 중도 포기한 인원을 제외하고 실험집단 14명, 통제집단 9명을 최종 분석하였다. 또한 집단상담의 회기별 진행내용정리, 만족도조사, 추수인터뷰를 바탕으로 질적 분석을 실시하여 양적연구의 결과를 보완하고자 하였다.
본 연구의 동기강화집단상담이 알코올중독자의 회복에 미치는 영향에 대한 검증결과는 다음과 같다.
첫째, 동기강화집단상담에 참여한 실험집단은 통제집단에 비해 변화동기, 변화동기의 하위요인인 인식, 실천이 증가하는 것으로 나타났으나 통계적으로 유의미한 차이를 나타내지는 않았다. 하지만 변화동기의 하위요인인 양가성은 집단간에서 통계적으로 유의미한 차이가 있었다.
둘째, 동기강화집단상담에 참여한 실험집단은 통제집단에 비해 단주자기효능감이 증가하는 것으로 나타났으나 통계적으로 유의미한 차이를 나타내지는 않았다.
셋째, 동기강화집단상담에 참여한 실험집단은 통제집단에 비해 실존적 영적안녕감이 통계적으로 유의미하게 증가하는 것으로 나타났다. .
넷째, 동기강화집단상담에 참여한 실험집단은 통제집단에 비해 작업동맹, 작업동맹의 하위요인인 목표합의, 과제동의, 정서적 유대에서 통계적으로 유의미하게 증가하는 것으로 나타났다.
위의 연구결과는 동기강화집단상담에 참여한 실험집단의 변화동기, 인식, 실천, 단주자기효능감은 통계적으로 유의미한 차이를 보이지 않았지만 긍정적인 변화가 있었으며, 나아가 양가성, 실존적 영적안녕감, 작업동맹, 목표합의, 과제동의, 정서적 유대는 통계적으로 유의미한 차이를 보였다. 이는 동기강화집단상담이 알코올중독자의 회복에 긍정의 영향을 미쳤음을 시사한다.
본 연구에서는 동기강화상담의 정신을 바탕으로 집단상담에 참여하는 내담자에게 스스로 자신의 문제를 깨닫고, 그 문제를 포함하여 지금까지 표현하지 못했던 마음속의 많은 감정들을 표현할 수 있도록 하였다. 또한 변화에 대한 구체적인 계획과 미래에 대한 비전을 세움으로써 변화에 대한 동기와 효능감을 증가시키고자 하였다. 집단상담이 진행되는 동안 내담자들의 반응과 집단상담 종결된 후 실시된 만족도조사 및 추수인터뷰를 통해서 내담자들의 회복여정에 긍정적인 변화가 시작되고 있음을 확인 할 수 있었다. 더욱이 내담에게 시작된 긍정적인 변화가 외부에서 강제로 넣어준 것이 아니라 내담자 스스로가 고민하고 결정하고 실천하여 이루어 낸 것으로 큰 의미가 있었다.
지금 이 순간에도 어디선가 알코올중독으로부터 벗어나기 위해 노력하고 있는 많은 내담자와 가족 모두가 회복할 수 있기를 기도합니다.

This study is intended to verify the impact of motivational group therapy on the recovery of alcoholics. And as an indication of recovery from alcoholism, the following factors were selected: Motivation for change and its sub-factors(awareness, ambivalence, and practice), alcohol abstinence self-efficacy, existential sense of spiritual well-being, therapeutic relationship and its sub-factors(target agreements, assignment agreements, emotional ties).
In a hospital located in city ‘D’, the purpose of the motivational group therapy was explained to 120 patients who have been diagnosed and receiving treatment for alcohol abuse and dependence according to the diagnostic criteria of 'DSM-IV’. Amongst them, 28 patients volunteered to become the experimental group and 24 patients who agreed to complete the questionnaires but not participate in the group therapy were the control group. Pre, post and follow-up examinations were conducted to both groups. Excluding the patients who either dropped out of the study or was discharged from the hospital, the final analysis of the research was conducted with 14 patients from the experimental group and 9 patients from the control group. Furthermore, for a quantitative research analysis, progress information by sessions, satisfaction surveys, and follow-up interviews of the group therapy have been conducted.
The research results of verifying the impact of motivational group therapy on the recovery of alcoholics are as follows.
First, there is improvement in change motivation, and its sub-factors of awareness and practice between the experimental group and the control group of the motivational group therapy, but the differences does not indicate a statistically significant. However, ambivalence, a sub-factor of change motivation, does indicate a statistically significant improvement.
Second, alcohol abstinence self-efficacy improved in the experimental group that participated in the motivational group therapy when compared with the control group, but the difference does not indicate a statistically significant improvement.
Third, existential sense of spiritual well-being improved in the experimental group that participated in the motivational group therapy when compared with the control group and the difference is statistically significant.
Forth, therapeutic relationship and its sub-factors of target agreements, assignment agreements, and emotional ties improved and it was statistically significant in the experimental group that participated in the motivational group therapy when compared with the control group.
The research results above indicate the experimental group that participated in the motivational group therapy show positive change in motivation for change, awareness, practice, and alcohol abstinence self-efficacy even though it is non-statistically significant, and there was statistically significant improvement in factors of ambivalence, existential sense of spiritual well-being, therapeutic relationship, target agreements, assignment agreements, emotional ties. This suggests that motivational group therapy is a positive impact for the recovery of alcoholics.
This study was conducted so the participants of group therapy based on the spirit of motivational therapy would realize their own problems, and express many different emotions that they did not allow themselves to express concerning their problems and even beyond. Also, by establishing specific plans for change and a vision for the future, the hope was to increase motivation for change and the effectiveness of that change. Through response data, satisfaction surveys and follow-up interviews during the process of group therapy, the start of a positive change was confirmed in recovery patients. Moreover, the fact that the positive change that started during therapy was not an outside force, but rather a self-process of the client to decide for himself is a great significance.
My prayer goes out to the recovery of clients and their families who are struggling to escape alcoholism even this very moment.
