    Radiotherapy plays a crucial role in curative management of patients with inoperable cases of lung cancer. However, therapeutic outcomes of radiotherapy are often not fully satisfactory and radioresistance of cancer cells is considered to be a major factor that prevents successful treatment. To understand the mechanism of radioresistance in lung cancer, it has been investigated for the potential involvement of PIM1-induced signaling pathway in radioresistant lung cancer cells. It implied that PIM1-specific inhibitors might act as novel therapeutic drugs to regulate radiosensitivity in lung cancer. SGI-1776, ETP-45299, and tryptanthrin were selected as candidates of PIM1 inhibitors from structure-based drug selection. With irradiation, the selected compounds suppressed PIM1-mediated radioresistant signaling by significant inhibition of PIM1 kinase activity, subsequently leading to increasing apoptosis and sensitizing lung cancer cells to radiation. Moreover, an in vivo xenograft model confirmed the radiosensitizing effects of three PIM1 inhibitors as observed in vitro. In addition, Notch1 could also serve as a potential pharmacological target for regulating radiosensitivity since Notch1 plays a critical role in cell fate decisions by modulating cellular processes under radiation exposure. Radiation-induced Notch1 overexpression could be correlated with radioresistance through activation of survival signaling in lung cancer. Based on the screening results for pharmacological properties, rhamnetin and cirsiliol were identified as candidate Notch1-regulating radiosensitizers. Two compounds significantly suppressed radiation-induced Notch1 overexpression, subsequently resulting in induction of apoptosis and inhibition of epithelial-mesenchymal transition (EMT) in lung cancer cells. Such radiosensitizing and EMT inhibition properties of rhamnetin and cirsiliol were observed both in vitro cellular system and in vivo xenograft mice model. Taken together, both investigations revealed that PIM1-specific inhibitors and Notch1-regulating compounds can act as novel radiosensitizers that enhance the radiotherapeutic efficacy by inhibiting radiation-induced PIM1 activation or Notch1 overexpression associated with radioresistance in lung cancer.

    방사선치료는 수술이 어려운 비소세포폐암에 대한 주된 치료법이다. 하지만 암세포가 가지는 방사선저항성은 방사선치료의 효율을 떨어뜨리는 주요 원인으로 작용한다. 비소세포폐암에서 나타나는 방사선저항성의 메커니즘을 규명하기 위한 선행연구를 통해, PIM1, PRAS40, FOXO3a, 14-3-3, protein phosphatases들의 상호작용이 방사선저항성을 결정짓는 한 가지 원인이 될 수 있음을 알게 되었고, 이들 중 원암유전자로 알려진 PIM1은 비소세포폐암 세포에서 방사선저항성에 대한 조절인자로 작용함을 알 수 있었다. 이를 바탕으로, PIM1-특이적 저해제가 방사선민감제로 작용할 것이며, 비소세포폐암의 방사선저항성을 조절할 것이라는 가설 하에 본 연구를 수행하게 되었다. 구조 기반 약물 선별법을 통해 SGI-1776, ETP-45299, trythanthrin을 방사선민감제 후보군으로 선별하였다. 이 약물은 PIM1의 mRNA/단백질의 발현양과 PIM1의 세포 내 위치 이동에는 영향을 미치지 않으면서 방사선조사에 따라 증가된 PIM1의 인산화효소적 활성도만을 선택적으로 저해하였다. 인산화효소로써의 활성도가 저해된 PIM1은 하위 인자인 PRAS40를 인산화하지 못하게 되며, PRAS40가 인산화되지 않으면 14-3-3과 FOXO3a와 함께 삼중 결합체를 구성하지 못하게 되어, FOXO3a가 핵 내에 머무르게 된다. FOXO3a는 핵 내에서 전사인자로 작용하여, 세포사멸-유도 단백질인 Bim과 FasL의 발현양을 증가시켜 세포사멸을 유도하게 되고, 궁극적으로 방사선저항성을 보이는 비소세포폐암 세포들이 방사선에 민감해지는 결과를 나타내게 된다. 생체 내 이종이식 생쥐 모델을 통해서 이러한 PIM1 저해제들의 방사선민감성 효과를 재확인하였다. 약 30% 정도의 비소세포폐암 환자에게서 Notch1의 활성도가 증가되어 있었으며, 특히 악성으로 발전한 다양한 암 조직에서 Notch1의 과발현 또는 과활성화가 두드러지게 나타나서 약물 혹은 방사선치료의 효율을 저해하는 원인으로 작용한다고 한다. 이를 바탕으로 세포막 단백질로써 세포 성장을 조절하여 세포의 운명을 결정짓는 역할을 주로 하는 Notch1의 과발현이 방사선저항성 획득과 연관되어 있을 것이며, Notch1의 저해를 통해 방사성민감성을 나타낼 수 있을 것이라는 가설을 세웠다. 천연물 선별을 통해, rhamnetin과 cirsiliol이 Notch1을 조절할 수 있는 새로운 방사선민감제로 작용할 수 있음을 보였다. Rhamnetin과 cirsiliol은 각각 방사선 조사에 의해 증가된 Notch1의 발현을 억제하여 비소세포폐암 세포의 성장을 억제시켰다. 이 과정에서 acetylation을 통해 p53이 활성화되며, 활성화된 p53에 의해 종양-억제 microRNA로 알려진 miR-34a의 발현이 증가하였다. miR-34a는 직접적으로 Notch1의 3′-UTR에 결합하여 Notch1 mRNA 발현을 감소시켰다. 감소된 Notch1의 발현으로 인해 하위-조절인자인 NF-κB의 활성도 역시 감소하여 세포 성장과 관련된 단백질의 발현양이 감소되어, 세포사멸이 유도되어 방사선에 민감해지는 것을 확인하였다. 그리고 방사선 조사에 따라 활성화된 Notch1에 의해 발현양이 증가하던 EMT 관련 단백질들 또한 rhamnetin과 cirsiliol이 병용처리 되었을 경우 발현양이 감소되었다. 이러한 rhamnetin과 cirsiliol의 방사선민감성 및 EMT 저해 효과를 생체 내 이종이식 생쥐 모델을 통해서 재확인하였다. 결론적으로, PIM1-특이적 저해제인 SGI-1776, ETP-45299, tryptanthrin과 Notch1-조절 천연물인 rhamnetin과 cirsiliol은 방사선 조사 시 활성화되는 방사선저항성과 관련된 신호전달 기작을 저해하는 새로운 방사선민감제로 작용하여 방사선치료의 효율을 증가시켜 줄 수 있음을 본 연구를 통해 규명하게 되었다.
